| Abstract: | การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาคุณภาพการรักษาผู้ป่วยใน ภายใต้ระบบกองทุนเงินทดแทนโดยใช้รูปแบบการศึกษาเป็นแบบเชิงพรรณนา ณ จุดเวลาหนึ่ง กลุ่มตัวอย่างประกอบด้วยเวชระเบียนจำนวน 272 ฉบับ เก็บข้อมูลโดยใช้การสุ่มใน 4 เขตพื้นที่ประกันสังคมที่เลือก ตามสัดส่วนของรายงานผลการดำเนินการ กองทุนเงินทดแทนปี 2549 ใช้ระยะเวลาเก็บในช่วง 1 กุมภาพันธ์ – 30 เมษายน 2551 และเวชระเบียนแต่ละฉบับในการศึกษานี้ถูกประเมินด้วยพยาบาลที่ผ่านการฝึกอบรม 2 คน เพื่อประเมินความสมบูรณ์ของเวชระเบียน เหตุการณ์ไม่พึงประสงค์ และคุณภาพการรักษาตามมาตรฐานโรงพยาบาลและบริการสุขภาพ ผลการศึกษาพบว่า ความสมบูรณ์ของเวชระเบียนผู้ป่วยในมีคะแนนเฉลี่ยเท่ากับ 2.60 (คะแนนเต็ม 5) การค้นหาเหตุการณ์ไม่พึงประสงค์ด้วย Global Trigger Tool พบว่ามีวันนอนรวมกันทั้งหมด 1599 วัน มีจำนวนสัญญาณบ่งชี้เหตุการณ์ไม่พึงประสงค์เท่ากับ 297 ครั้ง และพบเหตุการณ์ไม่พึงประสงค์ ทั้งหมด 20 ครั้ง โดยมีค่าระดับตัวชี้วัด เหตุการณ์ไม่พึงประสงค์ 12.5 ครั้งต่อ 1000 วันนอนและ เหตุการณ์ไม่พึงประสงค์ 7.4 ครั้งต่อ100 การเข้ารับการรักษาตัวในโรงพยาบาล และ ผลจากการใช้แบบประเมินที่สร้างขึ้นตามมาตรฐานโรงพยาบาลและบริการสุขภาพ ประเมินคุณภาพเวชระเบียนพบว่ามีระดับคะแนนเฉลี่ยรวมเท่ากับ 2.90 (คะแนนเต็ม 5) (SD= 0.67) และไม่พบความสัมพันธ์กับความสมบูณ์เวชระเบียน โดยสรุป จากการศึกษาในครั้งนี้พบว่า คุณภาพของการรักษาผู้ป่วยภายใต้ระบบกองทุนเงินทดแทนนั้นค่อนข้างเป็นที่ยอมรับได้ และแนะนำให้ใช้การตรวจสอบความสมบูรณ์ของเวชระเบียน การค้นหาเหตุการณ์ไม่พึงประสงค์ และการใช้แบบประเมินตามมาตรฐานโรงพยาบาลและบริการสุขภาพเป็นตัวชี้วัดในการประเมินคุณภาพการรักษาพยาบาลในระบบกองทุนเงินทดแทน |
| Other Abstract: | The purpose of this cross-sectional descriptive study was to explore the quality of inpatient care under The Workmen Compensation Fund. A sample group of 272 inpatient medical records were randomly selected from four area-based Social Security Offices according to proportions of the injury incidences reported in The 2006 Workmen Compensation Fund Report. Data were collected between February 1st – April 30th 2008. Each record was reviewed by two trained nurses for completeness, adverse events and compliance with selected Hospital Accreditation (HA) standards for patient care. The records scored 2.60 out of 5 in completeness. Given the 1,599 inpatient days of the sample, some 297 positive triggers were identified. These resulted in 20 adverse events, or 12.5 events per 1,000 inpatient days, or 7.4 events per 100 admissions. With respect to HA compliance, the average score was 2.9 out of 5 (SD=0.67), and any relationship with the record completeness was not found. In conclusion, the quality of patient care under The Workmen Compensation Fund was quite acceptable. Nevertheless screening for medical record completeness, advese events and compliance with standard of care should be installed as quality measures in the system. |
