dc.description.abstractความสำคัญและที่มาของปัญหา : PROMPT-QoL เป็นเครื่องวัดคุณภาพชีวิตด้านการรักษาโดยการใช้ยาที่พัฒนาขึ้นมาใหม่ แต่อย่างไรก็ตามยังไม่มีการศึกษาคุณสมบัติการวัดเชิงจิตวิทยาของ PROMPT-QoL ในกลุ่มประชากรขนาดใหญ่ วัตถุประสงค์ : ทดสอบคุณสมบัติเชิงจิตวิทยาของ PROMPT-QoL วิธีการวิจัย : ผู้ป่วยนอกโรคเรื้อรังที่รับการรักษา ณ โรงพยาบาลจุฬาลงกรณ์ระหว่างเดือนกรกฎาคม 2557 ถึง กุมภาพันธ์ 2558 จำนวน 400 คน ตอบแบบสอบถาม PROMPT-QoL, WHOQOL-BREF, EQ-5D-5L และ MTB เพื่อทดสอบความสามารถในการใช้ได้จริง ความเที่ยงเชิงความสอดคล้องภายใน ความเที่ยงของการทดสอบซ้ำ การวิเคราะห์องค์ประกอบเชิงยืนยัน ความตรงเทียบกับกลุ่มที่รู้ ความตรงเชิงลู่เข้า และความตรงตามเกณฑ์สัมพันธ์ ผลการวิจัย : เวลาเฉลี่ยในการตอบแบบสอบถาม คือ 11 นาที แบบสอบถาม PROMPT-QoL พบ ceiling effect สูงในมิติการได้รับผลกระทบจากการใช้ยาและอาการข้างเคียงของยาร้อยละ 24.1 และมิติความสัมพันธ์ทางด้านการรักษาร้อยละ 15.2 ด้านความเที่ยงพบว่า PROMPT-QoL มีค่า Cronbach’s alpha อยู่ระหว่าง 0.60 - 0.91 และมีค่า ICCs เป็น 0.58 – 0.80 ด้านความตรงพบว่า PROMPT-QoL มีมิติเป็นไปตามที่คาดไว้จากการวิเคราะห์องค์ประกอบเชิงยืนยัน และ PROMPT-QoL สามารถยืนยันความตรงเทียบกับกลุ่มที่รู้ในด้านอายุ ระดับการศึกษา จำนวนยาที่ใช้ การควบคุมโรค การเกิดอาการไม่พึงประสงค์จากการใช้ยา ทัศนคติในการใช้ยา และสิทธิการรักษา แต่ไม่มีความแตกต่างในด้านเพศ ส่วนความตรงเชิงลู่เข้าพบว่า ในทุกมิติของ PROMPT-QoL มีความสัมพันธ์เชิงบวกกับมิติคุณภาพชีวิตโดยรวมของการใช้ยา โดยมีค่าสหสัมพันธ์ระหว่าง 0.19 – 0.64 (p < 0.01) ความตรงตามเกณฑ์สัมพันธ์พบความสัมพันธ์ระหว่างคะแนนคุณภาพชีวิตด้านการใช้ยาของ PROMPT-QoL กับคะแนนคุณภาพชีวิตด้านสุขภาพของ WHOQOL-BREF และ EQ-5D-5L และคะแนนพฤติกรรมการใช้ยา MTB ส่วนใหญ่มีความสัมพันธ์ระดับต่ำถึงพอใช้ โดยมีค่าสหสัมพันธ์ระหว่าง 0.10 – 0.44 (p < 0.05) สรุปผลการวิจัย : แบบสอบถาม PROMPT-QoL มีคุณสมบัติการวัดเชิงจิตวิทยาทั้งในด้านความสามารถในการใช้ได้จริง ความเที่ยงและความตรงเพียงพอที่จะนำไปใช้ในผู้ป่วยโรคเรื้อรัง ณ โรงพยาบาลจุฬาลงกรณ์en_US
dc.description.abstractalternativeBackground: PROMPT-QoL is a new instrument for medication management of health-related quality of life (HRQoL); however, studies on its psychometric properties in a large population haven’t been done. Objectives: To evaluate psychometric properties of PROMPT-QoL. Methods: Four hundred chronic disease outpatients at King Chulalongkorn Memorial Hospital between July 2014 and February 2015 were surveyed. Psychometric properties of PROMPT-QoL were evaluated in terms of practicality (administration time and ceiling effect), reliability (internal consistency reliability and test-retest reliability) and validity (confirmatory factor analysis, known-groups validity, convergent validity and criterion validity). Results: The average administration time was 11 minutes. It was found a high ceiling effect in impacts of medicines and side-effects and therapeutic relationships with healthcare providers of 24.1% and 15.2%, respectively. Cronbach’s alpha coefficients were ranged from 0.60 - 0.91. The ICCs were 0.58 – 0.80. Confirmatory factor analysis supported construct validity of PROMPT-QoL. Known-groups validity tests were confirmed for the following factors: age, education level, number of taken medicines, disease control, experiencing adverse drug reactions, attitudes toward medication use and type of health insurance but not in gender. In convergent validity, there were positive correlations among the PROMPT-QoL scores and overall quality of life (Pearson’s correlation coefficients ranged = 0.19 – 0.64, p < 0.01). Most of the correlations between PROMPT-QoL and WHOQOL-BREF, EQ-5D-5L and MTB showed a little to fair correlations in criterion validity (Pearson’s correlation coefficients ranged = 0.10 – 0.44, p < 0.05). Conclusions: PROMPT-QoL has confirmed psychometric properties in terms of practicality, reliability and validity at King Chulalongkorn Memorial Hospital.en_US
