| Abstract: | การวิจัยครั้งนี้เป็นการวิจัยเชิงพรรณนา มีวัตถุประสงค์เพื่อศึกษาต้นทุนการพยาบาลต่อหน่วย ของผู้ป่วยโรคหลอดเลือดสมอง โรงพยาบาลน่าน ศึกษาต้นทุนการพยาบาลต่อหน่วย ของผู้ป่วยโรคหลอดเลือดสมองโรงพยาบาลน่าน จำแนกตามประเภทกิจกรรมการพยาบาล และศึกษาต้นทุนการพยาบาลต่อหน่วยของผู้ป่วยโรคหลอดเลือดสมอง โรงพยาบาลน่านจำแนกตามกลุ่มผู้ป่วยโรคหลอดเลือดสมอง โดยใช้ระบบต้นทุนกิจกรรม (ActivityBasedCostingSystem:ABC) กลุ่มตัวอย่างประกอบด้วย บุคลากรทางการพยาบาล 75 ราย และผู้ป่วยโรคหลอดเลือดสมอง 102 ราย เครื่องมือวิจัยมี 4ส่วน คือ 1) คู่มือการจำแนกกลุ่มผู้ป่วยโรคหลอดเลือดสมอง 2) พจนานุกรมกิจกรรมการพยาบาลผู้ป่วยโรคหลอดเลือดสมองตามแนวคิดของแนวคิดของ Smeltzer (2008) 3) แบบบันทึกเวลาในการปฏิบัติกิจกรรมการพยาบาลและ 4) แบบบันทึกข้อมูลค่าแรงบุคลากรทางการพยาบาล เครื่องมือวิจัยทั้ง 4 ส่วน ผ่านการตรวจความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ เก็บรวบรวมข้อมูลโดยการสังเกตการปฏิบัติกิจกรรมทางการพยาบาลของบุคลากรทางการพยาบาล ที่ปฏิบัติกิจกรรมการพยาบาลแก่ผู้ป่วยโรคหลอดเลือดสมอง แล้วนำข้อมูลมาวิเคราะห์โดยใช้สถิติการแจกแจงความถี่ ร้อยละ และค่าเฉลี่ย ผลการศึกษาที่สำคัญ สรุปได้ดังนี้ 1. ค่าเฉลี่ยต้นทุนการพยาบาลต่อหน่วย ตามแนวทางการพยาบาลผู้ป่วยโรคหลอดเลือดสมอง ของผู้ป่วยโรคหลอดเลือดสมอง โรงพยาบาลน่าน มีค่าต้นทุนต่อหน่วยเท่ากับ 9,015.93 บาท 2. ค่าเฉลี่ยต้นทุนการพยาบาลต่อหน่วย ตามแนวทางการพยาบาลผู้ป่วยโรคหลอดเลือดสมอง ของผู้ป่วยโรคหลอดเลือดสมองโ รงพยาบาลน่าน จำแนกตามประเภทกิจกรรมการพยาบาล พบว่ากิจกรรมการพยาบาลทางตรง มีค่าต้นทุนต่อหน่วยมากที่สุดคือ 5,901.51 บาท รองลงมาคือกิจกรรมการพยาบาลทางอ้อมด้านบริการ เท่ากับ 2,256.16 บาท กิจกรรมการบันทึกรายงาน เท่ากับ 858.26 บาท และกิจกรรมการพยาบาลที่เกี่ยวข้องกับหน่วยงาน เท่ากับ 30.31 บาทตามลำดับ 3.ค่าเฉลี่ยต้นทุนการพยาบาลต่อหน่วยตามแนวทางการพยาบาลผู้ป่วยโรคหลอดเลือดสมอง ของผู้ป่วยโรคหลอดเลือดสมอง โรงพยาบาลน่าน จำแนกตามกลุ่มผู้ป่วยโรคหลอดเลือดสมอง พบว่า กลุ่มผู้ป่วยโรคหลอดเลือดสมองรายที่กลับมานอนโรงพยาบาลซ้ำ มีต้นทุนการพยาบาลต่อหน่วยมากที่สุดคือ 12,859.10 บาท รองลงมาคือ กลุ่มผู้ป่วยโรคหลอดเลือดสมองรายใหม่ เท่ากับ 9,207.04 บาท และกลุ่มผู้ป่วยโรคหลอดเลือดสมองที่อยู่ในระยะสุดท้าย เท่ากับ 4,426.18 บาท |
| Other Abstract: | The purpose of this descriptive research was to analyze: 1) The average unit cost of nursing care for stroke patient, 2) Unit cost of nursing care for stroke patient were direct nursing care, indirect nursing care, unit related and documentation and 3) Unit cost of nursing care for stroke patient were classified into 3 groups of the medical unit, Nan Hospital. Using Activity Based Costing System (ABC) analysis, the research population composed of 2 groups; 1) 75 nursing staff, and 2) 102 in-patients with stroke. Data were collected from December 1, 2009 to January 31, 2010. Fourth instrumental methods were utilized in this study; 1) Stroke classification guidelines, 2) The nursing activity handbook for patients with stroke, 3) Nursing activity practices record, and 4) Nursing wage expenditure record. All instruments were tested for content validity and reliability. The major results of this study were as follows: 1. The average unit cost of nursing care for stroke patient was 9,015.93 bath per patient. 2. The average unit cost of nursing care: for stroke patient were classified into 4 nursing activities; 1) the direct nursing service cost was 5,901.51 bath, 2) indirect service cost was 2,256.16 bath, 3) documentation cost was 858.26 bath and 4) other unit related costs was 30.31 bath. 3.
