| Abstract: | การวิจัยนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์ เพื่อสร้างและพัฒนารูปแบบการพยาบาลผู้ป่วยศัลยกรรมกระดูกตามแนวคิดของแซนตี้ เปรียบเทียบความพึงพอใจของผู้ป่วยก่อนและหลังการใช้รูปแบบในกลุ่มทดลองและกลุ่มควบคุม และเปรียบเทียบความพึงพอใจของพยาบาลก่อนและหลังการจัดรูปแบบการพยาบาลผู้ป่วยศัลยกรรมกระดูกตามแนวคิดของแซนตี้ กลุ่มตัวอย่าง คือ ผู้ป่วยศัลยกรรมกระดูก จำนวน 40 คน แบ่งเป็นกลุ่มทดลอง 20 คน และกลุ่มควบคุม 20 คน และพยาบาลวิชาชีพที่ปฏิบัติงานประจำหอผู้ป่วยศัลยกรรมกระดูกจำนวน 10 คน เครื่องมือที่ใช้ในการสร้างและพัฒนารูปแบบการพยาบาลผู้ป่วยศัลยกรรมกระดูกมี 5 ชุดคือ 1) โครงการการจัดรูปแบบการพยาบาลผู้ป่วยศัลยกรรมพยาบาลผู้ป่วยศัลยกรรมกระดูก 2) แผนการอบรมเชิงปฏิบัติการ 3) แนวทางปฏิบัติพยาบาล 4) คู่มือปฏิบัติงาน 5) สื่อวีดีทัศน์ เรื่อง การดูแลตนเองเมื่อกระดูกหัก เครื่องมือที่ใช้ในการรวบรวมข้อมูลมี 3 ชุด คือ 1) แบบกำกับการทดลอง 2) แบบสอบถามความพึงพอใจของผู้ป่วย 3) แบบสอบถามความพึงพอใจของพยาบาล โดยแบบสอบความพึงพอใจของผู้ป่วยและพยาบาลทั้ง 2 ชุด ได้รับการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิจำนวน 5 ท่าน ได้ค่าดัชนีความตรงตามเนื้อหา เท่ากับ .69 และหาความเที่ยงโดยได้ค่าประสิทธิ์แอลฟาของครอนบาค เท่ากับ .97, .93 ตามลำดับ วิเคราะห์ผลการศึกษาโดยใช้โปรแกรมสำเร็จรูปหาคาเฉลี่ยอันดับที่ค่ามัธยฐาน ส่วนเบี่ยงเบนควอไทล์ ทดสอบความแตกต่างด้วยสถิติ Wilcoxon matched-pairs และ Mann-Whitney U-test ผลการวิจัยสรุปได้ดังนี้ 1. ได้รูปแบบการพยาบาลผู้ป่วยศัลยกรรมกระดูก แนวทางปฏิบัติการพยาบาล คู่มือการปฏิบัติงาน วีดีทัศน์ เรื่อง การดูแลตนเองเมื่อกระดูกหัก โดยมีสาระเรื่อง ผู้ประสานงาน ผู้ส่งเสริมความสุขสบาย การเป็นหุ้นส่วน/ผู้ชี้แนะ การจัดการความเสี่ยง การเป็นผู้เชี่ยวชาญทางเทคนิค 2. ความพึงพอใจของผู้ป่วยหลังได้รับการพยาบาลตามรูปแบบการพยาบาลผู้ป่วยศัลยกรรมกระดูก ตามแนวคิดของแซนตี้สูงกว่าก่อนการทดลอง อย่างมีนัยสำคัญทางสถิติที่ระดับ .01 3. ความพึงพอใจของพยาบาลหลังการใช้รูปแบบการพยาบาลผู้ป่วยศัลยกรรมกระดูกตามแนวคิดของแซนตี้สูงกว่าก่อนการทดลอง อย่างมีนัยสำคัญทางสถิติที่ระดับ .01 4. ความพึงพอใจของผู้ป่วยศัลยกรรมกระดูกหลังได้รับการพยาบาลของกลุ่มควบคุมและกลุ่มทดลองโดยรวมไม่แตกต่างกัน |
| Other Abstract: | The purposes of this quasi experimental research were to create the orthopaedics nursing based on Santy model in Bhumibol Adulyadej Hospital, and to study the effect of the orthopaedics nursing model on nurses' satisfaction, and patients' satisfaction between experimental group and control group. The research subjects consisted of 40 orthopaedic patients and 10 professional nurses who worked in orthopaedic unit. The orthopaedic patients were randomly assigned to either experimental group and control group by matched pair technique. Research instruments were training project of orthopaedic nursin model, training program, guidelines for orthopaedic nursing, handbook of nursing practice, and video media. Research data were obtained by questionnaires of orthopaedic patients and nurses' satisfaction. The content validity of the questionnaires of orthopaedic patients and nurses' satisfaction by 5 experts was. 96. The Cronbach's alpha coefficient of questionnaires were .97, and .93 respectively. Statistical methods used in data analysis were median, quartile deviation, Wilcoxon matched-pairs signed-rank test and Mann-Whitney U-test. The research findings were as follows: 1. Orthopaedics nursing model was created based on Santy model which included: guideline for orthopaedic nursing, handbook of nursing practice, observation form of nursing practice, and video media. The contents included co-ordinator, comfort enhancer, partner/guide, risk manager and technician 2. The orthopaedic patients' satisfaction in experimental group after the implementation of Santy orthopaedics nursing model was significantly higher than dose of before at p = .01 3. The overall nurses' satisfaction after implementation of Santy orthopaedics nursing model was significantly higher than dose of before at p = .01 4. The orthopaedic patients' satisfaction in experimental and control group were not significantly different. |
