dc.description.abstract	การวิจัยนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการส่งเสริมการดูแลช่องปากด้วยตนเองร่วมกับการอมกลั้วปากด้วยน้ำมันมะพร้าวบริสุทธิ์ต่ออาการเยื่อบุช่องปากอักเสบในผู้ป่วยมะเร็งศีรษะและคอที่ได้รับการฉายรังสี ร่วมกับยาเคมีบำบัด กลุ่มตัวอย่างคือ ผู้ป่วยมะเร็งบริเวณศีรษะและคอที่ได้รับรักษาด้วยการฉายรังสีร่วมกับยาเคมีบำบัดแบบผู้ ป่วยนอก หน่วยรังสีรักษาโรงพยาบาลระดับตติยภูมิหนึ่งแห่งในจังหวัดสุราษฎร์ธานี จำนวน 45 ราย แบ่งเป็น 3 กลุ่ม กลุ่มละ 15 ราย โดยกลุ่มทดลองที่ 1 ได้รับโปรแกรมการดูแลช่องปากด้วยการอมกลั้วปากด้วยน้ำมันมะพร้าวบริสุทธิ์ กลุ่มทดลองที่ 2 ได้รับโปรแกรมการส่งเสริมการดูแลช่องปากด้วยตนเองร่วมกับการอมกลั้วปากด้วยน้ำมันมะพร้าวบริสุทธิ์ และกลุ่มควบคุมได้รับ การพยาบาลปกติ เครื่องมือที่ใช้ในการทดลอง คือ โปรแกรมส่งเสริมการดูแลช่องปากด้วยตนเองร่วมกับการอมกลั้วปากด้วยน้ำ มันมะพร้าวบริสุทธิ์ ซึ่งประกอบด้วย การสร้างสัมพันธภาพ การให้ข้อมูล การฝึกทักษะปฏิบัติ และการส่งเสริมสนับสนุนร่วมกับการอมกลั้วปากด้วยน้ำมันมะพร้าวบริสุทธิ์ และโปรแกรมการดูแลช่องปากด้วยการอมกลั้วปากด้วยน้ำมันมะพร้าวบริสุทธิ์ ที่ได้ รับการตรวจสอบความตรงจากผู้ทรงวุฒิ เครื่องมือที่ใช้เก็บรวบรวมข้อมูล คือ แบบประเมินเยื่อบุช่องปากอักเสบของ Sonis et al. (1999) ซึ่งมีค่าความเที่ยงของการสังเกตได้เท่ากับ .952 วิเคราะห์ข้อมูลโดยใช้สถิติร้อยละ ค่าเฉลี่ย ค่าเบี่ยงเบนมาตรฐาน และสถิติวิเคราะห์ความแปรปรวนแบบวัดซ้ำ (Repeated Measures ANOVA) 1. คะแนนเฉลี่ยอาการเยื่อบุช่องปากอักเสบในผู้ป่วยมะเร็งศีรษะและคอที่ได้รับการฉายรังสีร่วมกับยาเคมีบำบัดภายในกลุ่มที่ได้รับโปรแกรมการดูแลช่องปากด้วยการอมกลั้วปากด้วยน้ำมันมะพร้าวบริสุทธิ์กลุ่มที่ได้รับโปรแกรมการส่งเสริมการดูแลช่องด้วยตนเองร่วมกับการอมกลั้วปากด้วยน้ำมันมะพร้าวและกลุ่มที่ได้รับการพยาบาลตามปกติแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. คะแนนเฉลี่ยอาการเยื่อบุช่องปากอักเสบในผู้ป่วยมะเร็งศีรษะและคอที่ได้รับการฉายรังสีร่วมกับยาเคมีบำบัดกลุ่มที่เข้าร่วมโปรแกรมการส่งเสริมการดูแลช่องด้วยตนเองร่วมกับการอมกลั้วปากด้วยน้ำมันมะพร้าวบริสุทธิ์ (กลุ่มทดลองที่ 2) ต่ำกว่ากลุ่มที่ได้รับโปรแกรมการดูแลช่องปากด้วยการอมกลั้วปากด้วยน้ำมันมะพร้าวบริสุทธิ์ (กลุ่มทดลองที่ 1) และกลุ่มที่ได้รับการพยาบาลตามปกติ ส่วนกลุ่มที่ได้รับโปรแกรมการดูแลช่องปากด้วยการอมกลั้วปากด้วยน้ำมัน มะพร้าวบริสุทธิ์ (กลุ่มทดลองที่ 1) ต่ำกว่ากลุ่มที่ได้รับการพยาบาลปกติ (กลุ่มควบคุม) อย่างมีนัยสำคัญทางสถิติที่ระดับ .05	en_US
dc.description.abstractalternative	The purpose of this quasi-experimental research was to examine the effects of oral self care promoting program and virgin coconut oil pulling on oral mucositis in head and neck cancer patients receiving concurrent chemoradiotherapy. The study was conducted at the Radiotherapy and Oncology Outpatient Unit of a tertiary hospital in Surattani province. A sample of 45 patients was assigned into three groups. Each group consisted of 15 patients. Participants in the experimental group 1 received the oral coconut oil pulling program . The experimental group 2 received oral self care program combining coconut oil pulling, and the control group received usual care. Research instruments was the oral self care promoting program and virgin coconut oil pulling. This program had four dimensions : (a) interactive nursing care, (b) didactic information, (c) self-care exercises (skills), and (d) supportive care combining coconut oil pulling. Oral mucositis was evaluated by using an assessment tool developed by Sonis et al. (1999). The instrument was validated by a panel of experts. The interrater reliability was .952. The data were analyzed and presented using percentage, mean, standard deviation and repeated measures ANOVA. 1. The oral mucositis mean score of head and neck cancer patients receiving the oral coconut oil pulling program, the experimental 2, and the control group were statistical different at the level of 0.5. 2. The oral mucositis mean scores of the patients receiving oral self care promoting program combining coconut oil pulling was lower than that of the patients receiving coconut oil pulling program. Also, the mean scores of the patients receiving coconut oil pulling program was lower than that of the group receiving usual care at statistical different level of 0.5.	en_US
