dc.description.abstractการวิจัยนี้เป็นการศึกษาความไม่สามารถใช้ยาตามสั่งของผู้ป่วยนอกคลินิกโรคข้อของโรงพยาบาลจุฬาลงกรณ์ระหว่างเดือนมิถุนายน 2533 ถึงเดือนมกราคม 2534 การสืบหาผู้ป่วยที่ไม่สามารถใช้ยาตามสั่งในผู้ป่วยโรคข้อจำนวน 250 ราย โดยวิธีสัมภาษณ์พบ 85 ราย (41.5%) จากบัตรตรวจโรคพบ 79 ราย (38.5%) เมื่อใช้สองวิธีร่วมกันพบ 121 ราย (59%) และมีผู้ป่วยที่ไม่สามารถใช้ยาตามสั่ง 43 ราย (20.9%) สามารถสืบหาได้ไม่ว่าจะใช้วิธีใดในสองวิธี พบว่าผู้ป่วยร้อยละ 51 มีลักษณะการใช้ยาไม่ตรงตามสั่งที่มีความสำคัญทางคลินิก และร้อยละ 51 เป็นลักษณะที่อาจก่อให้เกิดอันตรายได้ ในการวิเคราะห์หาปัจจัยที่เกี่ยวข้องกับความไม่สามารถใช้ยาตามสั่งโดยวิธีทดสอบไคสแควร์จากทั้งหมด 37 ปัจจัย ปรากฏว่าปัจจัยส่วนใหญ่ไม่มีความสัมพันธ์อย่างมีนัยสำคัญทางสถิติ ส่วนปัจจัยที่พบว่ามีความเกี่ยวข้องได้แก่ ปัญหาซับซ้อนของโรค แรงจูงใจในการใช้ยา (P<0.05) และความเห็นของผู้ป่วยต่อประสิทธิภาพของยา ค่ายา (P<0.1) ปัญหาความไม่สามารถใช้ยาตามสั่งในผู้ป่วยโรคข้อ มีความสำคัญและควรได้รับความสนใจจากบุคลากรทางการแพทย์ ปัจจัยเกี่ยวข้องที่พบจะช่วยในการระบุบ่งชี้กลุ่มผู้ป่วยที่ควรให้ความสนใจเป็นพิเศษเพื่อให้วิธีการป้องกันหรือแก้ไขปัญหาการใช้ยาด้วยตนเองของผู้ป่วยได้ต่อไปen_US
dc.description.abstractalternativeMedication non-compliance (MNC) problem was studied in 205 arthritic outpatients of rheumatology clinic at Chulalongkorn Hospital between June 1990 and January 1991. In detecting MNC, two different methods were used, patient interview and reviewing of patients’ OPD cards. Seperately considered of these two methods, 85 (41.5%) and 79 (38.5%) of the patients studies were found to be non-complied by interview and OPD card reviewing respectively. Combining both methods, 121 patients (59%) were in MNC group, and among these, 43 (20.9%) were easily detected by either method. 51% of the patients were considered having clinically significant problems with their MNC patterns, and 15% may be harzadous to health. On analysis of 37 factors proposed to be related to MNC using chi-square test, most factors were not statistically related except for complicated disease problem, motivation in drug using (at P<0.05), and patients’ attitude towards their drug efficacy, drug cost (at P<.01). MNC problems among arthritic patients need to receive particular attention from related health care providers. The relating factors found in this study are useful to characterize the patients who are prone to MNC problems and indicate specific interventions to prevent or improve the patients’ drug use problems.en_US
