dc.description.abstract	ศึกษาประสิทธิภาพในการลดระดับกรดยูริกของยาเบนซ์โบรมาโรน เปรียบเทียบกับยาอัลโลพูรีนอล และผลข้างเคียงหลังได้รับยาในผู้ป่วยที่มีภาวะกรดยูริกในเลือดสูง ที่มีการทำงานของไตบกพร่อง โดยศึกษาจากผู้ป่วยที่มีภาวะกรดยูริกในเลือดสูง ที่มีการทำงานของไตบกพร่องจำนวน 14 ราย ด้วยการเจาะเลือดวัดระดับกรดยูริกและเก็บปัสสาวะ 24 ชั่วโมงก่อนและหลังได้รับยาอัลโลพูรีนอล ที่ปรับขนาดตามค่าการทำงานของไตเป็นเวลา 1 เดือน จากนั้นหยุดยา 1 เดือน เจาะเลือดและเก็บปัสสาวะ 24 ชั่วโมงก่อนและหลังได้ยาเบนซ์โบรมาโรน ขนาด 100 มก./วันเป็นเวลา 1 เดือน จากนั้นนำผลต่างของระดับกรดยูริกที่ลดลงมาเปรียบเทียบกัน พร้อมทั้งเปรียบเทียบผลข้างเคียงทางคลินิก และผลการตรวจชีวเคมีทางห้องปฏิบัติการก่อนและหลังได้รับยาแต่ละชนิด ผลการศึกษาพบว่า ยาอัลโลพูรีนอลสามารถลดระดับกรดยูริกเฉลี่ยร้อยละ 27.41 โดยสามารถลดระดับกรดยูริกต่ำกว่า 6.4 มก./ดล. ร้อยละ 50 และยาเบนซ์โบรมาโรนสามารถลดระดับกรดยูริกเฉลี่ยร้อยละ 42.01 โดยสามารถลดระดับกรดยูริกต่ำกว่า 6.4 มก./ดล. ร้อยละ 78.57 เมื่อนำระดับกรดยูริกที่สามารถลดลงได้มาเปรียบเทียบระหว่างกลุ่ม พบว่ายาเบนซ์โบรมาโรนสามารถลดระดับกรดยูริกได้มากกว่า อย่างมีนัยสำคัญทางสถิติ (p = 0.02) และไม่พบผลข้างเคียงทางคลินิก หรือความผิดปกติจากผลตรวจเลือดทางห้องปฏิบัติการหลังได้ยาแต่ละชนิด สรุปได้ว่า ยาเบนซ์โบรมาโรนขนาด 100 มก./วัน สามารถลดระดับกรดยูริกในเลือดได้ดีกว่ายาอัลโลพูรีนอล ที่ปรับขนาดตามค่าการทำงานของไต ในผู้ป่วยที่มีระดับกรดยูริกสูงและมีการทำงานของไตบกพร่อง	en
dc.description.abstractalternative	To compare the efficacy in lowering serum uric acid level and side effect between benzbromarone and allopurinol in hyperuricemic patients with impaired renal function. Fourteen hyperuricemic patients with renal function impairment (creatinine clearance 20-70 ml/min) were included in this study. Blood samples and 24-hour urine were collected before and after taking allopurinol, of which the dose was adjusted to the creatinine clearance for one month. Then allopurinol was stopped and the patients remained on no uric lowering agent for one month. At the end of this wash-out period, blood sample and 24-hour urine were collected and benzbromarone 100 mg/day were started. The blood sample and 24-hour urine were recollected one month later. Serum uric acid leve and other blood chemistry were compared between the two drugs and before-after treatment in each group. The study shows that after taking allopurinol and benzbromarone for one moth, serum uric acid level decreased by 27.41% and 42.01%, respectively. Analysis between the two groups, benzbromarone is more efficacious than allopurinol in lowering serum uric acid level (p=0.02). No side effect was found in both groups. Conclusion : Benzbromarone 100 mg/day is superior to allopurinol at the dose adjusted to creatinine clearance in lowering serum uric acid level in hyperuricemic patients with impaired renal function.	en
