dc.description.abstractการวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาผลการให้คำแนะนำของเภสัชกรแก่ผู้ป่วยนอกที่รับประทานยาวาร์ฟารินที่โรงพยาบาลสงขลานครินทร์ โดยศึกษาในผู้ป่วยโรคลิ้นไมตรัลตีบ ผู้ป่วยที่ได้รับการผ่าตัดใส่ลิ้นหัวใจเทียม และผู้ป่วยที่มีภาวะหัวใจเต้นผิดจังหวะ จะทำการติดตามผู้ป่วย 3 ครั้งในช่วงเดือนกุมภาพันธ์-กันยายน พ.ศ. 2542 เพื่อศึกษาค่า INR ภาวะแทรกซ้อนจากการใช้ยาวาร์ฟาริน ได้แก่ ภาวะเลือดออกและภาวะลิ่มเลือดอุดตัน และความพึงพอใจของผู้ป่วยที่มีต่อการให้บริการ ผู้ป่วยที่ทำการศึกษาทั้งสิ้น 97 ราย เป็นผู้ป่วยโรคลิ้นไมตรัลตีบ 29 ราย (ร้อยละ 29.9), ผู้ป่วยที่ได้รับการผ่าตัดใส่ลิ้นหัวใจเทียม 42 ราย (ร้อยละ 43.3) และผู้ป่วยที่มีภาวะหัวใจเต้นผิดจังหวะ 26 ราย (ร้อยละ 26.8) ค่า INR เฉลี่ยของผู้ป่วยแต่ละโรค ได้แก่2.56+-1.05, 2.58+-1.40 และ 2.49+-1.27 ตามลำดับ ผลการให้คำแนะนำแก่ผู้ป่วยพบว่าสัดส่วนของค่า INR ของผู้ป่วยที่อยู่ในช่วงของการรักษาสำหรับแต่ละโรคก่อนและหลังได้รับคำแนะนำไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p>0.05) และผู้ป่วยที่ได้รับการผ่าตัดใส่ลิ้นหัวใจเทียมจะมีค่า INR ต่ำกว่าช่วงการรักษามากกว่าโรคอื่นๆ ส่วนภาวะแทรกซ้อนพบว่าภาวะเลือดออกในช่วงที่ให้คำแนะนำมีจำนวนสูงกว่าช่วงก่อนให้คำแนะนำ อาจจะเนื่องมาจากผู้ป่วยมีความรู้ในเรื่องดังกล่าวเพิ่มมากขึ้น ภาวะเลือดออกแบบไม่รุนแรงที่พบส่วนใหญ่ ได้แก่ จ้ำเลือดบริเวณผิวหนัง เลือดออกตามไรฟัน และประจำเดือนมามาก ส่วนอาการที่อาจบ่งชี้ว่าเกิดภาวะลิ่มเลือดอุดตันจะพบอาการชาตามปลายมือหรือปลายเท้า หน้ามืด และพูดไม่ชัด นอกจากนี้มีผู้ป่วย 3 รายที่เข้ารับการรักษาโรงพยาบาลเนื่องจากภาวะลิ่มเลือดอุดตัน โดยผู้ป่วย 2 รายเกิดภาวะลิ่มเลือดอุดตันในสมอง และผู้ป่วย 1 รายเกิดภาวะลิ่มเลือดอุดตันในปอด ผู้ป่วยทั้ง 3 รายเกิดภาวะลิ่มเลือดอุดตันเนื่องจากมีค่า INR ต่ำกว่าช่วงของการรักษา ผู้ป่วยที่ได้รับคำแนะนำจะมีระดับความรู้เกี่ยวกับยาวาร์ฟารินเพิ่มขึ้นอย่างต่อเนื่อง และผู้ป่วยส่วนใหญ่มีความพึงพอใจกับการให้คำแนะนำอยู่ในระดับดี ทั้งในแง่ของความพึงพอใจโดยรวม การสื่อสาร ความสะดวกสบาย ยกเว้นในแง่ของเวลาที่ผู้ป่วยรู้สึกว่าต้องรอรับคำแนะนำนาน ดังนั้นการให้คำแนะนำนาน ดังนั้นการให้คำแนะนำของเภสัชกร โดยการพูดคุยร่วมกับการแจกเอกสารเพิ่มเติมให้แก่ผู้ป่วยจะช่วยเพิ่มระดับความรู้เรื่องการใช้ยาให้แก่ผู้ป่วยได้en
dc.description.abstractalternativeThe objective of this study was to assess the effect of pharmacist's counseling on outpatients, with mitral stenosis (MS), mitral valve replacement (MVR), and atrial fibrillation (AF), receiving warfarin at Songklanagarind hospital. The patients were followed up for 3 times between February to September 1999 for their International Normalized Ratio (INR), complications including bleeding and thromboembolism, and patient satisfaction of the service. The data were collected on 97 patients. The number of patients with MS, MVR, and AF were 29 (29.9%), 42 (42.3%), and 26 (26.8%), respectively. Mean INR of these indications during the study period were 2.56+-1.05, 2.58+-1.40, and 2.49+-1.27, respectively. The result showed that the patient's INR which were in therapeutic range for each indication was not statistically different between before and after receiving pharmacist's counseling (p>0.05). The number of patients having lower INR than target range were greater in those with MVR than other groups of patient. Bleeding complications in the study period were reported more frequently than those in the pre-study period. This was possibly due to more knowledge of patients after counseling. Minor bleedings occurred most offten at the skin, gum, and vaginal. Sign and symptoms which could indicate thromboembolism were paresthesia, syncope, and slurred speech. The thromboembolism caused hospital admission in 3 patients; 2 with cerebral embolism, and 1 with pulmonary embolism, due to the INR lower than target range. The patients receiving pharmacist's counseling had increased knowledge about warfarin continuously. The patient satisfaction toward the pharmacy service was rated as good in the following areas : general satisfaction, pharmaceutical interpersonal manner, communication, and accessibility. The only exception was the patients felt waiting time for counseling was too long. It could be concluded that the pharmacist's counseling through verbal and written reinforcement improved patient knowledge about warfarin.en
