| Abstract: | การศึกษานี้เป็นการศึกษาเชิงวิเคราะห์โดยมีวัตถุประสงค์เพื่อศึกษาระดับความรู้ความเข้าใจของผู้ป่วยโรคไต เรื้อรังในเรื่องสภาวะความเจ็บป่วย การบริโภคอาหาร และการใช้ผลิตภัณฑ์เสริมอาหาร กลุ่มตัวอย่างเป็นผู้ป่วยโรคไต เรื้อรังที่เข้ารับการรักษา ณ โรงพยาบาลจุฬาลงกรณ์ ในช่วงเดือนสิงหาคมถึงเดือนพฤศจิกายน พ.ศ. 2558 โดยใช้แบบ ประเมินความรู้ร่วมกับการสัมภาษณ์ มีผู้เข้าร้วมวิจัยทั้งหมด 200 คน เป็นชาย 101 คน (ร้อยละ 50.5) และหญิง 99 คน (ร้อยละ 49.5) กลุ่มตัวอย่างส่วนใหญ่อายุมากกว่า 60 ปี (ร้อยละ 45.5) โดยเป็นผู้ป่วยที่ยังไม่ได้รับการบำบัด 4 ทดแทนไต 105 คน (ร้อยละ 52.5) ฟอกเลือด 60 คน (ร้อยละ 30.0) ปลูกถ่ายไต 25 คน (ร้อยละ 12.5) และล้างไต ทางช่องท้อง 10 คน (ร้อยละ 5.0) ผลการศึกษาพบว่าคะแนนเฉลี่ยความรู้ความเข้าใจในการดูแลตนเอง คือ 83.34 8.89 คะแนน โดยกลุ่มที่มีความรู้ระดับสูงมากที่สุด คือ กลุ่มที่ได้รับการปลูกถ่ายไต (ร้อยละ 80.0) รองลงมา คือกลุ่มที่ ได้รับการฟอกเลือด กลุ่มที่ไม่ได้รับการบำบัดทดแทนไต และกลุ่มที่ล้างไตทางช่องท้อง (ร้อยละ 75.0 ร้อยละ 56.2 และร้อยละ 40.0 ตามลำดับ) เมื่อพิจารณาความสัมพันธ์ระหว่างคะแนนความรู้ความเข้าใจในการดูแลตนเองกับตัว แปรต่างๆ พบว่าคะแนนความรู้มีความสัมพันธ์กับระยะเวลาที่ป่วยเป็นโรคไตเรื้อรัง และรูปแบบการบำบัดทดแทนไต (p = 0.042 และ p = 0.003 ตามลำดับ) การศึกษานี้พบว่ากลุ่มตัวอย่างส่วนใหญ่มีคะแนนความรู้สูง ทั้งนี้อาจ เนื่องจากผู้วิจัยได้ทำการศึกษาในโรงพยาบาลเพียงแห่งเดียว จึงทำให้ผู้ป่วยส่วนใหญ่ได้รับความรู้ในการดูแลตนเอง จากบุคลากรทางการแพทย์ในรูปแบบเดียวกัน อย่างไรก็ตามยังมีผู้ป่วยบางรายมีความเข้าใจไม่ถูกต้องเกี่ยวกับสภาวะ โรค การบริโภคอาหาร และการใช้ผลิตภัณฑ์เสริมอาหาร ข้อมูลที่ได้จากการศึกษานี้สามารถนำไปประยุกต์ใช้ในการ วางแผนหรือจัดโปรแกรมการให้ความรู้ทางด้านโภชนาการแก่ผู้ป่วยโรคไตเรื้อรัง เพื่อให้ผู้ป่วยสามารถดูแลตนเองได้ อย่างเหมาะสม ลดการดำเนินไปของโรคและภาวะแทรกซ้อนต่างๆ ได้ |
| Other Abstract: | This analytical study was designed to determine the knowledge in chronic kidney disease (CKD), food intake and dietary supplement consumption of CKD patients at King Chulalongkorn Memorial Hospital during August to November 2015 using a questionnaire and an interview. Two hundred participants were enrolled in the study, which were 101 males (50.5%) and 99 females (49.5%). Most of them were older than 60 years old (45.5%), There were 105 participants (52.5%) who have never received a renal replacement therapy (RRT), 60 (30.0%) received hemodialysis (HD), 25 (12.5%) received kidney transplantation (KT), and 10 (5.0%) received peritoneal dialysis (PD). The result showed that the average score in self-care knowledge was 83.34 ±8.89. Eighty percent of the participants with KT, 75.0% with HD, 56.2% at pre-dialysis stage, and 40.0% with PD received high score level. The associations of knowledge score with the duration of diagnosis of CKD and the types of RRT were found (p = 0.042 and p = 0.003 respectively). This study found that most of the participants received high self-care knowledge scores. This might be because the data collection was performed only in one hospital, which most patients received the same pattern of self-care knowledge from health professionals. However, some patients still misunderstood about the disease, food intake, and dietary supplement use. The information obtained from this study can be applied to plan a knowledge providing program for CKD patients so that the patients can have appropriate self-care, which can prevent the complications and delay the progression of the disease. |
