| Abstract: | การวิจัยนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อศึกษาผลของการพยาบาลโดยใช้โปรแกรมการจัดการตนเองต่อสมรรถภาพปอดและค่า C-reactive protein ในผู้ป่วยโรคปอดอุดกั้นเรื้อรัง โดยประยุกต์ใช้แนวคิดการจัดการตนเองของ Tobin และคณะ (1986) และการกำกับตนเองของ Kanfer and Goldstein (1980) เป็นกรอบแนวคิดในการวิจัย โปรแกรมประกอบด้วย 1. การประเมินปัญหาและความต้องการของผู้ป่วย 2. การวางแผนและปฏิบัติ 3. การติดตามประเมินผล กลุ่มตัวอย่าง คือ ผู้ป่วยโรคปอดอุดกั้นเรื้อรังที่เคยมีประสบการณ์อาการกำเริบจำนวน 60 คน เลือกเข้ากลุ่มควบคุมและกลุ่มทดลอง จับคู่ให้กลุ่มตัวอย่างมีความคล้ายคลึงกันในเรื่องอายุ ระดับความรุนแรงของโรคและประวัติการสูบบุหรี่ โดยที่กลุ่มควบคุมได้รับการพยาบาลตามปกติ และกลุ่มทดลองได้รับโปรแกรมการจัดการตนเอง เก็บรวบรวมข้อมูลโดยใช้แบบวัดการรับรู้อาการหายใจลำบาก เครื่องตรวจวัดระดับ C-reactive protein และ เครื่องตรวจสมรรถภาพปอดแบบพกพา โดยการวัดอัตราไหลสูงสุดของอากาศขณะหายใจออกและปริมาตรอากาศที่หายใจออกอย่างแรงใน 1 วินาที วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณา สถิติทดสอบค่าที (t-test) และสถิติทดสอบค่าซี (Z-test) ผลการวิจัยสรุปได้ดังนี้ 1.ค่าเฉลี่ยของอัตราไหลสูงสุดของอากาศขณะหายใจออกและปริมาตรอากาศที่หายใจออกอย่าง แรงใน 1 วินาทีของกลุ่มทดลองภายหลังเข้าร่วมโปรแกรมการจัดการตนเองสูงกว่าก่อนเข้าร่วมโปรแกรม อย่างมีนัยสำคัญทางสถิติ ที่ระดับ .05 2.ค่าเฉลี่ยของอัตราไหลสูงสุดของอากาศขณะหายใจออกและปริมาตรอากาศที่หายใจออกอย่าง แรงใน 1 วินาทีของกลุ่มที่เข้าร่วมโปรแกรมการจัดการตนเองสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติ ที่ระดับ .05 3.สัดส่วนของผู้ป่วยที่มี C-reactive protein ค่าบวก (> 5 mg./L) ภายหลังการทดลองของกลุ่มที่เข้าร่วมโปรแกรมการจัดการตนเองน้อยกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างไม่มีนัยสำคัญทางสถิติ |
| Other Abstract: | The purposes of this quasi experimental research were to examine the effects of nurse-led self–management program on lung function and C –reactive protein in chronic obstructive pulmonary disease patients. The Tobin’s self-management model and Kanfer’s self regulation technique were used as a conceptual framework. The participants consisted of 60 patients with COPD who experienced exacerbation: 30 participants were assigned to the control group and another 30 were in the experimental group. Both groups were matched in terms of age, disease severity and smoking history. The control group received conventional nursing care while the experimental group received the nurse-led self–management program. Dyspnea scale, lung function and C-reactive protein were assessed using Modified Borg’s scale, PiKo-1 Electronic Peak Flow Meter & FEV1 Meter and i-CHROMATM CRP test . Data were analyzed using descriptive statistic, t-test and Z-test The major findings were as follows: 1.The mean peak expiratory flow rate and mean forced expiratory volume per second of the experimental group post participating nurse-led self–management program were significantly higher than those at base line (p < .05). 2.The mean peak expiratory flow rate and mean forced expiratory volume per second of the experimental group participating nurse-led self–management program were significantly higher than those of the control group (p < .05). 3.There was no significant difference in the proportion of patients with C-reactive protein > 5 mg/L in the control group and the experiment group. |
