| Abstract: | การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อวิเคราะห์การมอบหมายงานในทีมพยาบาล โดยส่วนรวมและแต่ละลักษณะหอผู้ป่วย ในโรงพยาบาลศรีนครินทร์ โดยวิเคราะห์เชิงโครงสร้าง กระบวนการ และผลลัพธ์ และศึกษาความแตกต่างของการมอบหมายงานในทีมการพยาบาล ในแต่ละลักษณะหอผู้ป่วย และในเวรเช้า เวรบ่าย และเวรดึก ตัวอย่างประชากรคือ ทีมการพยาบาลในแต่ละเวร ที่ปฏิบัติในหอผู้ป่วย ในโรงพยาบาลศรีนครินทร์ จำนวน 214 ทีม ได้จาก 132 เวร ประกอบด้วย เวรเช้า เวรบ่าย และเวรดึก ประเภทละ 44 เวร ซึ่งปฏิบัติงานในหอผู้ป่วยจำนวน 22 หอผู้ป่วย สุ่มตัวอย่างประชากรโดยวิธีการสุ่มตัวอย่างแบบง่าย รวบรวมข้อมูลโดยใช้เครื่องมือ 6 ชุด สร้างโดยผู้วิจัยและเครื่องมือ 5 ชุด ผ่านกระบวนการทดสอบความตรงตามเนื้อหาและความเที่ยงภายในแล้ว คือ แบบสัมภาษณ์หัวหน้าทีมและสมาชิกทีม แบบสัมภาษณ์การปฏิบัติกิจกรรมพยาบาลโดยทีมการพยาบาล แบบตรวจร่างกายการมอบหมายงานและคู่มือสำหรับวิเคราะห์การมอบหมายงาน แบบตรวจรายการบันทึกทางการพยาบาลและคู่มือสำหรับวิเคราะห์ขั้นตอนของกระบวนการพยาบาล และแบบสำรวจความพึงพอใจในงานของบุคลากรพยาบาล ผลการวิจัยมีดังนี้คือ 1. ในโรงพยาบาลศรีนครินทร์ มีค่าเฉลี่ยของการมอบหมายงานในทีมการพยาบาล โดยส่วนรวม อยู่ในระดับปานกลาง ร้อยละ 66.38 และมีค่าเฉลี่ยของการมอบหมายงานในทีมการพยาบาลเชิงโครงสร้าง กระบวนการและผลลัพธ์ ตามลำดับ คือ ร้อยละ 43.86 อยู่ในระดับต่ำ, ร้อยละ 78.99 อยู่ในระดับดี, ร้อยละ 59.89 อยู่ในระดับปานกลาง สำหรับค่าเฉลี่ยของการมอบหมายงานในทีมการพยาบาล ในแต่ละลักษณะหอผู้ป่วย พบว่า หอผู้ป่วยอุบัติเหตุ-ระนะวิกฤต มีค่าเฉลี่ยโครงสร้างกระบวนการ และผลลัพธ์ สูงที่สุด โดยมีค่าเฉลี่ยตามลำดับ คือ ร้อยละ 55.28 อยู่ในระดับปานกลาง, ร้อยละ 85.31 อยู่ในระดับดีมาก, ร้อยละ 63.46 อยู่ในระดับปานกลาง ส่วนในหอผู้ป่วยจักษุ โสต ศอนาสิก ลาริงซ์ มีค่าเฉลี่ยของการมอบหมายงายในทีมการพยาบาลเชิงโครงสร้าง กระบวนการ และผลลัพธ์ต่ำที่สุด ซึ่งมีค่าเฉลี่ยตามลำดับคือ ร้อยละ 25.00 อยู่ในระดับต่ำมาก, ร้อยละ 64.54 อยู่ในระดับปานกลาง, ร้อยละ 54.19 อยู่ในระดับต่ำ 2. การมอบหมายงานในทีมการพยาบาล ในแต่ละลักษณะหอผู้ป่วย ในโรงพยาบาลศรีนครินทร์ ทั้งผลรวมและตามโครงสร้าง กระบวนการ และผลลัพธ์ มีความแตกต่างกันอย่างมีนับสำคัญทางสถิติที่ระดับ .05 3. การมอบหมายงานในทีมการพยาบาล ในเวรเช้า เวรบ่าย และเวรดึก ในโรงพยาบาลศรีนครินทร์ ทั้งผลรวม และตามโครงสร้างปละกระบวนการ มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 |
| Other Abstract: | This research was designed to analyze the nursing team assignment in Srinakharintr Hospital in total activities and in separate patient units in terms of structure, process and outcome; and to study the differences of nursing team assignment practicing in each patient unit and in each group of the nursing duty shifts. The sample, selected by using the simple random sampling method was two hundred and fourteen nursing team assignments which were composed of one hundred and thirty-two nursing duty shifts. Each group of duty shifts includes fourty-four morning, evening and night duty shifts. Six instruments, developed by the investigator five of them had gone through the process of content validation and internal reliability testing which were the Team Leader and Team Member Interview Schedule; Care Giving Interview Schedule; Assignment Checklist; Nursing Process Checklist; and Job Satisfaction Questionnaire. The major findings were: 1. The mean indicated the implementation of the nursing team assignment at moderate level (66.38%) and also showed the structure, process and outcome at the low, good and moderate levels respectively (43.86%), 78.99% and 59.89%). The emergency- Intensive Care Unit demonstrated the highest mean in practicing the total nursing team assignment and also in the structure, process and outcome which were at low, moderate and good level respectively (55.28%), 85.31% and 63.46%). However, the lowest mean of the nursing team assignment was found at the Eye-Ear-Mose-Throat Unit in total activities and also in the structure, process and outcome which were demonstrated the low, moderate and low level respectively (25.00%, 65.54% and 54.19%). 2. There were statistically significant differences at .05 level in total activities and in the structure, process and outcome of the nursing team assignment classified by the groups of nursing unit in Srinakharintr Hospital. 3. There were statistically significant differences as .05 level in all activities and in the structure and outcome of the nursing team assignment classified by morning, evening and night nursing duty shifts. |
