| Abstract: | พัฒนาแบบประเมินสมรรถนะ และกำหนดเกณฑ์ตัดสินผลการประเมินสมรรถนะพยาบาลวิชาชีพศัลยศาสตร์หัวใจและทรวงอก โรงพยาบาลศิริราช กลุ่มตัวอย่างประกอบด้วย พยาบาลวิชาชีพศัลยศาสตร์หัวใจและทรวงอก โรงพยาบาลศิริราช จำนวน 83 คน ขั้นตอนการสร้างแบบประเมิน กลุ่มตัวอย่างคือ ผู้เชี่ยวชาญทางการพยาบาลศัลยศาสตร์หัวใจและทรวงอก โรงพยาบาลศิริราช จำนวน 7 คน ขั้นตอนการสร้างเกณฑ์ตัดสินผลการประเมินสมรรถนะ กลุ่มตัวอย่างในการนำแบบประเมินไปใช้คือ พยาบาลวิชาชีพศัลยศาสตร์หัวใจและทรวงอก โรงพยาบาลศิริราช จำนวน 79 คน แบ่งเป็นหัวหน้าหอผู้ป่วย 3 คน เป็นผู้ประเมินผู้ใต้บังคับบัญชาของตน 76 คน และกำหนดเกณฑ์ตัดสินผลการประเมินสมรรถนะ กลุ่มตัวอย่างคือ ผู้เชี่ยวชาญทางการพยาบาลศัลยศาสตร์หัวใจและทรวงอก โรงพยาบาลศิริราช จำนวน 6 คน แบบประเมินสมรรถนะที่พัฒนาขึ้นเป็นเกณฑ์แบบรูบริค (Rubric) 4 ระดับ ผลการศึกษาสรุปได้ดังนี้ 1. สมรรถนะพยาบาลวิชาชีพศัลยศาสตร์หัวใจและทรวงอก โรงพยาบาลศิริราช ประกอบด้วย 9 ด้าน รายการสมรรถนะ 28 ข้อ 1) สมรรถนะด้านการประเมินผู้ป่วยศัลยศาสตร์หัวใจและทรวงอก 2) สมรรถนะด้านการ ให้การพยาบาลผู้ป่วยศัลยศาสตร์หัวใจและทรวงอก 3) สมรรถนะด้านการพยาบาลผู้ป่วยที่ทำหัตถการเพื่อการตรวจวินิจฉัยและรักษาในผู้ป่วยศัลยศาสตร์หัวใจและทรวงอก 4) สมรรถนะด้านการบริหารยาในผู้ป่วยศัลยศาสตร์หัวใจและทรวงอก 5) สมรรถนะด้านการปฏิบัติการช่วยฟื้นคืนชีพขั้นสูงและการแก้ปัญหาฉุกเฉิน 6) สมรรถนะด้านการใช้และดูแลเครื่องมืออุปกรณ์การแพทย์ภายในหน่วยงาน 7) สมรรถนะด้านการจัดการความเสี่ยงและภาวะแทรกซ้อนทางศัลยศาสตร์หัวใจและทรวงอก 8) สมรรถนะด้านการเสริมสร้างพลังอำนาจ 9) สมรรถนะด้านการวิจัยและพัฒนาทางการพยาบาล 2. ผลการตรวจสอบคุณภาพของแบบประเมิน พบว่า ความตรงเขิงเนื้อหา มีค่าเท่ากับ .89 และความเที่ยงระหว่างผู้ประเมิน ครั้งที่ 1 และ 2 มีค่าเท่ากับ .84 และ .80 ตามลำดับ 3. ผลการประเมินสมรรถนะพยาบาลวิชาชีพศัลยศาสตร์หัวใจและทรวงอก โรงพยาบาลศิริราช เท่ากับ 2.78 ซึ่งอยู่ในระดับดี 4. เกณฑ์ตัดสินสมรรถนะพยาบาลวิชาชีพศัลยศาสตร์หัวใจและทรวงอก โรงพยาบาลศิริราช เท่ากับ 2.36 สมรรถนะพยาบาลวิชาชีพศัลยศาสตร์หัวใจและทรวงอก โรงพยาบาลศิริราช แสดงตามสมการ ดังนี้ = .115 (ด้านการประเมิน) + .12 (ด้านการให้การพยาบาล) + .115 (ด้านการพยาบาลผู้ป่วยที่ทำหัตถการ) + .124 (ด้านการบริหารยา) + .139 (ด้านการปฏิบัติการช่วยฟื้นคืนชีพขั้นสูงและการแก้ปัญหาฉุกเฉิน) + .1 (ด้านการใช้และดูแลเครื่องมืออุปกรณ์การแพทย์ภายในหน่วยงาน) + .12 (ด้านการจัดการความเสี่ยงและภาวะแทรกซ้อน) + .086 (ด้านการเสริมสร้างพลังอำนาจ) + .081 (ด้านการวิจัยและพัฒนาทางการพยาบาล) |
| Other Abstract: | To develop and to set standard of cardiothoracic surgical professional nurse competency scale for Siriraj Hospital. The samples from Siriraj Hospital were 83 cardiothoracic surgical nurses consisted of 7 cardiothoracic surgical nurse experts, who participated in constructing the competency assessment form. The process was set standard of competency scale, the samples were 79 cardiothoracic surgical nurses consisted of 3 head nurses who assessed 76 cardiothoracic surgical professional nurses competency by competency assessment form. The standard score was set by 6 cardiothoracic surgical nurse experts from Siriraj Hospital. The assessment form was rubric scoring, 4 point scale. The results were as follow: 1. The cardiothoracic surgical nurses’ competency scale for Siriraj Hospital of 9 competencies with item. The 9 competencies were 1) Cardiothoracic surgery patient’s assessment 2) Cardiothoracic surgery nursing care 3) The nursing procedure 4) Drug administration 5) Cardiopulmonary resuscitation 6) Use and maintenance of medical appliance 7) Risks management 8) The empowerment 9) Research and development for nursing 2. The quality of instrument were content validity index (CVI) was .89 and inter rater reliability were .84 and .80 respectively. 3. The arithmetic mean of competency assessed by head nurse was 2.78 which in the good level. 4. The standard score of cardiothoracic surgical nurses competency scale in Siriraj Hospital was 2.36. The cardiothoracic surgical nurses competency scale in Siriraj Hospital= .115 (Cardiothoracic surgery patient’s assessment) + .12 (Cardiothoracic surgery nursing care) + .115 (The procedure nursing) + .124 (Drug administration) + .139 (Cardiopulmonary resuscitation) + .1 (Use and maintenance of medical appliance) + .12 (Risks management) + .086 (The empowerment) + .081 (Research and development for nursing). |
