การศึกษาการใช้โปรแกรมสุขภาพจิตศึกษาครอบครัว ต่อพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทในชุมชนและภาระการดูแลของผู้ดูแลผู้ป่วยจิตเภท โรงพยาบาลสมเด็จพระปิยมหาราชรมณียเขต จังหวัดกาญจนบุรี

Abstract

การศึกษาครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาผลของการใช้โปรแกรมสุขภาพจิตศึกษา ครอบครัวในผู้ป่วยจิตเภทต่อพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภท และภาระ การดูแลของผู้ดูแลก่อนและหลังการใช้โปรแกรมสุขภาพจิตศึกษาครอบครัว กลุ่มตัวอย่างคือผู้ป่วยจิตเภท จํานวน 20 คน และผู้ดูแลผู้ป่วยจิตเภท จํานวน 20 คน ได้จากการคัดเลือกแบบเจาะจงตามเกณฑ์คุณสมบัติจากผู้ป่วยจิตเภท ซึ่งมารับบริการที่คลินิกจิตเวช แผนกผู้ป่วยนอกและอยู่ในความ รับผิดชอบของโรงพยาบาลสมเด็จพระปิยมหาราชรมณียเขต จังหวัดกาญจนบุรีเครื่องมือที่ใช้ใน การศึกษาคือโปรแกรมสุขภาพจิตศึกษาสําหรับครอบครัว ซึ่งผ่านการตรวจความตรงเชิงเนื้อหาโดย ผู้ทรงคุณวุฒิจํานวน 3 คน เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลมี 2 ชุด คือแบบวัดภาระในการ ดูแลของผู้ดูแล และแบบวัดพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภท โดยเครื่องมือ ทั้ง 2 ชุดนี้มีค่าความเที่ยงสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ .91และ .86 ตามลําดับ วิเคราะห์ข้อมูลโดยใช้สถิติทดสอบที่ ( paired t-test) ผลการศึกษาที่สําคัญ สรุปได้ดังนี้ 1.พฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภท หลังการใช้โปรแกรม สุขภาพจิตศึกษาครอบครัวดีขึ้นกว่าก่อนการใช้โปรแกรมสุขภาพจิตศึกษาสําหรับครอบครัว อย่างมีนัยสําคัญทางสถิติที่ระดับ.05 (X = 64.25, S.D. = 3.02 และ X = 48.85, S.D. = 7.18 ตามลําดับ t = -12.07) 2.ภาระในการดูแลของผู้ดูแลหลังได้รับโปรแกรมสุขภาพจิตศึกษาครอบครัว ต่ํากว่าก่อน การได้รับโปรแกรมสุขภาพจิตศึกษาสําหรับครอบครัว อย่างมีนัยสําคัญทางสถิติที่ระดับ .05 (X = 2.91, S.D. = 9.84และ X = 3.61, S.D. = 1.66 ตามลําดับ, t = 13.60)

The purposes of this study were to compare medication adherence behaviors and burden among family caregivers of schizophrenic patients in community before and after received family psychoeducation program. Study samples composed of 20 schizophrenic patients and 20 caregivers of those patients who met the inclusion criteria. They were recruited from patients attending psychiatric clinic, out patient department, Somdejprapiyamaharachrommaneeyake hospital, Kanchanaburi Province. Study instrument was the family psychoeducation program which was validated for content validity by 3 experts. Data collection instruments were the caregiver burden scale and medication adherence scale. The Chronbach Alpha coefficient reliability of the two latter instruments were .91 and .86, respectively. Paired t-test was used in data analysis. The major results were as follows: 1. Medication adherence behaviors of patients with schizophrenia in community after using the family psychoeducation program was significantly higher than that before using such program, at the p.05 level (X = 64.25, S.D. = 3.02 and X = 48.85,S.D = 7.18, respectively; t= -12.07) 2. The burden among family caregivers of schizophrenic patients after using the family psychoeducation program was significantly lower than that before using such program, at the p.05 level (X = 2.91, S.D. = 9.84 and X = 3.61, S.D. = 11.66, respectively; t = 13.60).
