| Abstract: | การวิจัยเชิงคุณภาพครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาประสบการณ์การดูแลผู้ป่วยมะเร็งระยะลุกลามที่บ้านของผู้ดูแลในครอบครัว โดยใช้ระเบียบวิธีวิจัยแบบปรากฏการณ์วิทยาการตีความของ Martin Heidegger คัดเลือกผู้ให้ข้อมูลแบบเฉพาะเจาะจง ผู้ให้ข้อมูล คือ ผู้ดูแลในครอบครัวที่ทำหน้าที่เป็นผู้ดูแลหลักให้การดูแลผู้ป่วยที่ได้รับการวินิจฉัยจากแพทย์ว่าเป็นผู้ป่วยมะเร็งระยะลุกลาม เก็บรวบรวมข้อมูลโดยการสัมภาษณ์เชิงลึก บันทึกเทป วิเคราะห์ข้อมูลตามวิธีของ van Manen ข้อมูลอิ่มตัวจากผู้ให้ข้อมูล จำนวน 13 ราย ผลการวิจัยแบ่งออกได้เป็น 2 ส่วน ได้แก่ ส่วนที่ 1 การให้ความหมายของความเจ็บป่วยด้วยมะเร็งระยะลุกลาม แบ่งออกเป็น 2 ประเด็นหลัก ดังนี้ 1) ผู้ป่วยมีชีวิตอยู่ได้อีกไม่นาน และ 2) ยังหวังว่าการรักษาอาจจะช่วยให้หายได้ และส่วนที่ 2 ประสบการณ์การดูแลผู้ป่วยมะเร็งระยะลุกลามที่บ้านของผู้ดูแลในครอบครัว แบ่งออกเป็น 3 ประเด็นหลัก ดังนี้ 1) การดูแลเพื่อเพิ่มความสุขสบาย ไม่ทุกข์ทรมาน ครอบคลุมในเรื่อง 1.1) การดูแลความสะอาดและความสุขสบายตามสภาพร่างกายกายของผู้ป่วย 1.2) การดูแลให้ได้รับอาหารตามความต้องการของผู้ป่วย 1.3) การดูแลความสะอาดสิ่งแวดล้อมเพื่อส่งเสริมความสุขสบายและความปลอดภัย 1.4) การดูแลด้านจิตใจและอารมณ์ของผู้ป่วย 1.5) การดูแลตามความเชื่อทางศาสนา และ 1.6) การดูแลผู้ป่วยเพื่อลดความกลัวตาย 2) การได้รับความช่วยเหลือสนับสนุน ครอบคลุมในเรื่อง 2.1) การได้รับความช่วยเหลือสนันสนุนจากครอบครัว 2.2) การได้รับความช่วยเหลือสนับสนุนจากระบบสุขภาพ และ 2.3) การได้รับความช่วยเหลือสนันสนุนจากระบบสุขภาพชุมชน และ 3) การเรียนรู้และการปรับตัวจากการดูแล ครอบคลุมในเรื่อง 3.1) เพราะความไม่รู้จึงทำให้เครียดในช่วงแรกของการดูแล 3.2) เหนื่อยจากความรับผิดชอบหลากหลาย ร่างกายพักผ่อนไม่พอ และ 3.3) ภูมิใจ ดีใจที่ได้ดูแล และเป็นโอกาสที่ได้ตอบแทนพระคุณ จากผลการวิจัยนี้ นำมาเป็นข้อมูลพื้นฐานสำหรับทีมสุขภาพในการส่งเสริมศักยภาพของผู้ดูแลในครอบครัวให้สามารถดูแลผู้ป่วยมะเร็งระยะลุกลามที่บ้านได้ |
| Other Abstract: | This qualitative study was to study the experiences of caring for patients with advanced cancer at home of family caregivers. The phenomenological research methodology was employed based on the concept of Heidegger. Methods: The selected according to purposive sampling. The key informants were primary caregivers caring for patients diagnosed by a doctor as had advanced cancer. Data were collected using interview guidelines with in-depth interview. Data were analyzed by using van Manen’s method and audio recording and until saturated data with the 13 informants The results of the research revealed two parts consisting of part 1 the meaning of illness of advancer cancer were categorized into 2 main-themes : 1) Short life of patient 2) Expecting that the cancer may be cured by the treatment and part 2 the experiences of caring for patients with advanced cancer at home of family caregivers were categorized into 3 main-themes: 1) Caring for increasing of happiness and reducing of agony, that covered 1.1) Caring with cleanliness and happiness depended on the patient's physical condition, 1.2) Caring about the diet according to patient's requirement, 3) Caring with cleaned environment to promote the comfort and safety, 1.4) Attended about sentiment and emotion of the patients, 1.5) Caring the patients relating to the principle of religion, and 1.6) Caring the patients with reducing a fear of death. 2) Getting the encouragements and supports, that covered 2.1) Encouragements and support by family, 2.2) Encouragements and support by health system, and 2.3) Encouragements and support by community health system. And 3) Learning and adjusting of caregiver by caring experiences, that covered 3.1) Because of don't knew, it was stressful in the early stages of care, 3.2) Feeling a tiredness with various responsibilities and sleep deprivation, and 3.3) Feeling prides and satisfaction for patient caring and getting the opportunity to present the gratitude The results of this work was become as a based information for health care team which encouraged a potential caring of family caregivers to have ability to take care the advanced cancer patient at home. |
