dc.description.abstract	วัตถุประสงค์ของการวิจัยคือ เพื่อศึกษาทัศนคติของแพทย์ ข้าราชการและผู้มีสิทธิต่อระบบเบิกจ่ายตรงค่ารักษาพยาบาลรวมถึงวิเคราะห์งบประมาณด้านการรักษาพยาบาลทั้งก่อนและหลังการใช้ระบบเบิกจ่ายตรงภายใต้หนังสือเวียนของกรมบัญชีกลาง พร้อมทั้งหาค่าคาดหวังค่ารักษาพยาบาลทั้งคนไข้นอกและคนไข้ใน เพื่อคำนวณการร่วมจ่ายค่ารักษาพยาบาลที่อัตราต่างๆ โดยใช้วิธีการเก็บข้อมูลด้วยแบบสอบถามจากกลุ่มตัวอย่างแพทย์และข้าราชการ และข้อมูลการเบิกจ่ายค่ารักษาพยาบาลสิทธิสวัสดิข้าราชการของโรงพยาบาลโรงเรียนนายร้อยพระจุลจอมเกล้าของผู้ป่วยนอกและผู้ป่วยใน ผลการศึกษาสรุปได้ว่าระบบเบิกจ่ายตรงสวัสดิการรักษาพยาบาลข้าราชการ ที่ได้นำมาใช้ตั้งแต่ปีพ.ศ.2549 จนกระทั่งถึงปัจจุบัน พบว่ารัฐบาลต้องรับภาระค่ารักษาพยาบาลที่เพิ่มสูงขึ้นในแต่ละปี โดยในปีพ.ศ.2555 รัฐบาลมีค่าใช้จ่ายสูงถึง 61,314 ล้านบาท ดังนั้น รัฐบาลจึงมีแนวคิดในการควบคุมค่าใช้จ่ายโดยการจำกัดการใช้ยานอกบัญชียาหลักแห่งชาติที่มีราคาแพง 9 ประเภท รวมทั้งมีการเสนอแนวคิดการร่วมจ่ายเงินค่ารักษาพยาบาล ซึ่งการศึกษาครั้งนี้ได้ผลการศึกษาจากการทำแบบสอบถามแพทย์ผู้ปฏิบัติงาน พบว่าแพทย์ร้อยละ 70.97 เห็นด้วยกับการร่วมจ่ายค่ารักษาพยาบาล ในขณะที่ข้าราชการที่เห็นด้วยกับการร่วมจ่ายมีเพียงร้อยละ 43.33 ทั้งนี้เมื่อใช้ฐานข้อมูลการเบิกจ่ายค่ารักษาพยาบาลสิทธิสวัสดิการข้าราชการของโรงพยาบาลโรงเรียนนายร้อยพระจุลจอมเกล้า ข้อมูลผู้ป่วยนอกปีพ.ศ.2552-53 และปีพ.ศ.2556-58 ข้อมูลผู้ป่วยในปีงบประมาณ 2553-54 และ 2557 ผลจากการวิเคราะห์พบว่า ข้อมูลมีการแจกแจงแบบ lognormal ที่มีค่าคาดหวังค่าใช้จ่ายเป็นตัวกลางของข้อมูลที่เหมาะสม พบว่าค่าคาดหวังค่ารักษาพยาบาลผู้ป่วยนอกปีพ.ศ.2557 คิดเป็น 1,476.14 บาท หากกำหนดให้มีการร่วมจ่ายค่ารักษาพยาบาลร้อยละ 20 จะทำให้ข้าราชการต้องร่วมจ่ายค่ารักษาพยาบาลคิดเป็น 295.24 บาทต่อครั้งของการใช้บริการ ในขณะที่ค่าคาดหวังค่ารักษาพยาบาลผู้ป่วยในปีงบประมาณ 2557 คิดเป็น 11,136.29 บาท หากกำหนดให้มีการร่วมจ่ายค่ารักษาพยาบาลร้อยละ 20 จะทำให้ข้าราชการต้องร่วมจ่ายค่ารักษาพยาบาล 2,227.12 บาทต่อครั้งของการนอนโรงพยาบาล	en_US
dc.description.abstractalternative	The objective of this study are to analyze the attitude of doctors and civil servants towards The Civil Servant Medical Benefit Scheme (CSMBS), to analyze the budget of this scheme before and after 2006 under the policy of Comptroller General’s Department, Ministry of Finance. Computing the expected value of medical cost in case of OPD and IPD per visit including the calculation of the co-payment rates 5% , 10% , 15% and 20%.The research methodology using both of quantitiative and qualitative research such as questionnaire, the reimbursement data for OPD and IPD from Chulachomklao Royal Military Academy Hospital(CRMA). The CSMBS launched for 2006 and after that the expenditure of medical care was increasing every year. In year 2012, the expenditure was 61.314 billion baht. So the government had a policy to restrict usage of 9 expensive non-essential drugs and initiated the “co-payment system”. The results of this study found that the attitude of the doctors towards CSMBS showed that 70.97% said that the civil servants should co-pay for the medical cost. Meanwhile 43.33% of the attitude of civil servants they agreed with co-pay. The probability distribution function of reimbursement data was lognormal distribution. The expected value of medical cost per visit in case of OPD in 2009-10 and 2013-15 and IPD in 2010-11 and 2014 from CRMA hospital were estimated and the expected value of OPD in 2014 was 1,746.14 baht. For the expected value of IPD in 2014 was 11,136.29 baht, If co-payment system is introduced with the rate 20%, the patients had to pay for 295.24 and 2,227.12 baht for each visit respectively.	en_US
