| dc.description.abstract | การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการส่งเสริมพฤติกรรมการดูแลสำหรับผู้ดูแลเด็กโรคหืดวัย 1-5 ปีต่อระดับความรุนแรงของโรค กลุ่มตัวอย่างคือ เด็กโรคหืดวัย 1-5 ปีที่มาตรวจและรับการรักษาที่คลินิกโรคระบบทางเดินหายใจ แผนกกุมารเวชกรรม โรงพยาบาลชลบุรีและผู้ดูแล จำนวน 46 คู่ กลุ่มตัวอย่างได้รับการสุ่มเข้ากลุ่มควบคุม 24 คู่ และกลุ่มทดลอง 22 คู่ กลุ่มควบคุมได้รับการดูแลทางการพยาบาลตามปกติ ส่วนกลุ่มทดลองได้รับโปรแกรมการส่งเสริมพฤติกรรมการดูแลเด็กโรคหืดวัย 1-5 ปี เป็นเวลา 4 สัปดาห์ โปรแกรมถูกสร้างโดยใช้แนวคิดการดูแลตนเองที่จำเป็นเมื่อมีภาวะเบี่ยงเบนทางด้านสุขภาพของ Orem (2001) โดยได้รับการตรวจสอบและประเมินความเหมาะสมในการนำมาใช้จากผู้ทรงคุณวุฒิ 7 ท่าน โปรแกรมที่ให้ผู้ดูแลประกอบไปด้วย 1) การสร้างสัมพันธภาพระหว่างผู้ให้โปรแกรมและผู้ดูแลของเด็ก 2) การอธิบายถึงความสำคัญของพฤติกรรมการดูแลต่อระดับความรุนแรงของโรคหืด 3) การให้ความรู้ด้านต่างๆ เกี่ยวกับโรคหืด 4) การจัดสิ่งแวดล้อมสำหรับการดูแลเด็กด้วยตนเองของผู้ดูแลเก็บข้อมูลส่วนบุคคลและข้อมูลความรุนแรงของโรคหืด 2 ครั้ง คือ ก่อนและหลังสิ้นสุดโปรแกรม 2 สัปดาห์ โดยใช้แบบประเมินความรุนแรงของโรคหืดในเด็กวัย 1-5 ปี ซึ่งพัฒนาโดยผู้วิจัย มีค่าดัชนีความตรงตามเนื้อหาที่ .88 และค่าความเที่ยงของเครื่องมือที่ .84 ทำการวิเคราะห์ข้อมูลโดยสถิติเชิงพรรณนาเพื่อหาค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน และสถิติการวิเคราะห์ความแปรปรวนร่วม (ANCOVA) ผลการวิจัยสรุปได้ดังนี้ ระดับความรุนแรงของโรคในเด็กโรคหืดวัย 1-5 ปีในกลุ่มที่ผู้ดูแลได้รับโปรแกรมการส่งเสริมพฤติกรรมการดูแลต่ำกว่ากลุ่มที่ผู้ดูแลได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 | en_US |
| dc.description.abstractalternative | The purpose of this quasi-experimental research was to study the effect of childcare behavior promoting program for caregivers on severity of illness in 1-5 year old asthmatic children. Subjects of this study were 46 one to five years old asthmatic children and their caregivers at the pulmonary clinic of pediatrics department, Chonburi Hospital. Twenty-four pairs were randomly assigned to the control group and 22 pairs were in the experimental group. The control group received conventional nursing care while the experimental group received the four - week childcare behavior promoting program. This program was guided by the health-deviation self-care requisites of Orem (2001). It was verified by 7 experts. The program included relationship formation, explanation of the necessity of childcare behavior on severity of asthmatic illness, knowledge on asthma, and environmental management. Data were collected twice, before and 2 weeks after receiving the program, by the illness severity interview developed by the researcher. It’s content validity index was .88 and interrater reliability was .84. Data were analyzed by mean, standard deviation, and ANCOVA with pretest as the covariate, at the level of statistical significance of .05. It was found that the severity of illness in 1-5 year old asthmatic children after their caregivers received the program was lower than that of the group receiving conventional nursing care at the level of statistical significance of .05. | en_US |
