| Abstract: | การวิจัยกึ่งทดลองครั้งนี้ มีวัตถุประสงค์ คือ 1) เพื่อเปรียบเทียบพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทและภาระการดูแลของผู้ดูแล ก่อนและหลังได้รับการจัดการผู้ป่วยรายกรณี 2) เพื่อเปรียบเทียบพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทและภาระการดูแลของผู้ดูแล ระหว่างกลุ่มที่ได้รับการจัดการผู้ป่วยรายกรณีกับกลุ่มที่ได้รับการดูแลตามปกติ กลุ่มตัวอย่าง คือ ผู้ป่วยจิตเภทและผู้ดูแล กลุ่มละ 40 คน ที่อยู่ในความดูแลของโรงพยาบาล บางบัวทอง จัดเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 20 คน โดยการสุ่มและจับคู่ (matched pair) กลุ่มทดลองได้รับการดูแลตามรูปแบบการจัดการผู้ป่วยรายกรณี ส่วนกลุ่มควบคุมได้รับการดูแลตามปกติ เครื่องมือที่ใช้ในการวิจัย คือ คู่มือการจัดการผู้ป่วยรายกรณี แผนการดูแลผู้ป่วย แบบวัดพฤติกรรมการใช้ยาตามเกณฑ์การรักษา และแบบวัดภาระการดูแล ค่าความเที่ยงของแบบวัดพฤติกรรมการใช้ยาตามเกณฑ์การรักษาและแบบวัดภาระการดูแล มีค่าความเที่ยงสัมประสิทธิ์อัลฟาของครอนบาคเท่ากับ .92 และ .90 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติทดสอบที (t-test) ผลการวิจัยที่สำคัญ สรุปได้ดังนี้ 1. ค่าเฉลี่ยของคะแนนพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภท หลังได้รับการจัดการผู้ป่วยรายกรณีสูงกว่าก่อนได้รับการจัดการผู้ป่วยรายกรณี อย่างมีนัยสำคัญทางสถิติ ที่ระดับ .01 2. ค่าเฉลี่ยของคะแนนพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภท กลุ่มที่ได้รับการจัดการผู้ป่วยรายกรณีสูงกว่ากลุ่มที่ได้รับการดูแลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .01 3. ค่าเฉลี่ยของคะแนนภาระการดูแลของผู้ดูแลหลังได้รับการจัดการผู้ป่วยรายกรณีต่ำกว่าก่อนได้รับการจัดการผู้ป่วยรายกรณี อย่างมีนัยสำคัญทางสถิติที่ระดับ .01 4.ค่าเฉลี่ยของคะแนนภาระการดูแลของผู้ดูแล กลุ่มที่ได้รับการจัดการผู้ป่วยรายกรณีต่ำกว่ากลุ่มที่ได้รับการดูแลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .01 |
| Other Abstract: | The purposes of this quasi-experimental study were : 1) to compare the medication adherence behaviors of schizophrenic patients and burden of caregivers before and after receiving case management, and 2) to compare the medication adherence behaviors of schizophrenic patients and burden of caregivers who received case management, and those who received conventional nursing care. Research sample consisted of 40 schizophrenic patients, who received health service from Bangbuathong hospital, and 40 caregivers. Subjects were randomly assigned to the experimental group and the control group by matched pair technique. The experimental group received case management, whereas the control group received conventional nursing care. Research instruments were case management manual, clinical pathway, medication adherence behaviors scale, and caregiver burden scale. The reliability of the last two scales, using Chronbach’s Alpha coefficient were .92 and .90, respectively. The t-test was used in data analysis. Major findings were as follows: 1. Mean of medication adherence behaviors scores of schizophrenic patients who received case management after the experiment was significantly higher than that before the experiment, at .01 level. 2. Mean of medication adherence behaviors scores of schizophrenic patients who received case management was significantly higher than that of the group receiving conventional nursing care, at .01 level. 3. Mean of burden scores of caregivers who received case management after the experiment was significantly lower than that before the experiment, at .01 level. 4. Mean of burden scores of caregivers who received case management was significantly lower than that of the group receiving conventional nursing care, at .01 level. |
