| Abstract: | การวิจัยครั้งนี้เป็นการวิจัยเชิงพรรณนา มีวัตถุประสงค์ เพื่อศึกษาต้นทุน-ประสิทธิผล และต้นทุนกิจกรรมการพยาบาลของการฉีดสีวินิจฉัยและรักษาโรคหลอดเลือดหัวใจผ่านทางหลอดเลือดแดงที่ข้อมือและที่ขาหนีบ จำแนกตามระยะการทำหัตถการ กลุ่มตัวอย่าง ประกอบด้วย 1) ข้อมูลต้นทุน 2) พยาบาลวิชาชีพ จำนวน 36 คน และ 3) ผู้ป่วยที่ได้รับการฉีดสีวินิจฉัยและรักษาโรคหลอดเลือดหัวใจ จำนวน 60 คน เครื่องมือวิจัย ประกอบด้วย 2 ชุด คือ แบบบันทึกข้อมูลการวิเคราะห์ต้นทุน-ประสิทธิผล และแบบบันทึกข้อมูลการวิเคราะห์ต้นทุนกิจกรรมการพยาบาล ผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ และทดสอบความเที่ยงของเครื่องมือ เก็บรวบรวมข้อมูลต้นทุนจากแหล่งทุติยภูมิ และเก็บข้อมูลปฐมภูมิจากการสังเกตและบันทึกปริมาณเวลาในการปฏิบัติกิจกรรมการพยาบาล และการตอบแบบประเมินคุณภาพชีวิต EQ-5D ของผู้ป่วย วิเคราะห์ข้อมูลโดยใช้สถิติการแจกแจงความถี่ ร้อยละ และค่าเฉลี่ย ผลการวิจัยสรุปได้ดังนี้ 1. อัตราส่วนต้นทุน-ประสิทธิผลของผู้ป่วยที่ได้รับการฉีดสีวินิจฉัยและรักษาโรคหลอดเลือดหัวใจผ่านทางหลอดเลือดแดงระยะหลังทำหัตถการ 2 ชั่วโมง ที่ข้อมือมีค่า 155,036.47 บาท/หน่วยประสิทธิผล มีคะแนนอรรถประโยชน์ 79.55 และที่ขาหนีบมีค่า 4,025,357.33 บาท/หน่วยประสิทธิผล มีคะแนนอรรถประโยชน์ 74.83 ส่วนระยะหลังทำหัตถการ 24 ชั่วโมง ที่ข้อมือมีค่า 124,978.38 บาท/หน่วยประสิทธิผล มีคะแนนอรรถประโยชน์ 98.59 และที่ขาหนีบมีค่า 134,178.58 บาท/หน่วยประสิทธิผล มีคะแนนอรรถประโยชน์ 93.45 2. ต้นทุนกิจกรรมการพยาบาลของการฉีดสีวินิจฉัยและรักษาโรคหลอดเลือดหัวใจผ่านทางหลอดเลือดแดงที่ข้อมือมีค่า 1,144.02 บาท และที่ขาหนีบมีค่า 1,230.40 บาท จำแนกตามระยะการทำหัตถการ พบว่า ระยะขณะทำหัตถการมีต้นทุนกิจกรรมการพยาบาลมากที่สุด รองลงมา คือ ระยะหลังทำหัตถการ และระยะก่อนทำหัตถการมีค่าน้อยที่สุด |
| Other Abstract: | The purpose of this descriptive research was to analyze the cost-effectiveness and nursing activities costing of percutaneous coronary diagnostic and intervention procedures by radial and femoral access in 3 phase of procedures. The research samples were cost, 36 staff nurses and 60 patients undergoing percutaneous coronary intervention. The instruments were the record relating analyze the cost-effectiveness and the record relating analyze the nursing activities costing. All instruments were tested for content validity and reliability. Data were collected cost from secondary source, observation nursing activities and patients self-reported quality of life EQ-5D from primary source. Data were analyzed by using frequency, percentage and mean. The results of this study revealed that 1. The cost-effectiveness ratio of percutaneous coronary diagnostic and intervention in post-procedural 2 hour by radial access was 155,036.47 baht Utility Score 79.55 and femoral access was 4,025,357.33 baht Utility Score 74.83. Post-procedural 24 hour by radial access was 124,978.38 baht Utility Score 98.59 and femoral access was 134,178.58 baht Utility Score 93.45. 2. Nursing activities costing in percutaneous coronary diagnostic and intervention procedures by radial access was 1,144.02 baht and femoral access was 1,230.40 baht. Peri-procedural was the highest cost, section followed post-procedural and pre-procedural was the least cost. |
