| dc.description.abstract | หัตถการดูดไขมันบริเวณใบหน้าส่วนล่าง (V-Face liposuction) เพื่อปรับรูปร่างของใบหน้าให้กระชับมากขึ้นเป็นวิธีที่ได้รับการยอมรับอย่างมากในปัจจุบัน แต่อย่างไรก็ตามหัตถการนี้อาจก่อให้เกิดการบาดเจ็บและภาวะแทรกซ้อนต่อแขนง buccal (BN), marginal mandibular (MMN) และ cervical (CN) ของเส้นประสาทเฟเชียลได้ ดังนั้นการวิจัยนี้จึงมีวัตถุประสงค์เพื่อศึกษาตำแหน่งที่มีความเสี่ยงต่อการบาดเจ็บต่อแขนงของเซอวิโคเฟเชียลดิวิชัน โดยการวิจัยนี้ทำการศึกษาที่ middle และ lower face ในร่างของผู้บริจาคร่างกาย ทั้งหมด 15 ร่าง (เพศชาย 8 ร่าง, เพศหญิง 7 ร่าง) ทำการวัดระยะทางระหว่างแขนงของเซอวิโคเฟเชียลดิวิชันกับจุดอ้างอิงทางกายวิภาคศาสตร์ ได้แก่ ขอบล่างของกระดูกขากรรไกรล่าง (IBM) และเส้นสมมติที่ลากระหว่าง oral commissure และ tragus ของหู (IL) ส่วนความลึกของแต่ละแขนงจะทำการวัดจากผิวหนัง จากการศึกษาครั้งนี้ พบว่า ระยะทางของ BN ถึง IBM และ IL มีค่าเฉลี่ยเท่ากับ 21.96 ± 6.57 มม. และ 35.44 ± 6.14 มม., ระยะทางของ MMN ถึง IBM และ IL มีค่าเฉลี่ยเท่ากับ 16.97 ± 5.79 มม. และ 37.96 ± 6.96 มม. และระยะทางของ CN ถึง IBM และ IL มีค่าเฉลี่ยเท่ากับ 15.49 ± 6.46 มม.และ 43.41 ± 10.19 มม. ส่วนความลึกจากผิวหนังถึง BN, MMN และ CN มีค่าเฉลี่ยเท่ากับ 31.81 ± 5.23 มม., 19.40 ± 3.16 มม. และ 19.10 ± 3.03 มม. ตามลำดับ จากข้อมูลดังกล่าว สามารถนำมาสรุปผลเพื่อประเมินความเสี่ยงในระหว่างการทำหัตถการดูดไขมันบริเวณใบหน้าส่วนล่าง สรุปได้ว่า ที่ตำแหน่งหลังต่อขอบด้านหลังของ ramus 7.80 มม. มีความเหมาะสมสำหรับการทำ skin incision มากที่สุด และที่ตำแหน่งเหนือต่อ IBM 0-5 มม. และ 20 มม. ขึ้นไป ที่ระดับความลึกจากผิวหนังไม่เกิน 10 มม. จะมีความปลอดภัยต่อการบาดเจ็บของแขนงเส้นประสาททั้งสามและมีความเหมาะสมสำหรับการแทงเข็ม cannula มากที่สุด ส่วนที่ตำแหน่งเหนือต่อ IBM 5-20 มม. ที่ระดับความลึกจากผิวหนัง 10-15 มม. จะมีความเสี่ยงต่อการบาดเจ็บของแขนงเส้นประสาททั้งสามสูงถึงร้อยละ 39.33 | - |
| dc.description.abstractalternative | V-Face liposuction is the cosmetic surgical procedures for contouring the lower face. The buccal (BN), marginal mandibular (MMN) and cervical (CN) branches of the cervicofacial division of the facial nerve may be injured during the procedure. Therefore, the aim of this study is to provide the danger points for awareness during liposuction procedure at the lower face. This study was performed on bilateral middle and lower faces in 15 faces of Thai soft embalmed cadavers (male=8, female=7). The distances of branches of cervicofacial division were measured to inferior border of the mandible (IBM) and imaginary line (IL). Then, the depths of all branches were investigated to the facial skin. The distance of BN from IBM and IL were 21.96 ± 6.57 mm and 35.44 ± 6.14 mm. The distance of MMN from IBM and IL were 16.97 ± 5.79 mm and 37.96 ± 6.96 mm. The distance of CN from IBM and IL were 15.49 ± 6.46 mm and 43.41 ± 10.19 mm. The depth of BN, MMN and CN were 31.81 ± 5.23, 19.40 ± 3.16 and 19.10 ± 3.03 mm, respectively. According to the result data, there were analyzed the risk for awareness during liposuction procedure to avoid injury of all facial nerve branches. They concluded that at 7.80 mm posterior to the posterior border of ramus, there is the most appropriate skin incision location. The safest area for inserted blunt cannula during liposuction is located above approximately 0 to 5 mm and beyond 20 mm from IBM at the depth from the skin 0 to 10 mm. Maximum risk area is located approximately 5-20 mm from IBM at the depth from the skin 10 to 15 mm (39.33%). | - |
