| Abstract: | การวิจัยนี้มีจุดมุ่งหมาย เพื่อศึกษาปัญหาการขาดแคลนแพทย์ในส่วนภูมิภาค โดยศึกษาความสัมพันธ์ระหว่าง การเลือกสถานที่ทำงานของแพทย์หลังจากทำงานใช้ทุนเสร็จแล้ว กับ องค์ประกอบด้านภูมิหลัง ด้านปัญหาในการทำงาน และด้านเหตุผลส่วนตัว และศึกษาลักษณะสถานที่ทำงานในอุดมคติของแพทย์ กลุ่มตัวอย่างการวิจัยได้แก่ แพทย์รับทุนรัฐบาลที่เคยทำงานใช้ทุนในต่างจังหวัด จำนวน 561 คน เป็นแพทย์ซึ่งปัจจุบันทำงานในกรุงเทพฯ 101 คน และทำงานในต่างจังหวัด 460 คน เครื่องมือที่ใช้เก็บรวบรวมข้อมูลเป็นแบบสอบถามที่ผู้วิจัยสร้างขึ้น การวิเคราะห์ข้อมูลโดยคำนวณหาสัมประสิทธิ์พี (Ø) สัมประสิทธิ์สหสัมพันธ์แบบ point-biserial ค่ามัชฌิมเลขคณิต การทดสอบซี การทดสอบที และการวิเคราะห์เนื้อเรื่อง ผลการวิจัยได้ข้อสรุปที่สำคัญดังนี้ 1. การเลือกสถานที่ทำงานของแพทย์ สัมพันธ์กับภูมิลำเนาเดิม ที่ตั้งสถานศึกษาระดับประถมศึกษาตอนปลาย ระดับมัธยมศึกษาตอนต้น ระดับการศึกษาแพทยศาสตร์ และสถานที่ทำงานเป็นแพทย์ฝึกหัด กล่าวคือ แพทย์ที่มีภูมิหลังอยู่ในต่างจังหวัด เลือกทำงานในต่างจังหวัดมากกว่าแพทย์ที่มีภูมิหลังในกรุงเทพฯ ส่วนเพศและที่ตั้งสถานศึกษาระดับมัธยมศึกษาตอนปลายไม่สัมพันธ์กับการเลือกสถานที่ทำงานของแพทย์ 2. การเลือกสถานที่ทำงานของแพทย์ สัมพันธ์กับปัญหาการทำงานในระหว่างทำงานใช้ทุนในต่างจังหวัด ในบางเรื่องซึ่งได้แก่ ปัญหาเรื่องขาดความรู้ด้านการบริหารงานความขัดแย้งในการประสานงานกับฝ่ายปกครองของอำเภอ ขาดเจ้าหน้าที่วิทยาศาสตร์การแพทย์ ขาดยา ขาดเครื่องมือเครื่องใช้ และขาดบุคลากร กล่าวคือ แพทย์ที่ย้ายไปทำงานในกรุงเทพฯ มีปัญหาการทำงานในระหว่างที่ทำงานใช้ทุน มากกว่า แพทย์ที่ยังคงทำงานในต่างจังหวัด 3. เหตุผลส่วนตัวที่ทำให้แพทย์เลือกทำงานในกรุงเทพฯ หลังหมดสัญญาการทำงานใช้ทุนต่างจังหวัด ได้แก่ ต้องการอยู่ใกล้แหล่งวิชาการ ต้องการทำงานเฉพาะด้านรักษาพยาบาล โอกาสในการศึกษาต่อมีมาก ต้องการอยู่ในที่มีความสะดวกสบาย บิดามารดาอยู่ในกรุงเทพฯ ไม่ต้องการทำงานด้านบริหารงาน คู่สมรส (คู่รัก) อยู่ในกรุงเทพฯ และมีโอกาสหารายได้พิเศษได้มาก 4. เหตุผลส่วนตัวที่ทำให้แพทย์เลือกทำงานในต่างจังหวัด หลังจากหมดสัญญาการทำงานใช้ทุนต่างจังหวัด ได้แก่ ต้องการใช้ความรู้ให้เป็นประโยชน์แก่คนต่างจังหวัด ชอบชีวิตความเป็นอยู่ในต่างจังหวัด ไม่ชอบชีวิตความเป็นอยู่ในกรุงเทพฯ และชอบทำงานที่มีทั้งงานรักษาพยาบาลและงานอนามัย 5. ที่ทำงานในอุดมคติของแพทย์ มีลักษณะดังนี้ คือ มีความปลอดภัยมีโอกาสได้ช่วยเหลือคนไข้ มีสวัสดิการที่พักอาศัย มีอุปกรณ์การรักษาพยาบาลและยาเพียงพอ ตั้งอยู่ในที่การคมนาคมสะดวก มีโอกาสก้าวหน้าทางวิชาการ มีโอกาสก้าวหน้าทางวิชาชีพและสะดวกในการหาสถานที่เรียนที่มีคุณภาพให้บุตร |
| Other Abstract: | The purpose of this research was to study relationships between backgrounds, working problems and personal reasons of physicians and their selection of working location. The aim was to find an explanation for the shortage of physicians in the provincial areas. Sampling data were obtained 561 scholarship physicians, 101 of whom are now working in Bangkok, and the other 460 physicians were in the provinces. The questionnaire designed form by the researcher was used to collect the data. The coefficient phi (Ø), the point-biserial correlation coefficient, Arithematic mean, z-test, t-test and content analysis were employed in the analysis of the data. The research conclusions were as follow: 1. The selection of working location was related to such background factors as the place of birth, place of upper primary and lower secondary education, place of medical school and internship. The place of upper secondary education and sex were above their consideration. Physicians who have provincial backgrounds were more likely to work in the provincial areas, in contrast with those who have backgrounds in Bangkok areas. 2. The problems facing physicians working in provincial areas were the lack of knowledge in administration, and conflicts with district government officials. Inadequacy of medicine, medical instruments, and technicians were additional problems. Provincial physicians who later moved to work in Bangkok, had more problems than those who remain working in provincial areas. 3. Personal reasons for selecting to work in Bangkok stem from the fact that academic resources were readily available, and the preference for works in the curative field. Furthemore they have better opportunity of further their study. The reasons were that their parents and spouse lived in Bangkok; they didn’t want an administrative post ; and they could earn much money from working over time. 4. Personal reasons for selecting to work in provincial areas were that they would like to make useful contribution to the villages, the preferred living in provincial areas to Bangkok, and they wish to work both in curative and preventive care. 5. Their ideal working places were determined by security factors, the opportunity to take care of the patients, residential welfare, good transportation, adequacy of equipment and medical supply, educational and promotional opportunity. Besides, schooling opportunity should be available for their children. |
