Abstract: The project of “Child health (under 5 years old) information system development for holistic health care in health center” was operated by Dokkamtai District Health office Phayao Province. The objectives of this project were: 1. To develop information system that serves comprehensive and continuum public health service in 0 – 5 years old subject. 2. To obtain essential information of target group for a complete, timely, and precise reporting to superior 3. To reduce workload of information by applying minimal work load but high efficiency information technology.Methods This project was applied AIC technique to encourage all the personnel of the organization to participate in work improvement. The development emphasizes on human resource as a center. Initially, the information system of a target group consists of 0 to 5 year-old children to develop a system that serves a lifetimeResult The information system development with human-centered emphasis e.g. the 0 to 5 year-old target group, is a personal recording system. The recording system has designed a framework of information that is simple to record and file, with continuity for a lifetime, and integrated all standardized basic services of a health center with family and social context. By the way, there was an analysis of the basic service activities and the analysis of information required. The requirement was for planning, monitoring, and evaluation of services and impacts.Discussion and conclusion This project could achieve its goals. It has information of the target group (0 – 5 year) individually both in traits and quantity aspect, which is utilizable in service planning for the target group. The information could be used to follow up the outcome, assess the coverage of service, and quantity of service activity and could be continuously used for the target group from their birth to the present, which truly leads to the holistic care. The obtained information can be analyzed and processed for other organizations as required or routine reporting more precisely and quickly. Database with information technology could greatly reduce workload of health workers. They could spend more time for care service.
