| dc.description.abstract | การวิจัยนี้เป็นการวิจัยเชิงวิเคราะห์สหสัมพันธ์แบบภาคตัดขวาง มีวัตถุประสงค์เพื่อศึกษาความชุกและปัจจัยที่สัมพันธ์กับการเกิดภาวะง่วงนอนมากระหว่างวัน (Excessive Daytime Sleepiness; EDS) และภาวะโงกหลับกะทันหัน (Sleep Attack; SA) ในผู้ป่วยไทยโรคพาร์กินสัน โดยทำการศึกษาในผู้ป่วยที่เข้ารับการรักษาที่ศูนย์ความเป็นเลิศทางการแพทย์โรคพาร์กินสันและกลุ่มโรคความเคลื่อนไหวผิดปกติ โรงพยาบาลจุฬาลงกรณ์ ระหว่างเดือน มกราคม-มิถุนายน 2560 จำนวน 160 คน โดยใช้แบบสอบถามวัดระดับความง่วงนอน Epworth Sleepiness Scale (ESS) ฉบับภาษาไทย เพื่อประเมินการเกิด EDS และสอบถามการเกิด SA เพื่อประเมินการเกิด SA ผลการศึกษาพบว่า ผู้ป่วยมีอายุเฉลี่ย (±ส่วนเบี่ยงเบนมาตรฐาน) 64.03 ± 10.97 ปี ระยะเวลาเป็นโรคพาร์กินสันเฉลี่ย (±ส่วนเบี่ยงเบนมาตรฐาน) 9.20 ± 5.14 ปี และเป็นเพศชายร้อยละ 51.25 มีความชุกของการเกิด EDS ร้อยละ 28.75 (95%Cl: 22-36) และการเกิด SA ร้อยละ 9.38 (95%Cl: 5-14) ปัจจัยที่สัมพันธ์กับการเกิด EDS คือ อายุ (OR: 1.08, 95% CI: 1.02-1.14) ระดับความรุนแรงของโรค (Hoehn & Yahr stage) (OR: 3.70, 95% CI: 1.68-8.15) ขนาดยาของ dopamine agonists เมื่อเทียบเป็นขนาดยา levodopa ต่อวัน (dopamine agonist levodopa equivalent daily dose; DALEDD) (OR: 1.01, 95% CI: 1.003-1.012) และการได้รับยากลุ่ม benzodiazepine (OR: 0.20, 95% CI: 0.06-0.65) และปัจจัยที่สัมพันธ์กับการเกิด SA คือ ระดับความรุนแรงของโรค (Hoehn & Yahr stage) (OR: 2.50, 95% CI: 1.11-5.64) ขนาดยา DALEDD (OR: 1.006, 95% CI: 1.0005-1.01) และจำนวนชั่วโมงการนอนกลางคืน (OR: 0.63, 95% CI: 0.46-0.87) ดังนั้นจึงควรมีการติดตามเฝ้าระวังการเกิด EDS หรือการเกิด SA ในผู้ป่วยโรคพาร์กินสันที่มีปัจจัยที่เพิ่มโอกาสในการเกิดภาวะเหล่านั้นได้ | - |
| dc.description.abstractalternative | The objective of this cross-sectional and analytical study was to determine the prevalence of and factors associated with excessive daytime sleepiness (EDS) and sleep attack (SA) in Thai patients with Parkinson's disease (PD). One hundred and sixty PD patients attended the study during January to June, 2017 at Chulalongkorn Center of Excellence for Parkinson’s Disease and Related disorders, King Chulalongkorn Memorial Hospital. Epworth Sleep Scale (ESS) was used for evaluating the EDS and the presence of SA was determined from the patient’s interview. PD patients had the average age (±SD) of 64.03 ± 10.97 years, disease duration (±SD) of 9.20 ± 5.14 years and 51.25% of PD patients was male .The prevalence of EDS was 28.75% (95%Cl: 22-36) and of SA was 9.38% (95%Cl: 5-14). The EDS was independently associated with age (OR: 1.08, 95% CI: 1.02-1.14), Hoehn & Yahr stage (OR: 3.70, 95% CI: 1.68-8.15), dopamine agonist levodopa equivalent daily dose (DALEDD) (OR: 1.01, 95% CI: 1.003-1.012) and benzodiazepine (OR: 0.20, 95% CI: 0.06-0.65). Sleep attack was independently associated with Hoehn & Yahr stage (OR: 2.50, 95% CI:1.11-5.64), DALEDD (OR: 1.006, 95% CI: 1.0005-1.01) and sleep duration at night (OR: 0.63, 95% CI: 0.46-0.87). Therefore, PD patients who have the significant risk factors of EDS or SA should be monitored for the presence of EDS or SA. | - |
