| Abstract: | การศึกษานี้เป็นการศึกษาต้นทุนคุณภาพ ของการจัดบริการดูแลสุขภาพมารดาและทารก ของเครือข่ายโรงพยาบาลที่ขึ้นทะเบียนเป็นหน่วยบริการของสำนักงานหลักประกัน สุขภาพ ประจำจังหวัด ในระบบหลักประกันสุขภาพถ้วนหน้าของประเทศไทย ใช้วิธีที่ใช้ในการเก็บข้อมูลต้นทุนคุณภาพ คือ Prevention-Appraisal-Failure cost element method ( PAF model ) จากแหล่งข้อมูลดังนี้ ฐานข้อมูลการเบิกจ่ายค่ารักษาพยาบาลผู้ป่วยใน ฐานข้อมูลการจ่ายเงินชดเชยตามมาตรา 41 ฐานข้อมูลการบริหารจัดการงบประมาณ จากสำนักงานหลักประกันสุขภาพแห่งชาติ ฐานข้อมูลการสำมะโนประชากร จากสำนักงานสถิติแห่งชาติ ฐานข้อมูลทรัพยากรสุขภาพ จากกระทรวงสาธารณสุข เก็บข้อมูลตั้งแต่ปี 2550-2554 ประมวลผลในระดับจังหวัด ผลการศึกษาใช้มูลค่าปี พ.ศ 2550 เป็น ปีฐาน พบว่า ต้นทุนป้องกัน คิดเป็นมูลค่าเฉลี่ย 297 บาทต่อประชากร สิทธิหลักประกันสุขภาพ ( UC ) 1 รายต่อปี ต้นทุนการประเมิน คิดเป็นมูลค่าเฉลี่ย 22 บาทต่อประชากร UC 1 รายต่อปี และต้นทุนความสูญเสีย คิดเป็นมูลค่าเฉลี่ย 71 บาท ต่อประชากร UC 1 รายต่อปีหรือ เฉลี่ย 3,323 บาทต่อการเข้ารับบริการผู้ป่วยใน และพบว่าต้นทุนทั้งสามกลุ่ม มีแนวโน้มสูงขึ้น ทุกปี ทั้งนี้การวิเคราะห์ข้อมูลทางสถิติด้วยสมการเชิงเส้นแบบ Generalized Estimating Equations ( GEE ) พบว่า ต้นทุนป้องกัน และต้นทุนการประเมิน มีความสัมพันธ์แปรผันตรงกับการเพิ่มขึ้นของต้นทุนความสูญเสีย ( p – value < 0.001 ) สะท้อนว่ากรอบแนวคิดของการศึกษาต้นทุนคุณภาพตามรูปแบบ PAF Model อาจไม่สามารถนำมาประยุกต์ใช้ในประเมินผลการบริหารคุณภาพของระบบบริการสุขภาพ ได้โดยตรง ซึ่งอาจเป็นผลจากการให้ความสำคัญต่อความสูญเสียอย่างสูงในบริการสุขภาพ รวมถึงข้อจำกัดในการตีมูลค่าของต้นทุนความสูญเสียในมุมมองของหน่วยงาน ผู้จ่ายเงินที่เพิ่มขึ้นจากความสามารถในการป้องกันการเสียชีวิตของแม่และ เด็กในโรงพยาบาล |
| Other Abstract: | This study aimed to examine the cost of quality for maternal and newborn health in hospitals registered as providers with the provincial office of the National Health security officer under the Universal Coverage Sheme ( UC ) in Thailand. The Prevention – Appraisal- Failure Cost element Method ( PAF Model ) was applied,and analyses were done at the province level ,using data from the input claim database,the health budget administration database,the liability payment under section 41 of the National Health Security Act,population cencus of national statistic office, and the health resource database of the Ministry of public Health between the fiscal year 2550-2554, base on the year 2550 value, the prevention cost averaged at 297 bath per UC preson per year. The appraisal cost was 22 bath per Uc person per year and the failure cost was 71 bath per UC preson per year or 3,323 bath per admission. All three groups of the costs had increasing trends over the study period. The regression analysis using Generalized Estimating Equations ( GEE ) found positive linear association between the failure cost and the prevention cost and appraisal costs. The finding indicated that PAF Model might not be directly applied for evaluation quality management of the health service systems. This might be a result of how health service highly value failure, including limitation in estimating increasing failure costs which might be a result of successful prevention of inhospital maternal and newborn mortality from the payer perspective. |
