dc.description.abstract	การวิจัยครั้งนี้เป็นการวิจัยเชิงพรรณนา มีวัตถุประสงค์เพื่อศึกษาต้นทุนต่อหน่วยและต้นทุนกิจกรรมการพยาบาลผู้ป่วยมะเร็งระยะท้ายที่ได้รับการดูแลแบบประคับประคอง โรงพยาบาลนางรอง จังหวัดบุรีรัมย์ โดยจำแนกตามกิจกรรมพยาบาลตามมาตรฐานการดูแลแบบประคับประคอง ทำการศึกษาโดยเก็บข้อมูลย้อนหลังปีงบประมาณ 2560 คือตั้งแต่วันที่ 1 ตุลาคม 2559 จนถึงวันที่ 30 กันยายน 2560 และข้อมูลการวิเคราะห์ต้นทุนกิจกรรมการพยาบาลเป็นการศึกษาแบบไปข้างหน้า ระยะเวลาในการเก็บข้อมูลตั้งแต่วันที่ 15 มีนาคม 2561 ถึงวันที่ 2 มิถุนายน 2561 กลุ่มตัวอย่างประกอบด้วย บุคลากรทางการพยาบาลจำนวน 10 คน ประกอบด้วยพยาบาลวิชาชีพ 7 คน ผู้ช่วยเหลือคนไข้ 3 คน เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลประกอบด้วย 2 ส่วน ได้แก่ ส่วนที่ 1 เครื่องมือที่ใช้ในการวิเคราะห์ข้อมูลต้นทุนต่อหน่วย ส่วนที่ 2 เครื่องมือที่ใช้ในการวิเคราะห์ข้อมูลต้นทุนกิจกรรมการพยาบาลโดยผ่านการตรวจสอบคุณภาพด้านความตรงของเนื้อหาได้ค่า CVI เท่ากับ 0.99 และนำข้อมูลที่ได้มาวิเคราะห์ โดยใช้สถิติการแจกแจงความถี่ ร้อยละ และค่าเฉลี่ย ผลการศึกษาสรุปได้ดังนี้ 1. ต้นทุนต่อหน่วย หอผู้ป่วยดูแลประคับประคอง โรงพยาบาลนางรอง จังหวัดบุรีรัมย์พบว่าในปีงบประมาณ 2560 มีมูลค่าต้นทุนรวมทั้งสิ้น 2,037,037.02 บาท จากจำนวนผู้ป่วยที่เข้านอนพักรักษาที่หอผู้ป่วยดูแลประคับประคอง ทั้งหมด 124 คน โดยคิดตามสัดส่วนการปฏิบัติงานบนหอผู้ป่วยดูแลประคับประคอง คิดเป็นต้นทุนต่อหน่วย 16,427.73 บาท เมื่อพิจารณาต้นทุนประกอบด้วย ต้นทุนค่าแรงจะมีค่ามากที่สุดเท่ากับ 13,163.73 บาท ต้นทุนค่าวัสดุ เท่ากับ 2,206.21 บาทและต้นทุนค่าลงทุน เท่ากับ 1,057.79 บาท คิดเป็นอัตราส่วนร้อยละของต้นทุนค่าแรง : ต้นทุนค่าวัสดุ : ต้นทุนค่าลงทุน เท่ากับ 80.13 : 13.43 : 6.44 ตามลำดับ 2. ต้นทุนกิจกรรมการพยาบาลตามมาตรฐานการพยาบาลการดูแลผู้ป่วยมะเร็งระยะท้ายที่ได้รับการดูแลแบบประคับประคอง จำแนกตามประเภทกิจกรรมการพยาบาล พบว่า ผู้ป่วยมะเร็งระยะท้ายที่เข้ารับการรักษาโดยการดูแลแบบประคับประคอง มีต้นทุนกิจกรรมการพยาบาลทั้งหมดเท่ากับ เท่ากับ 16,486.77 บาทต่อราย ซึ่งกิจกรรมการพยาบาลที่มีต้นทุนมากที่สุดได้แก่ กิจกรรมการพยาบาลทางตรง ต้นทุนกิจกรรมการพยาบาล 9,285.20 บาท คิดเป็นร้อยละ 65.10 รองลงมาคือกิจกรรมการพยาบาลทางอ้อม ต้นทุนกิจกรรมการพยาบาล 2,491.50 บาท คิดเป็นร้อยละ 17.47 กิจกรรมการเขียนบันทึกต่าง ๆ ที่เกี่ยวกับงานพยาบาล มีต้นทุนกิจกรรมการพยาบาล 1,409.88 บาท คิดเป็นร้อยละ 9.88 ส่วนกิจกรรมที่มีต้นทุนน้อยที่สุดคือ กิจกรรมที่เกี่ยวข้องกับหน่วยงาน มีต้นทุนกิจกรรมการพยาบาล 1,077.34 บาท คิดเป็นร้อยละ 7.55	-
dc.description.abstractalternative	The purpose of this descriptive research was to study the unit costs and activity-based costing of nursing care for end-of-life cancer patients in the Palliative Care Unit, Nang Rong Hospital, Buriram Province. Nursing activities were categorized based on palliative care standards. The study was conducted by collecting retrospective data for the 2017 fiscal year (from 1 October 2016 to 30 September 2017) examine activity-based costing in end-of-life cancer patients in palliative care. Data on nursing activity-based cost analysis prospective study monitored the analysis of nursing activities; data were collected from 15 March 2018 to 2 June 2018.The sample was composed of 10 staff nurses and 10 patients. The following instrumentation was used in this study: 1) Record form on analysis of unit costs; 2) End-of-life for palliative care nursing activity dictionary and nursing activity practice record. For content validity, the CVI was 0.99. Descriptive statistics were used for data analysis. The research findings can be summarized as follows: 1. The unit cost of the Love and Care Center, Nang Rong Hospital, Buriram Province was 203,737.02 baht. The total direct cost was based on the number of patients admitted to the Love and Care Center at 124 patients. The total cost was 16,427.77 baht per unit, including labor costs as the highest at 13,163.73 baht, followed by material costs at 2,206.21 baht and capital costs at 1,057.79 baht. The ratio for labor cost : material cost : capital cost were 80.13 :13.43 : 6.44 respectively. 2. In terms of activity-based costs, the end-of-life palliative care nursing cost was 14,623.92 baht. The highest direct nursing activity cost was 9,285.20 baht, or 65.10 percent, while the indirect nursing activity-based cost was 2,491.50 baht, or 17.47 percent.
