dc.description.abstractการวิจัยนี้เป็นการวิจัยเชิงพรรณาศึกษา ณ จุดเวลาใดเวลาหนึ่ง มีวัตถุประสงค์เพื่อศึกษาหาความชุกและหาระยะเวลาในการเกิดภาวะไขมันในเลือดสูง รวมทั้งลักษณะของผู้ป่วยตลอดจนความสามารถในการควบคุมระดับไขมันในเลือดหลังการปลูกถ่ายไต ผู้ป่วยที่เข้าร่วมการวิจัยครั้งนี้จำนวน 133 รายเป็นผู้ป่วยและติดตามการรักษาหลังการผ่าตัดปลูกถ่ายไต ณ โรงพยาบาลจุฬาลงกรณ์ ตั้งแต่วันที่ 1 มกราคม 2544 ถึงวันที่ 31 ธันวาคม 2545 ผู้ป่วยเหล่านี้เป็นผู้ป่วยนอกที่ได้รับการตรวจวัดระดับไขมันในเลือดร่วมกับตรวจติดตามผลการรักษาเป็นเวลาอย่างน้อย 3 เดือน การวิจัยนี้ติดตามประวัติการรักษาและระดับไขมันในเลือดก่อนการปลูกถ่ายไต 6 เดือนและหลังการปลูกถ่ายไตเป็นระยะในเดือนที่ 1, 3, 6, 9, 12 และต่อไปทุก 6 เดือน โดยการวิจัยครั้งนี้ มีระยะเวลาติดตามเฉลี่ยเท่ากับ 3.46±2.22 ปี นับจากวันที่ทำการผ่าตัดปลูกถ่ายไต ความชุกของภาวะไขมันในเลือดสูงในระยะเวลา 5 ปีแรกหลังปลูกถ่ายไตสูงกว่าความชุกของภาวะไขมันในเลือดสูงก่อนการปลูกถ่ายไต โดยมีความชุกของภาวะไขมันในเลือดสูงมากที่สุด (ร้อยละ 84) และพบความชุกของภาวะคอเลสเทอรอลรวมในเลือดสูง (ร้อยละ 80) มากกว่าความชุกภาวะไตรกลีเซอไรด์ในเลือดสูง (ร้อยละ 22) ที่ 1 เดือนหลังการปลูกถ่ายไต ค่าเฉลี่ยของระดับคอเลสเทอรอลรวมจะสูงที่สุดในเดือนที่ 1 (243.27±46.16 มก./ดล.) และลดลงในเดือนที่ 6 หลังการปลูกถ่ายไต นอกจากนี้ยังพบว่าค่าเฉลี่ยของระดับ high-density lipoprotein cholesterol จะสูงขึ้นตลอด 9 เดือนหลังการปลูกถ่ายไต ในขณะที่ค่าเฉลี่ยของระดับไตรกลีเซอไรด์และ low-density lipoprotein cholesterol ไม่เปลี่ยนแปลงไปจากระยะก่อนการปลูกถ่ายไต จากการวิเคราะห์ความสัมพันธ์ระหว่างระดับไขมันในเลือด 3 เดือน หลังการผ่าตัดปลูกถ่ายไตกับลักษณะต่างๆของผู้ป่วย พบว่าขนาดยา prednisolone มีความสัมพันธ์กับระดับคอเลสเทอรอลรวมและเมื่อแยกวิเคราะห์เฉพาะกลุ่มผู้ป่วยที่ได้รับ cyclosporine พบว่าขนาดยา prednisolone และระดับความเข้มข้นของยา cyclosporine ในเลือดที่ต่ำสุด (C0) มีความสัมพันธ์กับระดับคอเลสเทอรอลรวมและ C0 ยังสัมพันธ์กับระดับ low-density lipoprotein cholesterol อีกด้วย นอกจากนั้นยังพบว่าการได้รับยากลุ่ม β-blockers ทำให้ระดับไตรกลีเซอไรด์เพิ่มขึ้นได้ ในขณะที่ระดับ high-density lipoprotein cholesterol ในผู้หญิงจะสูงกว่าในผู้ชายและระดับ high-density lipoprotein cholesterol มีความสัมพันธ์กับอายุของผู้ป่วยที่มากขึ้น
dc.description.abstractalternativeThe purposes of this cross-sectional descriptive study were to estimate prevalence of hyperlipidemia, determine characteristics of patients with high serum lipid level, estimate onset and duration of hyperlipidemia and determine ability to control serum lipid level in renal transplant patients. One hundred and thirty-three renal transplant patients who visited renal transplant clinic at King Chulalongkorn Memorial hospital during 1 January 2001 through 31 December 2002 were included into this study. Eligible patients were those who followed at the clinic for at least 3 months and their serum lipid levels were available. Clinical data and lipid profiles during 6 months before renal transplantation were collected retrospectively. After operation, lipid profiles were obtained retrospectively and prospectively at 1, 3, 6, 9, 12 months post-transplantation and every 6 months after that. The mean of follow up period is 3.46±2.22 years. At the end of the first month after transplantation, the highest prevalence of hyperlipidemia (84%) was seen. Prevalence of hypercholesterolemia was higher than prevalence of hypertriglyceridemia (80 and 22%, respectively). Prevalence of hyperlipidemia remained higher than baseline for at least 5 years. Mean of total cholesterol levels significantly elevated at the first month alter transplantation (243.27±46.16 mg/dl) and declined 6 months later. Mean of high-density lipoprotein cholesterol significantly increased after surgery and remained above baseline for 9 months. Serum triglyceride and low-density lipoprotein cholesterol were not significantly changed. We use data at 3 months post-transplantation to determine correlation between serum lipid levels and various factors. Prednisolone dose significantly correlated with serum total cholesterol level. Of patients who received cyclosporin, prednisolone dose and trough concentration of cyclosporin (C0) associated with total cholesterol level. C0 also associated with low-density lipoprotein level. β-blockers usage associated with increation of triglyceride level while increasing age and female sex associated with increased high-density lipoprotein cholesterol level after transplantation.
