dc.description.abstract	โรคกระดูกสันหลังส่วนเอวเสื่อมเป็นโรคที่พบมากในผู้สูงอายุ การหนาตัวของชิ้นเอ็น ligamentum flavum (LF) เป็นพยาธิสภาพที่สำคัญของโรคซึ่งทำให้เกิดการตีบแคบของช่องไขสันหลังส่วนเอวและมีการกดทับเส้นประสาท การหนาตัวของชิ้นเอ็น ligamentum flavum อาจเกี่ยวข้องกับ vascular endothelial growth factor (VEGF) ซึ่งมีบทบาทในการสร้างหลอดเลือดใหม่และส่งผลต่อการซ่อมแซมเนื้อเยื่อ ดังนั้นงานวิจัยนี้มีวัตถุประสงค์ เพื่อศึกษาการแสดงออกระดับยีนและโปรตีน VEGF ในชิ้นเอ็น ligamentum flavum ของผู้ป่วยโรคกระดูกสันหลังส่วนเอวเสื่อมที่มีช่องไขสันหลังส่วนเอวตีบแคบในระดับกระดูกสันหลังที่เกิดรอยโรคและ ligamentum flavum ในระดับกระดูกสันหลังที่ไม่เกิดรอยโรค จำนวน 24 ราย โดยวิเคราะห์การแสดงออกของ VEGF ระดับยีนด้วยวิธี reverse transcriptase- polymerase chain reaction (RT-PCR) และ real time RT-PCR รวมทั้งศึกษาการแสดงออกของโปรตีน VEGF ด้วยวิธี enzyme-linked immunosorbent assay (ELISA) และ immunohistochemistry จากผลการศึกษาการแสดงออก VEGF ใน LF ที่หนาตัวมีค่าสูงกว่าใน LF ปกติอย่างมีนัยสำคัญ (P=0.004) และค่าความเข้มของแถบการแสดงออกของ VEGF ใน LF ที่เกิดรอยโรคมีค่าสูงกว่าที่ไม่เกิดรอยโรคอย่างมีนัยสำคัญ (P=0.0002) นอกจากนี้ระดับโปรตีน VEGF ใน LF ที่หนาตัว มีค่ามากกว่าใน LF ปกติอย่างมีนัยสำคัญ (P=0.017) รวมทั้งระดับโปรตีน VEGF ใน LF หนาตัวมีความสัมพันธ์กับการหนาตัวของชิ้นเอ็น LF ซึ่งบ่งชี้ถึงระดับความรุนแรงของโรค ผลการศึกษาทางอิมมูโนพยาธิวิทยาพบการย้อมติดโปรตีน VEGF ภายในชิ้นเอ็น ligamentum flavum ที่เกิดรอยโรคมากกว่าชิ้นเอ็นที่ไม่เกิดรอยโรคและยังพบลักษณะของหลอดเลือดเล็ก ๆ ในชิ้นเอ็นที่เกิดรอยโรค นอกจากนี้ไม่พบความแตกต่างในการแสดงออกของ VEGF ระหว่างเพศชายและเพศหญิง อีกทั้งการแสดงออกของยีนและโปรตีน VEGF ไม่สัมพันธ์กับอายุในชิ้นเอ็นที่ไม่เกิดรอยโรค การศึกษานี้สรุปได้ว่าการแสดงออก VEGF ในชิ้นเอ็น ligamentum flavum ที่สูงขึ้นมีความสัมพันธ์กับการเสื่อมจากการหนาตัวของ LF ซึ่งอาจเป็นสาเหตุของโรคกระดูกสันหลังส่วนเอวเสื่อมที่มีช่องไขสันหลังส่วนเอวตีบแคบ
dc.description.abstractalternative	Lumbar spinal canal stenosis (LSCS) is a degenerative lumbar disease in the elderly. Hypertrophy of the ligamentum flavum (LF) is a potential element in the development of LSCS. Vascular endothelial growth factor (VEGF) may play an essential role in the LF hypertrophy. The aim of this research was to investigate the relationship of VEGF in LF from pathogenic and non-pathogenic lesions of patients with LSCS. Twenty-four patients with LSCS were enrolled in this study. We quantified localized VEGF expression in LF tissues. VEGF expression was determined using reverse transcriptase-polymerase chain reaction (RT-PCR) and real time RT-PCR. The VEGF protein level was analyzed by enzyme-linked immunosorbent assay (ELISA). The localization of VEGF was examined by immunohistochemical staining. The VEGF expression was significantly higher in hypertrophic LF of LSCS than that in non-pathologic LF of controls. VEGF was detected in the cytoplasm of LF fibroblasts, inflammatory cells, and endothelial cells. The average VEGF value of the hypertrophic LF was significantly elevated compared with that of controls (P=0.017). In RT-PCR, the mean optical density of VEGF was substantially higher in the hypertrophic LF than controls (P=0.0002). There was greater VEGF expression in LSCS patients as quantified by real time RT-PCR (P=0.004). The increased expression of VEGF was associated with the degenerative changes of hypertrophic LF, suggesting that VEGF could contribute to one of the mechanisms of pathogenesis in LSCS.
