| Abstract: | การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาผลการบริบาลทางเภสัชกรรมผู้ป่วยในที่โรงพยาบาลชุมชนจัตุรัส ระหว่าง 16 สิงหาคม ถึง 15 ธันวาคม 2542 โดยเภสัชกรดำเนินงานดูแลผู้ป่วยอย่างต่อเนื่องตั้งแต่รับผู้ป่วยเข้าพักรักษา จำหน่ายและเมื่อผู้ป่วยกลับมาติดตามผลการรักษาที่คลินิกผู้ป่วยนอกอีกครั้ง และประเมินความพึงพอใจของผู้ป่วยที่พักรักษาตัวในโรงพยาบาลตั้งแต่ 3 วันขึ้นไป ในช่วงที่ทำการศึกษา เภสัชกรสามารถให้การบริบาลทางเภสัชกรรมแก่ผู้ป่วยใน 1,038 ราย พบปัญหาที่เกี่ยวข้องกับการใช้ยา 328 ปัญหาในผู้ป่วย 250 ราย ปัญหาที่พบมากที่สุดคืออันตรกิริยาระหว่างยากับยา 92 ปัญหา (ร้อยละ 28.05) การไม่ได้รับยาตามแพทย์สั่ง 74 ปัญหา (ร้อยละ 22.56) และอาการไม่พึงประสงค์จากการใช้ยา 44 ปัญหา (ร้อยละ 13.41) ตามลำดับ เภสัชกรประสานงานกับผู้เกี่ยวข้องตามสาเหตุของปัญหา พบว่าแก้ไขปัญหาได้ 180 ปัญหา (ร้อยละ 54.88) ป้องกันได้ 65 ปัญหา (ร้อยละ 19.82) ต้องติดตามเฝ้าระวังอย่างใกล้ชิด 58 ปัญหา (ร้อยละ 17.68) และไม่สามารถแก้ไขได้ 25 ปัญหา (ร้อยละ 7.62) มีผู้ป่วย 160 ราย ที่ได้รับการบริบาลต่อเนื่องจากการเป็นผู้ป่วยในแล้วมารับการบริการต่อที่คลินิกผู้ป่วยนอก จากการประเมินความพึงพอใจของผู้ป่วยต่อกิจกรรมการบริบาลทางเภสัชกรรมของเภสัชกรจำนวน 186 ราย พบว่าผู้ป่วยส่วนมากมีความพึงพอใจในระดับมาก อายุเป็นปัจจัยเดียวที่มีความสัมพันธ์กับความพึงพอใจของผู้ป่วยอย่างมีนัยสำคัญทางสถิติ (P<0.05) ส่วนปัจจัยอื่น เช่น เพศ การศึกษา อาชีพ ค่ารักษาพยาบาล โรคเรื้อรัง และระยะเวลาที่เข้าพักรักษาตัวโรงพยาบาลไม่มีความสัมพันธ์กับความพึงพอใจของผู้ป่วยอย่างมีนัยสำคัญทางสถิติ (P>0.05) การปฏิบัติงานบริบาลทางเภสัชกรรมของเภสัชกรร่วมกับบุคลากรทางการแพทย์อื่นๆ สามารถค้นพบ แก้ไข หรือป้องกันปัญหาที่เกี่ยวข้องกับการใช้ยาได้ และผู้ป่วยได้รับประโยชน์สูงสุดจากการใช้ยาเป็นการเพิ่มคุณภาพในการดูแลผู้ป่วย |
| Other Abstract: | The purpose of this study was to determine the output of a pharmacist's pharmacetical care services at Chaturus Community Hospital during August 16, to December 15, 1999. The services aimed to provide continuity of care starting when the patients were admitted, discharged untel their follow-up visit at outpatient clinic. The satisfaction of the patients who stayed in hospital over 3 days were assessed. During studying, 1038 patients were offered the inpatient pharmaceutical care services, 328 drug related problems (DRPs) were identified in 250 patients. The most commonly found DRPs were : 92 drug-drug interaction problems (28.05%), 74 failure to receive medication problems (22.56%) and 44 adverse drug reaction problems (13.41%), respectively. After co-ordination with health care team, 180 problems (54.88%) were resolved, 65 problems (19.82%) could be prevented, 58 problems (17.68%) should be monitored closely and 25 problems (7.62%) were unable to resolve. There were 160 patients received continuity of pharmaceutical care services in the first follow-up visit at outpatient clinic. One hundred and eighty-six patients were assessed for their satisfactions toward pharmaceutical care that they received. Most patients ranked their satisfactions at high level. Age was the only factor that had statistically significant correlation with the patient satisfaction (P<0.05), but other factors such as sex, education, occupations, cost of treatment, chronic disease and length of stay showed no significant correlation. (P>0.05) The inpatient pharmaceutical care services done by the pharmacist in co-ordination with other health care professionals can identify, resolve or prevent drug related problems. The patients will receive maximal benefit from the drug consumption. This can lead to the improvement of patient care. |
