| Abstract: | งานวิจัยครั้งนี้เป็นการศึกษาความสัมพันธ์เชิงทำนาย มีวัตถุประสงค์ เพื่อศึกษาคุณภาพชีวิตของผู้ป่วยวัยผู้ใหญ่โรคมะเร็งลำไส้ใหญ่และทวารหนักที่มีทวารเทียม เพื่อศึกษาความสัมพันธ์ระหว่างความรู้ ความรู้สึกมีคุณค่าในตนเอง การดูแลตนเอง และการสนับสนุนทางสังคม กับคุณภาพชีวิตของผู้ป่วยวัยผู้ใหญ่โรคมะเร็งลำไส้ใหญ่และทวารหนักที่มีทวารเทียม และเพื่อศึกษาปัจจัยทำนายคุณภาพชีวิตของผู้ป่วยวัยผู้ใหญ่โรคมะเร็งลำไส้ใหญ่และทวารหนักที่มีทวารเทียม กลุ่มตัวอย่างคือผู้ป่วยวัยผู้ใหญ่โรคมะเร็งลำไส้ใหญ่และทวารหนักที่มีทวารเทียมที่แผนกผู้ป่วยนอก ในโรงพยาบาลระดับตติยภูมิขั้นสูง เขตกรุงเทพมหานคร จำนวน 110 คน คัดเลือกกลุ่มตัวอย่างแบบหลายขั้นตอน เครื่องมือที่ใช้ในวิจัย คือ แบบบันทึกข้อมูลส่วนบุคคล และข้อมูลเกี่ยวกับโรคมะเร็งลำไส้ใหญ่และทวารหนักและการรักษาที่ได้รับ แบบประเมินคุณภาพชีวิต แบบประเมินความรู้ของผู้ป่วยวัยผู้ใหญ่โรคมะเร็งลำไส้ใหญ่และทวารหนักที่มีทวารเทียม แบบสอบถามความรู้สึกมีคุณค่าในตนเอง แบบสอบถามการดูแลตนเอง และแบบสอบถามการสนับสนุนทางสังคม มีค่าความเที่ยง เท่ากับ 0.92, 0.78, 0.88, 0.95 และ0.84 ตามลำดับ วิเคราะห์ข้อมูลด้วย สถิติเชิงพรรณนา, ค่าสัมประสิทธ์สหสัมพันธ์ของเพียรสัน และสถิติการวิเคราะห์ถดถอยพหุคูณแบบขั้นตอน ผลการวิจัยสรุปได้ดังนี้ 1. คุณภาพชีวิตของผู้ป่วยวัยผู้ใหญ่โรคมะเร็งลำไส้ใหญ่และทวารหนักที่มีทวารเทียมอยู่ในระดับสูง (Mean =67.1, SD=14.56) 2. คุณภาพชีวิตของผู้ป่วยวัยผู้ใหญ่โรคมะเร็งลำไส้ใหญ่และทวารหนักที่มีทวารเทียมมีความสัมพันธ์ทางบวกในระดับสูงกับความรู้สึกมีคุณค่าในตนเอง และการดูแลตนเอง (r= 0.772 และ0.737 ตามลำดับ) ส่วนความรู้ และการสนับสนุนทางสังคมมีความสัมพันธ์ทางบวกในระดับต่ำ (r= 0.489 และ0.460 ตามลำดับ) 3. ความรู้ ความรู้สึกมีคุณค่าในตนเอง และการดูแลตนเอง ร่วมกันการทำนายคุณภาพชีวิตของผู้ป่วยวัยผู้ใหญ่โรคมะเร็งลำไส้ใหญ่และทวารหนักที่มีทวารเทียมได้ร้อยละ73.70 (R²=0.737) |
| Other Abstract: | The purposes of this predictive correlational research were to study the quality of life in colorectal cancer adult patients with colostomies, to examine their relationships of knowledge, self-esteem, self-care, social support and quality of life, and to examine the factors predicting their quality of life. The sample comprised 110 colorectal cancer adult patients with colostomies recruited from the outpatient department of a super tertiary care hospital in Bangkok by a multi-stage sampling strategy. The research instruments comprised patients’ demographic data and colorectal cancer data and treatment record forms, the quality of life index, knowledge assessment scale, self-esteem assessment scale, self-care assessment scale and social support assessment scale of colorectal cancer adult patients with colostomy’s. The reliabilities were 0.92, 0.78, 0.88, 0.95 and 0.84 respectively. Descriptive quantitative statistics, Pearson’s correlation, and stepwise multiple regression were used in statistical analyses. The major findings were as follows 1. Colorectal cancer adult patients with colostomies had a high level of quality of life. (Mean= 67.1, SD=14.56) 2. Self-esteem and self-care were significantly positively related to quality of life at the high level (r= 0.772 and 0.737 respectively). Knowledge and social support were significantly positively related to quality of life at a low level (r= 0.489 and 0.460 respectively). 3. Knowledge, self-esteem and self-care were predictors with73.70% of total variance explained for quality of life in colorectal cancer adult patients.(R²=0.737) |
