| Abstract: | การศึกษานี้เป็นการวิจัยเชิงพรรณนา มีวัตถุประสงค์เพื่อ ศึกษาความต้องการการดูแลของผู้สูงอายุที่มีภาวะสมองเสื่อมระยะเริ่มต้น ระยะกลางและระยะสุดท้าย โดยใช้แนวคิดความต้องการการดูแลผู้ป่วยสมองเสื่อมของ McWalter (1994) กลุ่มตัวอย่างคือผู้สูงอายุทั้งเพศชายและเพศหญิงที่มีอายุตั้งแต่ 60 ปีขึ้นไป ที่มารับบริการในแผนกผู้ป่วยนอกและได้รับการวินิจฉัยจากแพทย์ ว่ามีภาวะสมองเสื่อมจำนวนทั้งหมด 160 ราย เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ได้แก่ แบบประเมินสมรรถภาพสมองเบื้องต้นฉบับภาษาไทย (MMSE T - 2002) แบบประเมินระดับความรุนแรงของสมอง (CDR scale) แบบประเมินความต้องการการดูแลของผู้ป่วยสมองเสื่อม (Care Need Assessment in Dementia) ผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ ได้ค่าความตรงตามเนื้อหา เท่ากับ .87 ค่าความเที่ยง เท่ากับ .97 ข้อมูลที่ได้นำไปวิเคราะห์ค่าความถี่ ร้อยละ ค่าเฉลี่ยและความแปรปรวนตัวเดียวแบบวัดซ้ำ (Anova) ผลการวิจัยสรุปดังนี้ 1. ผู้สูงอายุที่มีภาวะสมองเสื่อมจำนวน 160 ราย พบเพศหญิง 108 ราย คิดเป็นร้อยละ 68 มีอายุเฉลี่ย 72.5 ปี มีอายุอยู่ระหว่าง 71 - 80 ปี และ 81 - 90 ปี มากที่สุด คิดเป็นร้อยละ 74 มีโรคประจำตัว 3 โรคคือ เบาหวาน ความดันโลหิตสูงและไขมันในเลือดสูง คิดเป็นร้อยละ 25 2. ผู้ป่วยสมองเสื่อมทั้ง 160 ราย จำแนกตามระยะ ได้แก่ ระยะเริ่มต้น 65 ราย คิดเป็นร้อยละ 41 ระยะปานกลาง 52 ราย คิดเป็นร้อยละ 32 และระยะสุดท้าย 43 ราย คิดเป็นร้อยละ 26.9 3. ความต้องการการดูแลในภาพรวมอยู่ในระดับปานกลาง เมื่อจำแนกตามระดับความต้องการ พบว่า ความต้องการการดูแล มาก คือ ความต้องการการดูแลด้านการใช้ชีวิตในชุมชน ด้านความคิดความจำ ด้านการมีปฏิสัมพันธ์ทางสังคมและด้านการดูแลบ้าน 4. ผลการเปรียบเทียบความต้องการการดูแลระหว่างกลุ่มผู้สูงอายุทั้ง 3 ระยะ พบว่า มีค่าเฉลี่ยความต้องการการดูแล ด้านการดูแลบ้าน ของผู้ป่วยสมองเสื่อมกลุ่มระยะเริ่มต้น มากกว่า กลุ่มระยะสุดท้าย ที่ระดับ .05 |
| Other Abstract: | This descriptive research aim to study care needs in older person with early, moderate and late stage dementia. The conceptual framework of this research based on concept care need in dementia of McWalter (1994). The participants consisted of 160 people with dementia from OPD department of 4 hospitals that diagnosis with dementia. The research screening instruments were Mini Mental Status Exam (MMSE-T 2002), Clinical Dementia Rating Scale (CDR) and Care Needs Assessment In Dementia. Validated by 5 experts,yielding CVI .87 and α reliability .97. The data analysis were descriptive statistics and one - way repreated ANOVA The results suggesed that 1. elderpersons with dementia were 160 cases by sex. Female 108 cases (68%) male, 52 (32%), mean age 72.5 years, between 71-80 years and 81-90 years (74%), approximately 25% of the underlying 3 disease, diabetes, hypertension and hyper lipidemia. 2. 65 persons (40.6%) were in early stage, 52 persons (32.5%) were in moderate stage and 43 persons (26.9%) were in late stage. 3. care needs as a whole, with the care needs of the community living, cognitive memory, social interaction and in care homes, on the other side there are 3 more care needs are moderate. 4.
