| Abstract: | การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาสมรรถนะพยาบาลวิชาชีพหน่วยผู้ป่วยวิกฤตหัวใจและหลอดเลือด โดยใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลเป็นผู้เชี่ยวชาญด้านการดูแลผู้ป่วยวิกฤตหัวใจและหลอดเลือด จำนวน 19 คน ประกอบด้วยแพทย์ผู้เชี่ยวชาญด้านการดูแลผู้ป่วยวิกฤตหัวใจและหลอดเลือด พยาบาลหัวหน้าหอผู้ป่วยวิกฤตหัวใจและหลอดเลือด พยาบาลวิชาชีพระดับปฏิบัติการหน่วยผู้ป่วยวิกฤตหัวใจและหลอดเลือด และอาจารย์ผู้สอนหรือรับผิดชอบหลักสูตรการดูแลผู้ป่วยวิกฤตหัวใจและหลอดเลือด ในโรงพยาบาลระดับตติยภูมิ วิธีดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ ขั้นตอนที่ 1 สัมภาษณ์เกี่ยวกับสมรรถนะพยาบาลวิชาชีพหน่วยผู้ป่วยวิกฤตหัวใจและหลอดเลือด ขั้นตอนที่ 2 นำข้อมูลที่ได้มาวิเคราะห์เนื้อหาแล้วนำมาสร้างแบบสอบถามให้ผู้เชี่ยวชาญแสดงความคิดเห็นเกี่ยวกับระดับความสำคัญของข้อคำถามแต่ละข้อ และขั้นตอนที่ 3 นำข้อมูลที่ได้มาคำนวณค่ามัธยฐาน ค่าพิสัยระหว่างควอไทล์และส่งแบบสอบถามไปให้ผู้เชี่ยวชาญยืนยันคำตอบ หลังจากนั้น นำข้อมูลที่ได้มาคำนวณค่ามัธยฐาน ค่าพิสัยระหว่างควอไทล์เพื่อสรุปผลการวิจัย ผลการวิจัยพบว่า สมรรถนะของพยาบาลวิชาชีพหน่วยผู้ป่วยวิกฤตหัวใจและหลอดเลือด ประกอบด้วยสมรรถนะ 5 ด้าน ดังนี้ 1) ด้านการปฏิบัติการพยาบาลเฉพาะทางผู้ป่วยวิกฤตหัวใจและหลอดเลือด 2) ด้านการบริหารยาโรคหัวใจและหลอดเลือด 3) ด้านการพัฒนาคุณภาพการรักษาพยาบาลผู้ป่วยวิกฤตหัวใจและหลอดเลือด 4) ด้านการจัดการทางการพยาบาลผู้ป่วยวิกฤตหัวใจและหลอดเลือด และ 5) ด้านการตัดสินใจทางจริยธรรมในการพยาบาลผู้ป่วยวิกฤตหัวใจและหลอดเลือด |
| Other Abstract: | The purpose of this study was to explore the nursing competency in coronary care unit .The subject were 19 experts in critical coronary care including cardiologist, nurse manager of coronary care unit, professional nurse who expert in coronary care unit and instructor who teach or responsible in critical coronary care curriculum. The Delphi technique consisted of 3 steps. Step 1, all experts were described about the nursing competency in coronary care unit. Step 2, data were analyzed by using content analysis for developing the rating scales questionnaire. All items in the questionnaire were ranked the level of competency by a prior panel of expert. Step 3, data were analyzed by using median and interquatile range which was developed a new version of questionnaire. The new questionnaire was sent to previous experts for confirm the previous ranked items. Data were analyzed again by median and interquatile range to summarize the study. The results of the study were presented that the nursing competency in coronary care unit consisted of 5 components as follow: 1) Nursing specialty of critical cardiovascular patients 2) Administration of cardiovascular drug 3) Quality development in critical coronary care 4) Nursing management of critical coronary care and 5) Ethical decision making in patient with critical coronary care. |
