| Abstract: | การศึกษาเพื่อหาค่าที่ปลอดภัยในการผ่าตัดทำ Posterior transpedicular screw fixation ในกระดูกสันหลังส่วนอกระดับ T1 ถึง T12 วัตถุประสงค์เพื่อหาขนาดและค่ามุมของ Thoracic pedicle เพื่อหาตำแหน่งทางเข้าและขนาดที่เหมาะสมของ Pedicular screw วิธีการ ศึกษาจากกระดูกสันหลังส่วนอก (Dry bone) ซึ่งมีความสมบูรณ์จำนวน 50 ตัวอย่าง แบ่งเป็นเพศชาย 32 ตัวอย่าง และเพศหญิง 18 ตัวอย่าง นำค่าที่ได้มาคำนวณหาค่าเฉลี่ย พิสัย และส่วนเบี่ยงเบนมาตรฐาน อุปกรณ์ที่ใช้ในการวัดคือ Vernier caliper และกล้องถ่ายรูปดิจิติอลใช้คู่กับ Programe Adobe Photoshop แบ่งหัวข้อในการวัดออกเป็น 1) ค่าความกว้างของ Thoracic pedicle 2) ค่าความสูงของ Thoracic pedicle 3) ค่ามุม Transverse 4 ) ค่ามุม Sagittal 5) ระยะทางจากตำแหน่งทางเข้าของ Pedicular screw ถึง Lateral border of superior articular process และ Upper border of transverse process 6) ค่าระยะทางจากตำแหน่งทางเข้าของ Pedicular screw ถึง ขอบด้านหน้าของกระดูกสันหลัง ผลจากการวัดพบว่า ค่าความกว้างของ Thoracic pedicle มีค่าเฉลี่ยต่ำสุด 4.82 มม. ที่ระดับ T5 ส่วนค่าเฉลี่ยสูงสุด 8.78 มม. ที่ระดับ T11 ค่าความสูงของ Thoracic Pedicle มีค่าเฉลี่ยต่ำสุดที่ระดับ T1 เท่ากับ 9.20 มม. ส่วนค่าเฉลี่ยสูงสุดอยู่ที่ระดับ T11 เท่ากับ 16.27 มม. ค่ามุม Transverse ที่ระดับ T1 มีค่าเฉลี่ย 36.23 องศา และลดลงไปเรื่อยๆจนถึงระดับ T12 ที่มีค่าเฉลี่ย -5.68 องศา ค่ามุม Sagittal มีค่าเฉลี่ยต่ำสุดที่ระดับ T9 เท่ากับ 89.71 องศา ส่วนค่าเฉลี่ยสูงสุดอยู่ที่ระดับ T1 เท่ากับ 110.73 องศา ค่าระยะทางจากจุดตำแหน่งทางเข้าของ Pedicular screw ถึง Lateral border of superior articular process มีค่าเฉลี่ยต่ำสุดที่ระดับ T7 เท่ากับ 4.68 มม. ส่วนค่าเฉลี่ยสูงสุดอยู่ที่ระดับ T1 เท่ากับ 7.62 มม. ค่าระยะทางจากจุดตำแหน่งทางเข้าของ Pedicular screw ถึง Upper border of transverse process มีค่าเฉลี่ยต่ำสุดที่ระดับ T3 เท่ากับ 3.22 มม. ส่วนค่าเฉลี่ยสูงสุดอยู่ที่ระดับ T12 เท่ากับ 4.71 มม. ค่าระยะทางจากจุดตำแหน่งทางเข้าของ Pedicular screw ถึง ขอบด้านหน้าของกระดูกสันหลัง มีค่าเฉลี่ยต่ำสุดที่ระดับ T1 เท่ากับ 30.37 มม. ส่วนค่าเฉลี่ยสูงสุดอยู่ที่ระดับ T9 เท่ากับ 41.21 มม. ผลการศึกษาสรุปได้ว่าความยาวของ Pedicular screw ตั้งแต่ระดับ T1-T12 มีค่าเฉลี่ยประมาณ 30-40 มม. การเลือกใช้ Screw ที่มีความยาวมากกว่า 40 มม. อาจทำให้เกิดอันตรายต่อโครงสร้างที่อยู่ใกล้เคียงกับกระดูกสันหลังส่วนอกได้ ส่วนตำแหน่งทางเข้าของ Pedicular screw ที่ระดับ T1-T2 อยู่ด้านในถัดจาก Lateral border of superior articular process ประมาณ 6-7 มม. และอยู่ด้านล่างต่ำกว่า Upper border of transverse process ประมาณ 3-4 มม. และที่ระดับ T3-T12 อยู่ด้านในถัดจาก Lateral border of superior articular process ประมาณ 4-5 มม.และอยู่ด้านล่างต่ำกว่า Upper border of transverse process ประมาณ 3-4 มม. |
| Other Abstract: | The study of the data for posterior transpedicular screw fixation in thoracic spine from T1 to T12 was performed in dried thoracic spine. The objective is to measure the linear and angular dimensions of thoracic pedicles to determine the entry point of the pedicular screw and screw size. Methods : Fifty dried thoracic spines from T1 to T12, 32 males and 18 females, were studied. The mean, range and standard deviation were calculated for all of the specimens. The measurements of the diameter and the angle of thoracic pedicles were done by using a vernier caliper, photographs taken by a digital camera, and adobe photoshop programe. The following parameters were measured : 1) pedicle width 2) pedicle height 3) transverse angle 4) sagittal angle 5) the distance from the entry point of the pedicular screw to the lateral border of superior articular process and the upper border of transverse process 6) length from the entry point to anterior cortex of the vertebral body. Results : The narrowest pedicle was 4.82 mm. at T5. The largest pedicle was 8.78 mm. at T11. The lowest pedicle was 9.20 mm. at T1. The highest pedicle was 16.27 mm. at T11. The transverse angles decrease gradually from 36.23ํ at T1 to -5.68ํ at T12. The smallest sagittal angle was 89.71ํ at T9. The largest sagittal angle was 110.73ํ at T1. The shortest distance from the entry point of the pedicular screw to the lateral border of superior articular process was 4.68 mm. at T7 and the longest distance was 7.62 mm. at T1. The distance from the entry point to the upper border of transverse process was between 3.22 mm. and 4.71 mm. The shortest length from the entry point to the anterior cortex of vertebral body was 30.37 mm. at T1. The longest length from the entry point to the anterior cortex of vertebral body was 41.21 mm. at T9. Conclusion : The average length of the pedicular screw for T1-T12 is 30-40 mm. Using a screw longer than 40 mm. may result in nearby structure injury. For T1-T2, the projection point of the pedicle axis is approximately 6-7 mm. medial to the lateral border of superior articular process and 3-4 mm. inferior to the upper border of transverse process. For T3-T12, the projection point of the pedicle axis is approximately 4-5 mm. medial to the lateral border of superior articular process and 3-4 mm. inferior to the upper border of transverse process. |
