| Abstract: | บทนำ : โรคไมแอสทีเนีย กราวิส เป็นโรคระบบประสาทที่เกิดความผิดปกติของสารสื่อประสาทระหว่างเส้นประสาทและกล้ามเนื้อที่พบได้บ่อยที่สุด โดยผู้ป่วยในกลุ่มนี้อาจมีอาการผิดปกติที่กล้ามเนื้อตาเพียงอย่างเดียวหรือเป็นกับกล้ามเนื้อลายทั่วร่างกายจนถึงขั้นมีการหายใจล้มเหลวได้ ปัจจุบันยังไม่ผลการตรวจทางห้องปฏิบัติการใดที่สามารถพยากรณ์ได้ว่าผู้ป่วยรายใดจะมีโอกาสเกิดภาวะการหายใจล้มเหลว วัตถุประสงค์ : เพื่อศึกษาถึงความสัมพันธ์ของระดับ soluble C5b-9(C5b-9) กับระดับความรุนแรงของโรคไมแอสทีเนีย กราวิส แบบทั่วตัว(generalized myasthenia gravis GMG) วิธีการศึกษา : การศึกษานี้เป็นการวิจัยนำร่องเชิงวิเคราะห์ ณ จุดเวลาใดเวลาหนึ่ง โดยศึกษาในผู้ป่วยGMG ที่เข้ารับการรักษาแบบผู้ป่วยในที่โรงพยาบาลจุฬาลงกรณ์ โดยผู้ป่วยจะได้รับการประเมินความรุนแรงของโรควันละครั้งโดยใช้ quantified myasthenia gravis strength score (QMGSS) ร่วมกับการเจาะเลือดเพื่อวัดระดับ C5b-9 สองครั้งต่อสัปดาห์ในสองสัปดาห์แรก หลังจากนั้นจะเจาะเลือดเพื่อวัดระดับC5b-9สัปดาห์ละครั้งจนกว่าผู้ป่วยจะถูกจำหน่ายออกจากโรงพยาบาล ร่วมกับเก็บข้อมูลด้านประชากรศาสตร์และข้อมูลพื้นฐานเกี่ยวกับตัวโรคของผู้ป่วย หลังจากนั้นหาความสัมพันธ์ระหว่างความรุนแรงของโรคกับระดับC5b-9ในเลือดโดยการใช้สมการ generalized estimating equations(GEE)หาค่าcoefficient เพื่อดูว่าระดับC5b-9มีความสัมพันธ์กับค่าQMGSSหรือไม่ ผลการศึกษา : มีผู้ปวยทั้งหมด 9 ราย เป็นหญิง 7 ราย อายุเฉลี่ย 47.4 ปี ระยะเวลาที่เป็นโรคเฉลี่ย 9.8ปี ผู้ป่วยทุกรายมีผลบวกต่อacetyl choline receptor antibody(AchRAb) ค่าเฉลี่ยของระดับ C5b-9ในกลุ่มคนปกติเท่ากับ 92.45ng/ml ค่าเฉลี่ยของระดับC5b-9แรกรับของกลุ่มผู้ป่วยGMGเท่ากับ 254.8 ng/ml (p=0.001) ค่าเฉลี่ยQMGSSแรกรับเท่ากับ 22.8พบว่ามีผู้ป่วย3รายมีการติดเชื้อขณะเข้ารับการรักษาโดยพบว่าผู้ป่วยในกลุ่มนี้มีระดับการเปลี่ยนแปลงของC5b-9ไม่แตกต่างจากกลุ่มที่ไม่มีการติดเชื้อ ค่าcoefficient จาก generalized estimating equationsเท่ากับ-0.273(ค่า pเท่ากับ 0.829) พบว่า ระดับ C5b-9ไม่มีความสัมพันธ์กับความรุนแรงของโรคในผู้ป่วยGMG สรุป ระดับC5b-9แรกรับในผู้ป่วยGMGมีความแตกต่างจากกลุ่มคนปกติอย่างมีนัยสำคัญทางสถิติ(p=0.001) แต่ระดับ C5b-9ไม่มีความสัมพันธ์กับความรุนแรงของโรคในผู้ป่วยGMGตลอดช่วงเวลาที่เข้ารับการรักษา |
| Other Abstract: | Backgorund : Myasthenia gravis (MG) is the most common of neuromuscular junction disease. Wide range of clinical presentations and severity of disease, only ocular symptoms to respiratory failure. Nowadays we have no laboratory test for predict the outcome in myasthenic patients who progress to respiratory failure. Objective : To determine the relationship between serum soluble C5b-9 level and severity of generalized myasthenia gravis(GMG) Methods : A pilot cross-sectional analytic study was conducted. In-patients diagnosed with generalized myasthenia gravis were recruited. All of the patients were evaluated the severity of disease once a day with quantified myasthenia gravis strength score (QMGSS) and blood draw for C5b-9 level twice a week at first two week and once a week thereafter. Demographic data were shown as percent and mean. A generalized estimating equations (GEE) was conducted to determine the relationship between serum soluble C5b-9 level and severity of generalized myasthenia gravis. Results : A total of 9 patients were recruited in this study. Female 7 in 9 patients (77.7%). The mean age is 47.4 years.The mean duration of disease is 9.8 years. All of patients have positive result for acetylcholine receptor antibody(AchRAb).The mean baseline C5b-9 level of healthy group is 92.45ng/ml. The mean baseline C5b-9 level of GMG group is 254.89 ng/ml. The mean baseline QMGSS is 22.78. Coefficient value from GEE is -.273 (p-value 0.829)Three patients has current infection during admission. C5b-9 level is not correlate with infectious process. There has no relationship between serum soluble C5b-9 level and severity of generalized myasthenia gravis. Conclusions : There has no relationship between serum soluble C5b-9 level and severity of generalized myasthenia gravis and infection may be not interfere C5b-9 level. |
