| Abstract: | ศึกษาความหมายและประสบการณ์พัฒนาคุณภาพการพยาบาลผู้ป่วยระยะสุดท้ายของหัวหน้าหอผู้ป่วย โดยใช้ระเบียบวิธีวิจัยเชิงคุณภาพแบบบรรยายความ (Qualitative descriptive) ผู้ให้ข้อมูลคือหัวหน้าหอผู้ป่วย ที่มีประสบการณ์การพัฒนาคุณภาพการพยาบาลผู้ป่วยระยะสุดท้าย ที่ปฏิบัติงานในโรงพยาบาลมหาวิทยาลัยของรัฐแห่งหนึ่ง เป็นผู้ที่มีผลงานหรือโครงการพัฒนาคุณภาพการพยาบาลผู้ป่วยระยะสุดท้ายและเป็นที่ยอมรับจากหน่วยงานอื่น จำนวน 14 ราย เก็บรวบรวมข้อมูลโดยการสัมภาษณ์แบบเจาะลึก และการบันทึกเทป นำข้อมูลที่ได้มาถอดความแบบคำต่อคำ และวิเคราะห์เนื้อหาตามวิธีการของโคไลซี่ ผลการศึกษาพบว่า หัวหน้าหอผู้ป่วยให้ความหมายของการพัฒนาคุณภาพการพยาบาลผู้ป่วยระยะสุดท้ายไว้ 3 ความหมาย คือ 1) การดูแลตามความต้องการของผู้ป่วยและญาติ 2) การดูแลตามมาตรฐานเชิงวิชาชีพ และ 3) การดูแลเพื่อให้ผู้ป่วยทุกข์ทรมานน้อยที่สุด ส่วนประสบการณ์การพัฒนาคุณภาพการพยาบาลผู้ป่วยระยะสุดท้ายของหัวหน้าหอผู้ป่วย แบ่งเป็น 3 ประเด็นหลักคือ ประเด็นหลักที่ 1 ระยะเริ่มต้นของการพัฒนาคุณภาพการพยาบาลผู้ป่วยระยะสุดท้าย แบ่งเป็น 2 ประเด็นย่อย คือ 1) เริ่มจากนโยบายของโรงพยาบาลและมีคนขับเคลื่อน และ 2) เริ่มจากจิตสำนึกส่วนตัว: อยากช่วยให้ตายอย่างสงบและงดงาม ประเด็นหลักที่ 2 การพัฒนาคุณภาพการพยาบาลผู้ป่วยระยะสุดท้ายของหัวหน้าหอผู้ป่วยคือ 1) การดูแลด้วยความเข้าใจ โดย (1.1) เตรียมสถานที่ อุปกรณ์และบุคลากร (1.2) เพิ่มสมรรถนะของบุคลากร และ (1.3) จัดทำคู่มือการดูแลผู้ป่วยระยะสุดท้าย 2) การดูแลด้วยความเอาใจใส่ โดย (2.1) จัดกิจกรรมที่สอดคล้องกับปัญหาและความต้องการของผู้ป่วย และ (2.2) ปรับปรุงบริการอย่างต่อเนื่อง ประเด็นหลักที่ 3 ผลของการพัฒนาคุณภาพการพยาบาลผู้ป่วยระยะสุดท้าย ดังนี้ 1) ผลต่อผู้ป่วยระยะสุดท้ายและครอบครัว แบ่งเป็น 3 ประเด็น คือ (1.1) สามารถเผชิญกับสถานการณ์คุกคามได้ดีขึ้น (1.2) ได้ตายอย่างสงบ (1.3) ครอบครัวลดความโศกเศร้า 2) ผลต่อบุคลากร แบ่งเป็น 3 ประเด็น คือ (2.1) สุขใจกับผลการพัฒนา (2.2) ได้แลกเปลี่ยนเรียนรู้ และ (2.3) ได้รับการยอมรับจากทีมสหสาขาวิชาชีพ 3) ผลต่อหน่วยงานแบ่งเป็น 2 ประเด็น คือ (3.1) การมีส่วนร่วมในการปรับระบบงานให้ดีขึ้น และ (3.2) สร้างเครือข่ายการเรียนรู้ จากผลการศึกษาครั้งนี้ทำให้เข้าใจเกี่ยวกับ ความหมายและประสบการณ์การพัฒนาคุณภาพการพยาบาลผู้ป่วยระยะสุดท้ายของหัวหน้าหอผู้ป่วย โดยผู้บริหารทางการพยาบาลสามารถนำข้อค้นพบที่ได้จากการศึกษาครั้งนี้ไปสร้างแรงจูงใจ และสนับสนุนหัวหน้าหอผู้ป่วยในการพัฒนาคุณภาพการพยาบาลผู้ป่วยระยะสุดท้ายได้อย่างมีคุณภาพต่อไป |
| Other Abstract: | To explore head nurses experiences of the development of nursing quality for end of life care. Fourteen head nurses who had experience of end of life care in a selected university hospital participated in the study. In-depth tape-recorded interviews were used to collect data. All interviews were transcribed verbatim and analyzed using the content analysis method of Colaizzi. The study findings are as follows: The experiences of head nurses regarding the improvement of nursing quality in end of life care reflect 3 areas of importance: 1) Care management must be based on patients’ and relatives’ needs 2) Care management must conform to professional standards 3) Care management must be based on the palliative needs of patients. In the development of quality end of life care in nursing two parameters must be considered: 1) the policies of the hospital and 2) personal conscience: the importance of enabling a peaceful end of life for the patient. As for the phases of development of quality nursing for end of life care. Head nurses must: 1) Manage the care with understanding (1.1) Prepare workplaces, equipment, and staff appropriately (1.2) Improve staff capability to deal with conditions of the job and (1.3) Develop a manual of care based on standards of practice 2) Pay attention to care management (2.1) Provide activities that are related to the patients’ problems and needs (2.2) Continue to improve the quality of care generally. Outcomes of the development of nursing quality for end of life care should provide the following benefits to patients and their families: (1.1) Enable them to cope better with threatening situations (1.2) Help them to go through the dying process peacefully (1.3) Reduce patients’ families grief. 2) The effects on professionals involved in end of life care should lead to: (2.1) Satisfaction with the outcomes of quality care development (2.2) Exchange of relevant knowledge (2.3) Acceptance from the multidisciplinary team working in the department. 3) Beneficial effects for the organizations concerned will (3.1) Contribute to the improvement and quality of care and (3.2) Establish a learning network. The results of this study give an understanding of the meaning of the experiences of head nurses regarding the development of quality nursing for end of life care. Nurse executives could use the findings of this study to motivate and support head nurses in the development of quality nursing care |
