| Abstract: | การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อเปรียบเทียบอาการคลื่นไส้ อาเจียน ขย้อนของผู้ป่วยหลังผ่าตัดทางนรีเวชกรรม ในระยะหลังการผ่าตัดชั่วโมงที่ 4, 8 และ 24 ระหว่างกลุ่มทดลองกับกลุ่มควบคุม กลุ่มตัวอย่างคือผู้ป่วยที่รับการรักษาด้วยการผ่าตัดทางนรีเวชกรรม ในวิทยาลัยแพทยศาสตร์กรุงเทพมหานครและวชิรพยาบาล จำนวน 60 คน แบ่งเป็นกลุ่มควบคุม 30 คน และกลุ่มทดลอง 30 คน กลุ่มควบคุมได้รับการพยาบาลตามปกติ ในขณะที่กลุ่มทดลองได้รับโปรแกรมการให้ข้อมูลก่อนผ่าตัดร่วมกับการกดจุด ซึ่งประกอบด้วย 4 ขั้นตอน คือ 1) การประเมินประสบการณ์การมีอาการ 2) การให้ข้อมูล 3) การบำบัดแบบผสมผสานด้วยการกดจุดตามแนวคิดการแพทย์แผนจีน 4) การประเมินผล เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล คือ แบบวัดอาการคลื่นไส้ อาเจียน ขย้อนของ Rhodes (1999) มีค่าความเที่ยงจากการคำนวณค่าสัมประสิทธิ์แอลฟาของคอนบราค เท่ากับ .89 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงบรรยาย สถิติทดสอบค่าที และการทดสอบเอฟ ผลการวิจัยสรุปได้ดังนี้ 1. อาการคลื่นไส้ อาเจียน ขย้อนของกลุ่มทดลองลดลงตามลำดับ ตามช่วงเวลาหลังการผ่าตัดอย่างมีนัยสำคัญทางสถิติ โดยพบว่า กลุ่มทดลองมีอาการมากที่สุดในชั่วโมงที่ 4 และมีอาการน้อยที่สุดในชั่วโมงที่ 24 ส่วนกลุ่มควบคุม มีอาการคลื่นไส้ อาเจียน ขย้อนเพิ่มขึ้นตามลำดับ ตามช่วงเวลาหลังการผ่าตัด 2. 2. อาการคลื่นไส้ อาเจียน ขย้อน ของกลุ่มทดลองน้อยกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติใน ทุกช่วงเวลาหลังการผ่าตัด คือชั่วโมงที่ 4 (X-bar ทดลอง =1.67, X-bar ควบคุม = 4.00, t = 2.78, p<.05) ชั่วโมงที่ 8 (X-bar ทดลอง = 1.27, X-bar ควบคุม = 6.07, t = 4.58, p<.05) และชั่วโมงที่ 24 (X-bar ทดลอง = 0.17, X-bar ควบคุม = 6.10, t = 6.40, p<.05) |
| Other Abstract: | This quasi experimental, aimed to examine effects of the preoperative information giving with acupressure program on postoperative nausea, vomiting, and retching between the experimental group, and the control group at the 4, 8, and 24 hours after surgeries. The sample of 60 female treated with gynecologic surgery was recruited from BMA Medical College, and Vajira Hospital. The control group received routine nursing care while the experimental group received the preoperative information giving with acupressure program that consisted of four steps; assessment of symptoms experience, information giving, acupressure on P6 acupoint, and evaluation of the program. The collecting instruments used for data were the Rhodes Index of Nausea, Vomiting and Retching (INVR) (Rhodes, 1999).The reliability of this instrument with Conbach’s alpha coefficient was .89. Data were analyzed by descriptive statistics, t-test, and F-test. The results were as follows : 1. The nausea, vomiting, and retching in experimental group at 4, 8, and 24 hours after surgeries were significantly decreased respectively (p<.05), by highest score at 4 hours and minimum score at 24 hours, while control group were increased, respectively. 2. The nausea, vomiting, and retching at 4, 8, and 24 hours after surgeries of the experi- mental group were significantly lower than those of the control group (4 hours: X-bar experiment = 1.67, X-bar control = 4.00, t = 2.78, p<.05; 8 hours: X-bar experiment = 1.27, X-bar control = 6.07, t = 4.58, p<.05 and 24 hours: X-bar experiment = 0.17, X-bar control = 6.10, t = 6.40, p<.05). |
