Abstract: An approach was developed to examine the provider cost of bone marrow transplantation in hematologic disorders from the day of admission to the day of discharge and follow up six months. In this study costing principles have been reviewed and the retrospective determination of direct provider cost for bone marrow transplantation was used to the individual medical record. The costs were calculated per patient related to individual treatment of each patient with due to volume and unit cost. The cost data of 56 patients was examined from the relevant departments in Chulalongkorn Hospital. The direct provider cost per case treated was averaged to be Bt.645,535, cost per patient with intermediate effectiveness was Bt.821,590 and cost per patient with full effectiveness was Bt.1,063,234 in 1997's price. The costs were classified by inputs to be capital cost and recurrent cost There were 8.87% of total cost to be capital costs and the recurrent costs were 91.13%, which can be classified into 2 components : the personnel cost accounted for 19.16% and the material costs were 80.84%. In material costs, the highest category was drug cost, 63.27% and the second came the blood cost which was 10.13%. The important implication of this study was how the hospital would cope with the high cost of catastrophic illness among hematologic disorders. This study provided a basis for Social Security Office to consider the rate appropriate for reimbursement in bone marrow transplantation.
