dc.description.abstractการวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษา 1. ต้นทุนของผู้ให้บริการการรักษาพยาบาลต่อครั้งของบริการจำแนกตามประเภทของบริการ 2. ต้นทุนรวมเฉลี่ยของผู้ให้บริการต่อผู้ป่วยหนึ่งราย จำแนกตามประเภทผู้ป่วย Dengue fever, Dengue hemorrhagic fever ระดับ 1,2,3,4 3. ต้นทุนรวมเฉลี่ยของผู้รับบริการต่อการป่วย 1 ครั้ง 4. Cost Recovery Ratio ของโรงพยาบาลเด็ก 5. การประมาณต้นทุนในการรักษาโรคไข้เลือดออก พ.ศ. 2538-2540 ประชากรที่ศึกษา ได้ศึกษาจากผู้ป่วยในของโรงพยาบาลเด็กที่ได้รับการวินิจฉัยจากแพทย์ว่า เป็น Dengue fever, Dengue hemorrhagic fever ระดับ 1,2,3,4 เป็นจำนวน 70 ราย ตั้งแต่กลางเดือนตุลาคมถึงเดือนธันวาคม ข้อมูลได้จากเวชระเบียนผู้ป่วย และได้จากการสอบถามแพทย์ พยาบาล เจ้าพนักงานวิทยาศาสตร์ เจ้าหน้าที่แผนกรังสี เจ้าหน้าที่แผนกโภชนาการ ข้อมูลอีกส่วนหนึ่งได้จากการสอบถามญาติผู้ป่วย ผลการวิจัย 1.ต้นทุนของผู้ให้บริการการรักษาพยาบาลต่อครั้งของบริการ มีดังนี้ Complete blood count 50.00 บาท ปัสสาวะ 46.79 บาท อุจจาระ 54.83 บาท Clinical chemistries (ตับ) 101.84 บาท Clinical chemistries (ไต) 59.82 บาท Eletrolytes 65.65 บาท Blood gas 43.16 บาท Coagulogram 55.61 บาท Hematocrit 18.28 บาท Blood pressure 4.79 บาท การตรวจอุณหภูมิร่างกาย 6.00 บาท การตรวจชีพจร 4.67 บาท การตรวจของแพทย์ OPD และเมื่อแรกรับเป็นผู้ป่วยใน 108.13 บาท การตรวจเยี่ยมผู้ป่วยของแพทย์ 36.76 บาท การใช้รังสีวินิจฉัย 35.93 บาท การให้ 5℅ Dextrose ½ Normal Saline Solution และ 5℅ Dextrose Ringer Acetate ต่อ 500 cc 100.99 บาท Plasma substitute (Dextran 40) ต่อ 100 cc 145.54 บาท ยาลดไข้ 5.19 บาท tourniquet test 11.05 บาท อาหารวันละ 81.01 บาท ค่าเสื่อมราคาของห้องพักรักษาพยาบาลและค่าสาธารณูปโภค วันละ 26.91 บาท 2. ต้นทุนรวมเฉลี่ยของผู้ให้บริการต่อผู้ป่วย 1 ราย สำหรับผู้ป่วย Dengue fever เท่ากับ 1,485.97 บาท Dengue hemorrhagic fever ระดับ 1,2,3,4 เท่ากับ 1,341.86 บาท 1,592.92 บาท 2,212.88 บาท และ 1,618.09 บาท ตามลำดับ ถ้าไม่จำแนกผู้ป่วยตามประเภทของการเจ็บป่วย ต้นทุนรวมเฉลี่ยต่อผู้ป่วย 1 รายเท่ากับ 1,650.34 บาท 3. ต้นทุนรวมเฉลี่ยของผู้รับบริการต่อผู้ป่วย 1 ราย เท่ากับ 3,367.49 บาทผู้รับบริการได้จ่ายค่ายาและค่าเวชภัณฑ์รวมทั้งค่าที่พัก ค่าอาหารให้กับโรงพยาบาล 1,373.42 บาท 4. Cost Recovery Ratio ที่โรงพยาบาลได้รับเท่ากับ 0.83 เท่าของที่ได้จ่ายไป 5. การประมาณต้นทุนในการรักษา พ.ศ. 2538-2540 เท่ากับ 45,521,956.64 บาท 127,934,324.90 บาท 74,844,599.60 บาท เมื่อจำนวนผู้ป่วยที่พยากรณ์ได้ พ.ศ. 2538-2540 เท่ากับ 28,815 ราย 75,684 ราย 40,997 ราย ตามลำดับ และมีดัชนีราคาผู้บริโภคเพิ่มขึ้นร้อยละ 7 ในพ.ศ. 2538-2539 และร้อยละ 8 ในพ.ศ. 2540-
dc.description.abstractalternativeThe Objective of this research are to study 1. The cost of provider per unit of each type of health care. 2. The average total cost of provider per case : Dengue fever, Dengue hemorrhagic fever type 1,2,3,4 3. The average total cost of receiver per case. 4. The cost recovery ratio of the Children Hospital. 5. The estimated cost of curative care for Dengue fever and Dengue hemorrhagic fever in 1995-1997. The study population consists of 70 inpatients diagnosed as patients of Dengue fever, Dengue hemorrhagic fever type 1,2,3,4 at Children’s Hospital during the mid of October until the end December, 1994. The information from the patient’s files, hospital personnel and patient’s relatives are used. The finding of this research are 1. The cost of provider per unit of each type of health care is as follows : complete blood count 50.00 baht, urinalysis 46.97 baht, stool examination 54.83 baht, Clinical chemistries (liver function) 101.84 baht, Clinical chemistries (kidney) 59.82 baht, eletrolytes 65.65 baht, Blood gas 43.16 baht, coagulogram 55.61 baht, hematocrit 18.28 baht, Blood pressure 4.79 baht, temperature 6.00 baht, pulse 4.67 baht, physical examination at out patient department and first time inpatient department 108.13 bath, follow up by doctors 36.76 bath, x-ray 35.93 baht 5℅ Dextrose ½ Normal saline solution and 5℅ Dextrose Ringer Acetate per 500 cc 100.99 bath. plasma substilute 145.54 baht per 100 cc, paracetamol 5.19 baht, tourniquet test 11.05 baht, food 81.01 bath, depreciation for building and utilities 26.91 baht. 2. The average total cost of provider per case : dengue fever 1.485.97 baht, dengue hemorrhagic fever type 1,2,3 and 4 are 1,341.86 baht , 1592.92 baht, 2,212.88 baht, 1.618.09 baht respectively. The average total cost from all cases 1,650.34 baht. 3. The average total cost of receiver per case is 3,367.49 baht. The receiver has paid 1,373.42 baht for medicine, medical equipments, room and board. 4. The Cost Recovery Ratio for curative care at Children Hospital is 0.83 of the total expenditure. 5. The estimated cost of curative care in Thailand from 1995 to 1997 will be 45,521,956.64 baht, 127,934,324.90 baht, 74,844,599.30 baht when the number of patients predicted for 1995-1997 are 28,815 cases, 75,684 cases and 40,997 cases respectively and the consumer price indexes increases 7 percent in 1995-1996 and 8 percent in 1997.-
