| Abstract: | จุดประสงค์: เพื่อศึกษาการสูญเสียน้ำทางผิวหนังเมื่อทาสารประกอบน้ำมันบนผิวหนังทารกคลอดก่อนกำหนดที่อยู่ในตู้อบและมีภาระตัวเหลืองรักษาด้วยการส่องไฟ รูปแบบการศึกษา: การศึกษาเชิงทดลอง สถานที่ศึกษา: หน่วยทารกแรกเกิด โรงพยาบาลจุฬาลงกรณ์ ประชากรที่ศึกษา: เด็กทารกแรกเกิดคลอดก่อนกำหนด อายุครรภ์น้อยกว่าหรือเท่ากับ 34 สัปดาห์ อายุไม่เกิน 7 วัน ที่มีภาวะตัวเหลืองได้รับแสงบำบัดอยู่ในตู้อบ (Incubator) แต่ไม่มีภาวะหายใจเร็ว หรือใช้เครื่องช่วยหายใจ วิธีการศึกษา: ศึกษาเด็กทารกแรกเกิดคลอดก่อนกำหนดจำนวน 30 คน ทาสารประกอบน้ำมันปริมาณ 1.5 ซี.ซี บนผิวหนังบริเวณลำตัวและแขนขาซีกขวาในขณะที่นอนคว่ำ โดยร่างการซีกซ้ายไม่ได้ทาน้ำมัน วัดค่าการสูญเสียน้ำทางผิวหนัง (TEWL) อุณหภูมิอากาศเหนือผิวหนัง ความชื้นสัมพัทธ์อากาศเหนือผิวหนัง ก่อนทาน้ำมันและหลังทาน้ำมันที่ 30 นาที และ 4-6 ชั่วโมง โดยวัดร่างกายทั้งซีกขวาและซ้าย 3 ตำแหน่งคือ ต้นแขน หลังและน่อง นำมาเปรียบเทียบว่า ณ เวลาต่าง ๆ มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติหรือไม่ โดยหาค่า P value ที่ความเชื่อมั่น 95% โดยใช้สถิติ Paired student T-test ผลการศึกษา: พบว่าการทาสารประกอบน้ำมันบนผิวหนังทารกขณะให้แสงบำบัด สามารถลดการสูญเสียน้ำทางผิวหนัง (TEWL) ใน 6 ชั่วโมงแรก อย่างมีนัยสำคัญทางสถิติ โดยที่เวลา 30 นาทีหลังทาน้ำมันพบว่า ผิวหนังสูญเสียน้ำลดลงร้อยละ 29 (P value < 0.0024) และที่เวลา 4-6 ชั่วโมงหลังทาน้ำมันผิวหนังสูญเสียน้ำลดลงร้อยละ 26 (P value < 0.0109) อุณหภูมิอากาศเหนือผิวหนังและความชื้นสัมพัทธ์อากาศเหนือผิวหนังข้างที่ทาน้ำมันไม่แตกต่างจากข้างที่ไม่ทาน้ำมัน (P value > 0.18) หลังจากทาน้ำมันบนผิวหนังไม่พบลักษณะผื่นแดงหรือขุยที่ผิวหนัง สรุป: การทาน้ำมันบนผิวหนังทารกคลอดก่อนกำหนด ที่มีภาวะตัวเหลืองได้รับแสงบำบัด มีผลช่วยลดการสูญเสียน้ำทางผิวหนังอย่างมีนัยสำคัญทางสถิติ โดยไม่มีผลต่อการเปลี่ยนแปลงอุณหภูมิอากาศเหนือผิวหนังขณะให้แสงบำบัด |
| Other Abstract: | Objective: To determine the effects of clear topical ointment on transepidemal water loss in preterm infants receiving phototherapy. Design : Experimental study. Setting : Neonatal Unit Department of Pediatrics, King Chulalongkorn Memorial Hospital. Population : Jaundiced preterm infants, gestational age <34 weeks and postnatal age <7 days, received conventional phototherapy in incubator. Infants with respiratory distress were excluded. Interventions : Thirty preterm infants were included. 1.5 ml of clear topical ointment was applied on the dorsal surface of only the right side of the trunk and extremities while the left side received no topical treatment. Data collection included transepidermal water loss (TEWL) measurement, ambient temperature and ambient humidity before and at 30 min., 4-6 hours after application of ointment during phototherapy. The measurements were executed both the right and the left side in 3 positions; upper arm, back, lower leg. The collected data was compared by using statistic method, paired student T-test. Result : Clear topical ointment therapy significantly decreased TEWL in the first six hours after application during phototherapy. TEWL waa reduced by 29% (P value < 0.0024) when measured at 30 minutes after the application and 26% (P value < 0.0109) at 4-6 hours after the application. Ambient temperature and humidity were not significantly different (P value > 0.18). There was no rash or scale after application of the ointment. Conclustion : The application of clear topical ointment on the skin of jaundiced preterm infants, receiving phototherapy, reduced TEWL significantly. In addition, there was no effects concerning ambient temperature during phototherapy |
