dc.description.abstract	ไลเคนพลานัสในช่องปากเป็นโรคอักเสบเรื้อรังชนิดหนึ่งที่พบได้บ่อย มีความเกี่ยวข้องกับความผิดปกติของภูมิคุ้มกันชนิดที เซลล์เป็นสื่อ เคยมีรายงานว่าทูเมอร์เนคโครซิสแฟกเตอร์-แอลฟา มีความเกี่ยวข้องกับการเกิดรอยโรคนี้ จุดประสงค์ของการศึกษานี้เพื่อที่จะหาความสัมพันธ์ระหว่างการแสดงออกของ ทูเมอร์เนคโครซิสแฟกเตอร์-แอลฟาในรอยโรคไลเคนพลานัสในช่องปากก่อนและหลังการรักษาด้วยยาฟลูโอซิโนโลน อะเซทโทไนด์ชนิดขี้ผึ้ง ความเข้มข้น 0.1% ผู้ป่วยไลเคนพลานัสในช่องปากจำนวน 18 ราย ได้รับการตัดชิ้นเนื้อก่อนและหลังการรักษาด้วยยาฟลูโอซิโนโลน อะเซทโทไนด์ชนิดขี้ผึ้งเป็นเวลา 4 สัปดาห์ ร่วมกับศึกษาเนื้อเยื่อปกติจำนวน 20 ราย ด้วยวิธีการเดียวกันโดยส่งตรวจด้วยวิธีการทางพยาธิวิทยาและอิมมูโนฮิสโตเคมีที่ย้อมด้วยแอนติบอดีต่อ ทูเมอร์เนคโครซิสแฟกเตอร์-แอลฟา ผลการศึกษาพบว่าผู้ป่วย 16 ใน 18 ราย (88.89 เปอร์เซ็นต์) แสดงปฏิกิริยาที่ให้ผลบวกต่อทูเมอร์เนคโครซิสแฟกเตอร์-แอลฟา ส่วนใหญ่พบในโมโนนิวเคลียร์เซลล์ มี 10 ราย (55.56%) ที่พบการแสดงออกของ ทูเมอร์เนคโครซิสแฟกเตอร์แอลฟา ในเคอราติโนซัยต์ จำนวนโมโนนิวเคลียร์เซลล์ที่ให้ผลบวกต่อทูเมอร์เนคโครซิสแฟกเตอร์-แอลฟาก่อนการรักษาด้วยยาฟลูโอซิโนโลน อะเซทโทไนด์ชนิดขี้ผึ้ง ความเข้มข้น 0.1% สูงกว่าภายหลังการรักษาที่ระดับนัยสำคัญทางสถิติ (p=0.000) และในเนื้อเยื่อปกติ (p=0.000) ยิ่งไปกว่านั้นจำนวนโมโนนิวเคลียร์เซลล์ ภายหลังการรักษาด้วยยาฟลูโอซิโนโลน อะเซทโทไนด์ชนิดขี้ผึ้ง ความเข้มข้น 0.1% ต่ำกว่าก่อนรักษาที่ระดับนัยสำคัญ (p=0.000) แสดงว่าทูเมอร์เนคโครซิสแฟกเตอร์-แอลฟาอาจมีความเกี่ยวข้องกับการเกิดพยาธิสภาพในรอยโรคไลเคนพลานัสในช่องปากในผู้ป่วยไทยและยาฟลูโอซิโนโลน อะเซทโทไนด์ชนิดขี้ผึ้งความเข้มข้น 0.1% มีผลทำให้มีการแสดงออกของทูเมอร์เนคโครซิสแฟกเตอร์-แอลฟาลดลง	en
dc.description.abstractalternative	Oral lichen planus (OLP) is a common chronic inflammatory disease involving T-cells mediated immunity. TNF-{alpha} has been reported to be involved in the disease process. The purpose of this study was to clarify the relationship between the expression of TNF-{alpha} and OLP before and after treatment with fluocinolone acetonide in orabase (FAO) 0.1%. Eighteen Thai patients with atrophic or erosive OLP were recruited. Biopsy specimens were taken before and 4 weeks after treatment with FAO 0.1% and sent for histopathologic examination where they were immunohistochemically stained with antibody to TNF-{alpha}. Twenty normal mucosa specimens were identically processed. Sixteen out of 18 cases (88.89%.) of OLP exhibited positive staining for TNF-{alpha}. Most of the TNF-{alpha} was observed in the mononuclear cells. Ten cases (55.56%) of OLP demonstrated TNF-{alpha} expression in keratinocytes. The number of mononuclear cells positive for TNF-{alpha} before the treatment with FAO 0.1% in orabase was statistically significant higher than that after the treatment (p=0.000) and in the normal mucosa (p=0.000). Moreover, the number of mononuclear cells after treatment with FAO 0.1% in orabase was statistically significant lower than before the treatment (p=0.000). Our study demonstrated that TNF-{alpha} may be associated with the immunopathogenesis of OLP in Thai patients and FAO 0.1% had an effect on the reduction of TNF-{alpha} expression.	en
