| Abstract: | การวิจัยครั้งนี้ มีวัตถุประสงค์เพื่อสำรวจรายการปฏิบัติการพยาบาลผู้ป่วยโรคระบบหายใจในโรงพยาบาลชุมชนจังหวัดสุพรรณบุรีตามแนวคิดของ Nursing Interventions Classification (NIC) และจัดกลุ่มรายการปฏิบัติการพยาบาลตามข้อวินิจฉัยทางการพยาบาลของสมาคมวินิจฉัยทางการพยาบาลแห่งอเมริการเหนือ (NANDA) กลุ่มตัวอย่าง คือ ผู้ป่วยโรคระบบหายใจ 45 ราย เครื่องมือที่ใช้ในการวิจัยประกอบด้วย แบบสังเกต แบบตรวจสอบเอกสาร และแบบสัมภาษณ์ผู้ป่วยที่ผู้วิจัยสร้างขึ้นเอง วิเคราะห์ข้อมูลโดยการแจกแจงความถี่และค่าร้อยละ ผลการวิจัยพบว่า 1.รายการปฏิบัติการพยาบาลผู้ป่วยโรคระบบหายใจในโรงพยาบาลชุมชนจำแนกหมวดได้ด้งนี้ การพยาบาลโดยตรง : ประกอบด้วยการปฏิบัติการพยาบาล 6 หมวด คือ 1) หมวดการดูแลร่างกาย 14 รายการ 2)หมวดการดูแลเพื่อควบคุมระดับสมดุลเลือด 23 รายการ 3) หมวดการดูแลจิตสังคม 6 รายการ 4) หมวดการดูแลเพื่อป้องกันอันตราย 10 รายการ และ 5) หมวดการดูแลเพื่อสนับสนุนการใช้บริการสุขภาพ 13 รายการ 2.รายการการปฏิบัติการพยาบาลจัดกลุ่มตามข้อวินิจฉัยการพยาบาลได้จำนวนรายการ ดังนี้ 2.1 ลักษณะการหายใจเปลี่ยนแปลง 17 รายการ 2.2เสี่ยงต่อการติดเชื้อ17 รายการ 2.3 อุณหภูมิร่างการผิดปกติ 13 รายการ 2.4 ร่างกายเสี่ยงต่อการขาดน้ำ 13 รายการ 2.5 ได้รับสารอาหารไม่เพียงพอกับความต้องการของร่างการ 13 รายการ 2.6การเปลี่ยนแปลงของเยื่อบุช่องปาก 7 รายการ 2.7เสี่ยงต่อการสำลัก 10 รายการ 2.8ความสมบูรณ์ของผิวหนังเสียไป 10 รายการ 2.9ท้องผูก 12 รายการ 2.10 ความสามารถในการทำกิจกรรมลดลง 11 รายการ 2.11 ไม่สามารถในการทำกิจกรรมลดลง 14 รายการ 2.12 ไม่สามารถขับเสมหะได้อย่างประสิทธิภาพ 31 รายการ 2.13 การแลกเปลี่ยนก๊าซบกพร่อง 14 รายการ 2.14 ไม่สามารหายใจได้เอง 24รายการ 2.15 ความสามารถในการดูตนเอง 14 รายการ 2.16 อ่อนเพลียหรืออ่อนแรง 7 รายการ 2.17 การเปลี่ยนแปลงการกำซาบของเลือดและออกซิเจนในในเนื้อเยื่อ 26 รายการ 2.18 แลลแผนการนอนหลับปกติ 13 รายการ 2.19 ความเจ็บปวดไม่สุขสบาย 15 รายการ 2.20 ขาดความรู้เรื่องและวิธีการรักษาพยาบาล 5 รายการ 2.21 วิตกกังวล 8 รายการ 2.22 การสูญเสียอำนาจ 9 รายการ 2.23 ความบกพร่องในการติดต่อสื่อสาร 8 รายการ 2.24 ความสัมพันธ์ทางเพศเปลี่ยนแปลง 5รายการ 2.25 ภาวะขาดสิ่งยึดเหนี่ยวทางใจ 6 รายการ |
| Other Abstract: | The purpose of this research were to survey the nursing intervention lists of respiratory system patients in community hospital in Changwad Suphunburi in accordance with Nursing Interventions Classification (NIC) and the North American Nursing Diagnosis Association (NANDA). The samples were 45 respiratory system patients. Three sets of instruments, observation form, document check list form, questionnaires, were developed by researcher. The obtained data were analyzed through percentage and frequency distribution. The results of study were as follow: 1. According with NIC, the nursing intervention lists of respiratory system patient in community hospital in Changwad Suphunburi were as follow: Direct nursing care: there were 6 domains of nursing care as 1) physical: basic domain 14 items 2) physical: complex domain 23 items 3) behavioral domain 6 items 4) safety domain 10 items and 5) family domains 1 item Indirect nursing care: there were 1 domain identified as health system 13 items 2. According with NANDA, the obtained nursing intervention lists of respiratory system patients can be grouped into 25 groups as follow: 2.1 respiratory function, list for altered 17 items 2.2 infection, risk for 17 items 2.3 body temperature, risk for altered 13 items 2.4 fluid volume deficit 13 item 2.5 nutrition, altered: less than body requirements 13 items 2.6 oral mucous membrane, altered 7 items 2.7 risk for aspiration 10 items 2.8 skin integrity, impaired 10 items 2.9 constipation 12 items 2.10 activity intolerance 11 items 2.11 ineffective airway clearance 14 items 2.12 ineffective breathing pattern 31 items 2.13 impair gas exchange 14 items 2.14 inability to sustain spontaneous ventilation 24 items 2.15 self-care deficit 14 items 2.16 fatigue 7 items 2.17 tissue perfusion, altered: cardiopulmonary 26 item 2.18 sleep pattern disturbance 13 items 2.19 comfort, altered 15 items 2.20 knowledge deficit 5 items 2.21 anxiety 8 items 2.22 powerlessness 9 items 2.23 communication, impaired 8 items 2.23 communication, impaired 8 items 2.25 spiritual distress 6 items |
