| Abstract: | การวิจัยแบบกึ่งทดลองครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการวางแผนจำหน่ายต่อพฤติกรรมการป้องกันโรคอุจจาระร่วงของผู้ดูแลในเด็กอายุต่ำกว่า 5 ปี โดยใช้แนวคิดการวางแผนจำหน่ายของกองการพยาบาล กระทรวงสาธารณสุข ( 2539 ) ตามรูปแบบ M-E-T-H-O-D กลุ่มตัวอย่าง คือ ผู้ดูแลเด็กในโรงพยาบาลเอกชนแห่งหนึ่งจำนวน 30 คน แบ่งเป็นกลุ่มทดลอง 15 คน และกลุ่มควบคุม 15 คน โดยจับคู่ให้มีความคล้ายคลึงกันในด้าน การศึกษา อายุ และประเภทของผู้ดูแล กลุ่มทดลองได้รับโปรแกรมการวางแผนจำหน่าย ที่ประกอบด้วย 5 ขั้นตอน คือ 1) การประเมินปัญหาและความต้องการการดูแลภายหลังจำหน่าย 2) การวินิจฉัยปัญหาสุขภาพและความต้องการการดูแลภายหลังจำหน่าย 3) การกำหนดแผนการจำหน่ายผู้ป่วย 4) การปฏิบัติตามแผนการจำหน่าย และ 5) การประเมินผล โดยมีคู่มือการวางแผนจำหน่ายการป้องกันโรคอุจจาระร่วงในเด็ก แผนการสอนเรื่องการป้องกันโรคอุจจาระร่วงในเด็ก ภาพพลิก และคู่มือการป้องกันโรคอุจจาระร่วงในเด็ก ประกอบการดำเนินกิจกรรม เก็บรวบรวมข้อมูล โดยใช้แบบสอบถามพฤติกรรมการป้องกันโรคอุจจาระร่วง ที่มีค่าดัชนีความตรงตามเนื้อหาเท่ากับ 0.97 และสัมประสิทธิ์อัลฟาครอนบาคเท่ากับ 0.89 วิเคราะห์ข้อมูลโดยใช้สถิติ Dependent t-test และ Independent t-test ผลการวิจัย พบว่า 1.ค่าเฉลี่ยคะแนนพฤติกรรมการป้องกันโรคอุจจาระร่วงของผู้ดูแลเด็กหลังได้รับโปรแกรม ( x=125.73 +8.63) สูงกว่าก่อนได้รับโปรแกรมการวางแผนจำหน่าย ( x=118.40 +11.36) อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (t = -2.651, df =14, p < 0.05) 2.ค่าเฉลี่ยความต่างของคะแนนพฤติกรรมการป้องกันโรคอุจจาระร่วงของผู้ดูแลเด็กที่ได้รับโปรแกรมการวางแผนจำหน่าย ( d =7.3 3 +10.71) สูงกว่ากลุ่มควบคุมที่ได้รับการพยาบาลตามปกติ ( d = -5.60 +15.41) อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (t = 2.699, df =28, p < 0.05) |
| Other Abstract: | The purpose of this quasi-experimental research was to examine the effect of a discharge planning program on diarrhea preventive behavior of caregiver for children under five years old. A discharge planning concept of M-E-T-H-O-D developed by Bureau of Nursing Ministry of Public Health was used as a conceptual framework. The sample were 30 caregivers of diarrhea children in private hospital, 15 per group. The experimental group received a discharge planning program and the control group received routine nursing care. Subjects were matched by education level, age, and their relation to children. The intervention consisted of five steps: 1) Problems and needs assessment for caring after discharge, 2) Identification of health problems and care needs after discharge, 3) Discharge planning for patients, 4) Implementation of discharge planning, and 5) Evaluation. Data were collected using diarrhea preventive behavior questionnaire. Its content validity index was 0.97 and Cronbach ’s alpha coefficient was 0.89. Data were analyzed using dependent t-test and independent t-test. It was found that: 1.The mean score of diarrhea preventive behaviors of caregivers for children with diarrhea after receiving discharge planning program (x =125.73 +8.63 ) was significantly higher than before receiving the program (x =118.40 +11.36) ) (t =-2.651, df = 14, p <0.05). 2.The different score mean of diarrhea preventive behaviors of caregivers for children with diarrhea who received discharge planning program ( d= 7.33 + 10.71) was significantly higher than that of the control group (d = -5.60 + 15.41) (t = 2.699, df =28, p < 0.05). |
