dc.description.abstract	ในปัจจุบันมีการรักษาด้วยเทคนิค V-Y plasty นั้นถูกนำ มาใช้ในการรักษา chronic Achilles tendon tear อย่างแพร่หลาย แต่ยังไม่มีการศึกษาลักษณะของ gastrocnemius insertion ที่มีส่วนเกี่ยวข้องต่อการผ่าตัดด้วยวิธีดังกล่าว การศึกษานี้มีจุดประสงศ์ที่จะศึกษาลักษณะทางกายวิภาคของ gastrocnemius insertion ที่เกี่ยวข้องกับการผ่าตัดด้วยวิธี V-Y plasty โดยเฉพาะ โดยทำการศึกษาความยาวของ gastrocnemius aponeurosis ในตำแหน่งต่างๆ ความยาวของ Achilles tendon ความกว้างและความหนาของ Achilles tendon ในตำแหน่งเหนือต่อจุดเกาะ 2 ซม.และระยะทางระหว่างขอบด้านข้างของ Achilles tendon ในตำแหน่งนี้ถึงเส้นประสาท sural โดยทำการศึกษาในขาอาจารย์ใหญ่ทั้งหมด 83 ข้าง พบว่า ลักษณะ gastrocnemius insertion ในตำแหน่ง median attachment ร้อยละ 96.3 เป็นแบบ indirect attachment คือมีจุด gastrocnemius insertion ต่ำกว่าจุดของ musculotendinous junction ซึ่งมีความเหมาะสมต่อการผ่าตัดแบบ V-Y plasty ทั้งนี้การศึกษาความยาวเฉลี่ยของ median aponeurosis พบว่ามีค่าเท่ากับ 5.6±3.1ซม. medial aponeurosis และ lateral aponeurosis มีค่าเท่ากับ 1.3±1.2ซม. และ 2.9±2.3 ซมตามลำดับ ความยาวของ Achilles tendon มีค่าเท่ากับ 21.1±2.1 ซม.ความกว้างและความหนาของ Achilles tendon ในตำแหน่ง 2 ซม. เหนือต่อจุดเกาะของ Achilles tendon มีค่าเท่ากับ 13.1±1.9 มม. และ 6.1±1.3 มม. ตามลำดับ ความหนาของ Achilles tendon ในตำแหน่งกึ่งกลางของกระดูก calcaneus มีค่าเท่ากับ 4.8±0.8 มม. จากข้อมูลดังกล่าวข้างต้นพบว่าการผ่าตัดด้วยวิธี V-Y plasty นั้นสามารถแบ่งออกได้เป็นสองกลุ่มคือกลุ่มที่ไม่ต้องทำการเลาะ gastrocnemius aponeurosis เพิ่ม พบร้อยละ 13.8 โดยกลุ่มนี้ความยาวเฉลี่ยของ median aponeurosis เท่ากับ 10.9±2.3 ซม. ซึ่งเป็นความยาวที่สามารถพลิก gastrocnemius aponeurosis มาทำการเย็บซ่อมแซมการฉีกขาดของเอ็นร้อยหวายได้เลย และกลุ่มที่ต้องทำการเลาะ gastrocnemius aponeurosis เพิ่ม พบร้อยละ 86.3โดยกลุ่มนี้มีความยาวเฉลี่ยของ median aponeurosis เท่ากับ 4.8±2.3 ซม. จึงต้องทำการเลาะ gastrocnemius aponeurosis เพิ่มอีก 4.9±2.5 ซม. เพื่อที่จะสามารถซ่อมแซมการฉีกขาดของเอ็นร้อยหวายได้ ข้อมูลของ Achilles tendon พบว่ามีความเหมาะสมต่อการใช้เป็น Achilles tendon allograft ในการผ่าตัดซ่อมแซมเอ็นข้อเข่าฉีกขาดได้เป็นอย่างดี เพราะมีเส้นเอ็นที่ยาวและหนาสามารถรองรับเทคนิคการผ่าตัดได้หลายรูปแบบ และข้อมูลระยะทางเฉลี่ยระหว่างขอบด้านข้างของ Achilles tendon เหนือต่อจุดเกาะปลาย 2 ซม. ถึงเส้นประสาท sural พบว่ามีค่าเท่ากับ 7.7±3.2มม. ซึ่งข้อมูลนี้จะช่วยลดการบาดเจ็บที่เกิดขึ้นต่อเส้นประสาท sural ระหว่างทำหัตถการต่างๆบริเวณข้อเท้าด้านนอกได้	en_US
dc.description.abstractalternative	Nowadays, the V-Y plasty or musculotendinous lengthening of gastrosoleus complex is widely use in the surgical reconstruction for chronic Achilles tendon tear. The objectives of this study were to described the pattern of gastrocnemius insertion and quantify variation in the length of gastrocnemius aponeurosis for chronic Achilles tendon reconstruction. The aim of this study included dimension of Achilles tendon for application of Achilles tendon allograft. Measurement done in 83 specimens in this study. There were 96.3% of specimens with indirect median attachment .The mean median aponeurosis length was 5.6±3.1 cm. The mean medial aponeurosis length and the mean lateral aponeurosis length were 1.3±1.2 cm. and 2.9±2.3 cm. respectively. The average length of the Achilles tendon was 21.1±2.1 cm. The mean width and thickness of the Achilles tendon at the point 2 cm. proximal to it’s insertion were 13.1±1.9 mm. and 6.1±1.3 mm. respectively. The mean thinkness of the Achilles tendon on the center of calcaneus was 4.8±0.8 mm. This data demonstrate that the V-Y plasty can divide into two groups. There were 13.8 % of specimens in the first group. The mean median apoeurosis length in the first group was 10.9±2.3 cm. In this group the median aponeurosis length was enough to reconstruction of chronic Achilles tendon tear. There were 87.3% of specimens in the second group. The mean median apoeurosis length in this group was 4.8±2.3 cm. In this group the surgeon should be extending dissection of gastrocnemius aponeurosis in 4.9±2.5 cm. for bridging the gap of chronic Achilles tendon tear. The dimension of Achilles tendon shows that Achilles tendon is suitable to use as Achilles tendon allograft for anterior cruciate ligament and posterior cruciate ligament reconstruction. It has a large bone plug that can be design to an appropriate size for both inlay and trantibial techniques. In addition, Achilles tendon thickness and length are abundant to use for both single-bundle and double-bundle reconstruction techniques. The length between lateral border of Achilles tendon to sural nerve at the point 2 cm. proximal to Achilles insertion was 7.7±3.2 mm. This data will aid the surgeon in preventing iatrogenic complication of sural nerve.	en_US
