| Abstract: | การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อเปรียบเทียบอาการหายใจลำบากในผู้ป่วยโรคปอดอุดกั้นเรื้อรังก่อน และหลังได้รับโปรแกรมการจัดการตนเอง และเปรียบเทียบอาการหายใจลำบากของผู้ป่วยโรคปอดอุดกั้นเรื้อรังระหว่างกลุ่มที่ได้รับโปรแกรมการจัดการตนเอง และกลุ่มที่ได้รับการพยาบาลตามปกติ กลุ่มตัวอย่างคือ ผู้ป่วยโรคปอดอุดกั้นเรื้อรังวัยผู้ใหญ่ที่มีอายุตั้งแต่ 35-59 ปี ที่มารับการรักษาที่แผนกผู้ป่วยนอก โรงพยาบาลศูนย์ยะลา ที่มีคุณสมบัติตามเกณฑ์ที่กำหนด 40 ราย โดยเก็บรวบรวมข้อมูลในกลุ่มควบคุมก่อนจนครบ 20 ราย แล้วจึงดำเนินการเก็บรวบรวมข้อมูลในกลุ่มทดลองอีก 20 ราย กลุ่มตัวอย่างทั้งสองกลุ่มจะมีความคล้ายคลึงกันในเรื่องของอายุ ระดับความรุนแรงของโรค กลุ่มควบคุมได้รับการพยาบาลตามปกติ ส่วนกลุ่มทดลองได้รับโปรแกรมการจัดการตนเองที่พัฒนามาจากแนวคิดการจัดการตนเอง และเทคนิคการกำกับตนเองประกอบไปด้วย 3 ระยะ คือ ระยะที่ 1 ผู้ป่วยประเมินปัญหาตนเอง ระยะที่ 2 วางแผนการปฏิบัติ และพัฒนาทักษะ ระยะที่ 3 เป็นการติดตามผลการกำกับตนเองตามเป้าหมาย เครื่องมือที่ในการวิจัยประกอบด้วย 1. เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล และแบบวัดอาการหายใจลำบาก Modified Borg's scale 2. เครื่องมือดำเนินการทดลอง ประกอบด้วย โปรแกรมการจัดการตนเองที่พัฒนามาจากแนวคิดการจัดการตนเอง และเทคนิคการกำกับตนเอง สถิติที่ใช้ในการวิเคราะห์ข้อมูลได้แก่ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สถิติ Kolmogorov-Smirnov test และสถิติทดสอบที (Independent t-test และ Paired t-test) ผลการวิจัย 1. อาการหายใจลำบากของกลุ่มทดลองภายหลังได้รับโปรแกรมการจัดการตนเองน้อยกว่าก่อนการทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ([mean] [subscript ก่อนการทดลอง] = 7.25, [mean][subscript หลังการทดลอง]=5.70, t=6.307, p<.05) 2. อาการหายใจลำบากภายหลังเข้าร่วมโปรแกรมการจัดการตนเองกลุ่มทดลองน้อยกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ([mean] [subscript กลุ่มควบคุม] = 7.60, [mean][subscript กลุ่มทดลอง] = 5.70, t = 3.80, p<.001) |
| Other Abstract: | The purpose of this study was to examine the effect of the Self-Management Program on dyspnea in patients with Chronic Obstructive Pulmonary Disease (COPD), A total of 40 patients with COPD were recruited from the out-patient department, Yala Hospital. The participants were devided into one control group and one experimental group. Two groups were matched in terms of age, disease severity. The control group received conventional care while the experimental group attended a six-week Self-Management Program. Within the context of Tobin's conceptualization of self-management and Kanfer's notion of self-monitoring, the program included patients self-assessment and goal setting, individualized education of dyspnea management strategies, pulmonary rehabilitation, and self-monitoring skill teaching. To enhance the implementation of the program, Yavi-language education materials including the booklet and VCD were provided. Regarding the measurement tool, the Modified Borg Scale was use to measure perceived dyspnea at the beginning of the program and 6 weeks after completing the program. Data were analyzed using descriptives statistic and t-test The major finding were as follows: 1. At the end of program, the mean score on dyspnea of the experiment group significantly decreased ([mean] [subscript pre] = 7.25, [mean] [subscript post] = 5.70, t = 6.307, p < .05) 2. The posttest mean score on dyspnea of the experiment group was significantly lower than that of the control group ([mean] [subscript control] t=7.60, [mean] [subscript experimen] = 5.70, t=3.80, P<.001) |
