dc.description.abstract	ความเป็นมาของปัญหา พยาบาลที่ปฏิบัติงานในโรงพยาบาลจิตเวชเป็นกลุ่มที่ให้บริการด้านสุขภาพจิตและดูแลผู้ป่วยจิตเวช ต้องเผชิญกับปัญหาสุขภาพจิตที่เพิ่มขึ้น ความคาดหวังต่อคุณภาพบริการที่สูงขึ้น ตลอดจนการปฏิรูปภาครัฐที่อาจส่งผลกระทบต่อสุขภาพกายและจิตใจของพยาบาลซึ่งต้องรับภาระที่หนักขึ้น ดังนั้นการดูแลสุขภาพทั้งร่างกายและจิตใจของพยาบาลจึงเป็นสิ่งที่สำกัญยิ่ง วัตถุประสงค์ เพื่อศึกษาภาวะสุขภาพทั่วไป ภาวะซึมเศร้าและปัจจัยที่เกี่ยวข้องกับภาวะสุขภาพและภาวะซึมเศร้าของพยาบาลที่ปฏิบัติงานในโรงพยาบาลจิตเวช สังกัดกรมสุขภาพจิต ในเขตภาคกลาง รูปแบบการวิจัย เป็นการวิจัยเชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่ง ประชากรที่ศึกษา พยาบาลที่ปฏิบัติงานในโรงพยาบาลจิตเวช 4 แห่ง จำนวน 589 คน เครื่องมือวิจัย แบบประเมินภาวะสุขภาพทั่วไป (SF-36) แบบสอบถามปัญหาสุขภาพเพื่อตรวจวัดหาโรคซึมเศร้า (Health-Related Self-Reported (HRSR) Scale และแบบสอบถามเกี่ยวกับปัจจัยต่าง ๆ ชนิดตอบเองที่ผ่านการทดสอบแล้ว การรวบรวมข้อมูล เดือนธันวาคม2546-มีนาคม 2547 ได้รับแบบสอบถามส่งกลับคืนจำนวน 506 คน (85.9%) สถิติที่ใช้ Chi-square test ผลการวิจัย พยาบาลผู้ตอบกลับส่วนใหญ่เป็น เพศหญิง (87.5%) อายุระหว่าง 21-4รปี (71.8%) สถานภาพสมรสคู่ (61.5%) เป็นพยาบาลวิชาชีพ(68.0%) ปฏิบัติงานในคลินิก (70.8%) ผ่านการอบรมเฉพาะทางจิตเวช (70.8%) และประสบการณ์งานพยาบาลจิตเวชมากกว่า 10 ปี (64.4%) รายได้รวมมากกว่า 10,000 บาท (75%) ผลการสำรวจภาวะสุขภาพทั่วไปของพยาบาลที่ปฏิบัติงานในโรงพยาบาลจิตเวช พบว่า ภาวะสุขภาพทั่วไปอยู่ในระดับดีเยี่ยม, ดีและไม่ดี ร้อยละ 26.3, 24.7, 49.0 ตามลำดับ โดยมีปัจจัยในด้าน เพศ, อายุ, บทบาทความรับผิดชอบในงาน, การตรวจสุขภาพประจำปี, การใช้ยานอนหลับ, การออกกำลังกาย, การรับประทานอาหาร,โอกาสก้าวหน้าในงาน, ความขัดแย้งกับผู้บังกับบัญชา, สภาพแวดล้อมในการทำงาน, สถานที่อาศัย, บรรยากาศความสัมพันธ์ในครอบครัว, รายได้เทียบกับรายจ่าย, การหาเลี้ยงครอบครัวโดยลำพัง มีความสัมพันธ์กับภาวะสุขภาพทั่วไปอย่างมีนัยสำคัญทางสถิติ (p< 0.05) โดยมีความพึงพอใจในการปฏิบัติงาน 63% และระบุไม่สามารถทนต่อสภาพเศรษฐกิจในปัจจุบันได้ 12.1% ส่วนผลการสำรวจภาวะซึมเศร้า พบว่ามีอัตราความชุก 2.6% โดยมีภาวะเครียดและซึมเศร้าที่ควรได้รับการรักษา 1.0% มีภาวะซึมเศร้ารุนแรง 1.6% และจะพบภาวะซึมเศร้าสูงขึ้นในกลุ่มผู้ที่มีสุขภาพไม่ดีถึง 5.2% สรุป ผลการศึกษาแสดงทั้งสภาวะสุขภาพกายและสุขภาพจิตของพยาบาลในโรงพยาบาลจิตเวช ว่าสมควรได้รับการพัฒนาอย่างจริงจัง โดยต้องคำนึงถึงปัจจัยต่าง ๆ ที่มีผลกระทบกับภาวะสุขภาพ ผู้บริหารในทุกระดับขององค์กร ควรให้ความสำคัญและส่งเสริมคุณภาพชีวิตของพยาบาลที่ปฏิบัติงานในโรงพยาบาล เพื่อเป็นแบบอย่างการพัฒนาองค์กรอันจะส่งผลต่อคุณภาพงานพยาบาลและประสิทธิภาพขององค์กรต่อไป
dc.description.abstractalternative	Background: Nurses in psychiatric hospitals faced higher workload from the increase members mental disorder. Higher expectation in the best quality of nursing care and also government and health done reform. Thus physical and mental health status of nurses are very important and necessary to determine. Objective: To study the general physical , mental health and the prevalence of depression and related factors via self-reported among nurses in various Psychiatric Hospitals under the Mental Health Department in the central region of Thailand. Research Design: Cross-Sectional Descriptive Study Participants: The five hundred and six in 4 Psychiatric Hospitals. Data Collection: During December 2003 - March 2004. The returned rate was 506 out of 589 (88.15%.). The instruments: Short Form 36 Health Survey Thai version questionnaire (SF-36) and Health-Related Self-Reported (HRSR) Scale Statistical Method : Chi-square test Result: The result showed that the majority of nurses were female. (87.5%), the age 21-45 years-old (71.8%), married (61.5%), professional nurses (68.0%), worked in clinical area (70.8%), Special psychiatric nurse trained (70.8%), experienced more than 10 years(64.4%), income above 10,000 baht (75%). General physical health of nurses were very good, good and poor health (26.3, 24.7, 49.0%). The determinants to general physical health status of nurses were sex, age, status in job, health examination, disease, substance (drug), exercise, eating, cheerfulness, to go forward in job, to oppose with boss, environment at work place, lodgings, relationship in family, revenue and expense, working with lonely were statistically significant differences at the 0.05 level. Satisfaction rate in work system was 63%. Prevalence rate of depression was 2.6% (depression and higher need to treated 1.0% and severe depression 1.6%). Poor health group was demonstrated higher prevalence of depression rate (5.2%). Conclusion: The results showed physical and mental health of nurses in Psychiatric Hospitals should be improving and linkage with related determinants found. Quality of life of nurses are crucial and need highly attention of the leaders at all level. Role model in healthy nurse workforces of Psychiatric hospitals would lead to the effectiveness of organization.
