| Abstract: | การวิจัยเชิงอนาคตแบบ EDFR นี้มีวัตถุประสงค์ เพื่อศึกษาสมรรถนะของพยาบาลระดับปฏิบัติการ ตามแนวคิดบันไดอาชีพ โรงพยาบาลตติยภูมิ ผู้ให้ข้อมูล คือ ผู้เชี่ยวชาญ คัดเลือกตามคุณสมบัติที่กำหนดไว้ จากความเชี่ยวชาญ 4 กลุ่ม คือ ผู้เชี่ยวชาญการพยาบาลที่เป็นกรรมการของสภาการพยาบาล 3 คน การบริหารการพยาบาล 6 คน การพยาบาลศึกษา 6 คน และการปฏิบัติการพยาบาล 6 คน รวมผู้เชี่ยวชาญทั้งหมดจำนวน 21 ท่าน วิธีดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ ขั้นตอนที่ 1 การสัมภาษณ์เกี่ยวกับสมรรถนะของพยาบาลระดับปฏิบัติการ ขั้นตอนที่ 2 นำข้อมูลที่ได้มาวิเคราะห์เนื้อหา แล้วสร้างแบบสอบถามเพื่อให้ผู้เชี่ยวชาญแสดงความคิดเห็นระดับความจำเป็นของข้อคำถามแต่ละข้อ ขั้นตอนที่ 3 นำข้อมูลที่ได้คำนวณค่ามัธยฐาน ค่าพิสัยระหว่าง ควอไทล์ และส่งแบบสอบถามกลับไปให้ผู้เชี่ยวชาญสรุปสมรรถนะที่จำเป็น เพื่อเป็นการยืนยันอีกครั้งหนึ่ง หลังจากนั้นนำข้อมูลที่ได้มาคำนวณหาค่ามัธยฐาน ค่าพิสัยระหว่างควอไทล์ อีกครั้ง เพื่อสรุปผลการวิจัย ผลการวิจัยพบว่า 1) พยาบาลระดับปฏิบัติการ ควรมี 4 ระดับ คือ 1.5) พยาบาลระดับ 1 ควรมีประสบการณ์ 0-2 ปี 1.6) พยาบาลระดับ 2 ควรมีประสบการณ์ 2-5 ปี 1.7) พยาบาลระดับ 3 ควรมีประสบการณ์ 5-10 ปี 1.8) พยาบาลระดับ 4 ควรมีประสบการณ์ 10-15 ปี 2) สมรรถนะที่จำเป็นของพยาบาลมี 7 ด้าน คือ 2.8) ด้านการดูแลผู้ป่วย 2.9) ด้านการพัฒนาตนเองเกี่ยวกับ ความรู้ วิชาการ วิจัย และการใช้เทคโนโลยี 2.10) ด้านภาวะผู้นำและการบริหารจัดการ 2.11) ด้านมนุษยสัมพันธ์และการติดต่อสื่อสาร 2.12) ด้านการตัดสินใจและการแก้ปัญหา 2.13) ด้านการพัฒนาและประกันคุณภาพ 2.14) ด้านจริยธรรมและจรรยาบรรณวิชาชีพ พยาบาลระดับปฏิบัติการทั้ง 4 ระดับ จะมีสมรรถนะแตกต่างกันในแต่ละด้านโดย สมรรถนะที่จำเป็นของพยาบาลระดับ 1 ระดับ 2 ระดับ 3 และระดับ 4 มีจำนวน 20 20 23 และ 20 ข้อตามลำดับ |
| Other Abstract: | The purpose of this Ethnographic Delphi Future Research (EDFR) was to explore core competencies of clinical nurses, tertiary hospitals. Participants consisted of 21 experts: 3 committee members from the Thailand Nursing Council, 6 experts in nursing administration, 6 experts in nursing education, and 6 experts in clinical nursing. The EDFR consisted of 3 steps. Step 1: All experts were asked to describe core competencies of clinical nurse working in tertiary hospitals. Step 2: Study data were analyzed using content analysis and then a questionnaire was developed. All items in the questionnaire were ranked level of competency by a panel of experts. Step 3: Items were analyzed using mean and interqurtile range and then a new questionnaire was developed and sent to experts for confirmation of item ranking. Ranked items were analyzed using mean and interquatile range and used summarized competencies of clinical nurses. The major findings were as follows: 1) There were 4 levels of clinical nurses including: 1.1)Clinical nurse 1 with working experience 0-2 years 1.2) Clinical nurse 2 with working experience 2-5 years 1.3) Clinical nurse 3 with working experience 5-10 years 1.4) Clinical nurse 4 with working experience 10-15 years 2) There were 7 competencies of clinical nurses including: 2.1) Nursing care 2.2) Self-development involving knowledge of nursing, research, and use of technology 2.3) Leadership and management 2.4) Human relationship and communication 2.5) Decision making and problem solving 2.6) Quality development and quality assurance 2.7) Nursing ethics Clinical nurses in each level had the core competencies but different in details of each competency. There were 20, 20, 23, and 20 items of core competencies of clinical nurses at levels 1, 2, 3, and 4 respectively. |
