| Abstract: | การวิจัยนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์ เพื่อเปรียบเทียบจำนวนวันนอนใน โรงพยาบาลของผู้ป่วยโรคหลอดเลือดสมอง และเปรียบเทียบความสามารถในการดูแลผู้ป่วยโรค หลอดเลือดสมองของผู้ดูแล ก่อนและหลังการใช้โปรแกรมการวางแผนการจำหน่ายผู้ป่วยโรค หลอดเลือดสมอง โรงพยาบาลน่าน กลุ่มตัวอย่างมี 2 กลุ่ม คือ ผู้ป่วยโรคหลอดเลือดสมอง จำนวน 20 คนที่เป็นโรคหลอดเลือดสมองชนิดตีบตัน และผู้ดูแลผู้ป่วยโรคหลอดเลือดสมอง จำนวน 20 คน ที่เข้ารับการรักษาในโรงพยาบาล ทำการสุ่มแบบเจาะจง โปรแกรมการวางแผนการจำหน่ายผู้ป่วย โรคหลอดเลือดสมอง ประกอบด้วย 1)โครงการอบรมเรื่องการวางแผนการจำหน่ายผู้ป่วยโรค หลอดเลือดสมอง โรงพยาบาลน่าน แผนการอบรมเชิงปฏิบัติการ 2) คู่มือการปฏิบัติการวาง แผนการจำหน่ายผู้ป่วยโรคหลอดเลือดสมองสำหรับพยาบาล 3) คู่มือสำหรับผู้ดูแลในการดูแล ผู้ป่วยโรคหลอดเลือดสมอง ซึ่งได้รับการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิจำนวน 5 คน ส่วนเครื่องมือที่ใช้ในการรวบรวมข้อมูลมี 2 ชุด คือ แบบบันทึกจำนวนวันนอน และแบบวัด ความสามารถในการดูแลผู้ป่วยของผู้ดูแลตรวจสอบความเที่ยงของเครื่องมือเท่ากับ .92 วิเคราะห์ ข้อมูลด้วย ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติทดสอบที (T-test) ผลการวิจัยสรุปดังนี้ 1. จำนวนวันนอนในโรงพยาบาลของผู้ป่วยโรคหลอดเลือดสมองหลังการใช้โปรแกรม การวางแผนการจำหน่ายผู้ป่วยโรคหลอดเลือดสมอง น้อยกว่า(6.20 วัน) ก่อนการใช้โปรแกรมการ วางแผนการจำหน่ายผู้ป่วยโรคหลอดเลือดสมอง อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 2. ความสามารถในการดูแลผู้ป่วยโดยรวม ของผู้ดูแล หลังการใช้โปรแกรมการวางแผน จำหน่ายผู้ป่วยโรคหลอดเลือดสมองสูงกว่าก่อนการใช้โปรแกรมการวางแผนจำหน่ายผู้ป่วยโรค หลอดเลือดสมอง อย่างมีนัยสำคัญทางสถิติ ที่ระดับ 0.05 |
| Other Abstract: | The purposes of this quasi experimental research were to compare length of hospital stays for stroke patients and caring ability of stroke patient caregivers before and after the implementation of a discharge planning program for stroke patients at Nan Hospital. Subjects were 20 stroke patients who were Ischemic stroke and 20 stroke patient caregivers who were purposively assigned. The discharge planning program included 1) project discharge planning for stroke patients in Nan Hospital, a training workshop program, 2) discharge planning guidelines for stroke patients, stroke patients discharge planning handbooks for nurses, 3) handbooks on stroke patient care for caregivers that were tested for content validity by a panel of five experts. The data collection instruments included length of hospital stay records and a caring ability of stroke patient caregivers questionnaire. The caring ability of stroke patient caregivers questionnaire had a Cronbach’s alpha coefficient of 0.92. All data were analyzed by percentage, mean, standard deviation and t-test. The research findings were as follows: 1. The length of hospital stays for stroke patients after the implementation of a discharge planning program for stroke patients were significantly lower (6.20 day) than before the implementation of the program, at the 0.5 level. 2. The overall caring ability of stroke patient caregivers after the implementation of a discharge planning program for stroke patients was significantly higher than before the implementation of the program, at the 0.5 level |
