| Abstract: | การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษารูปแบบความสัมพันธ์เชิงสาเหตุระหว่างปัจจัยที่มีอิทธิพลต่อการพัฒนาคุณภาพโรงพยาบาลชุมชนในจังหวัดพระนครศรีอยุธยา กลุ่มตัวอย่างที่ใช้ในงานวิจัยคือ บุคลากรของโรงพยาบาลชุมชนในจังหวัดพระนครศรีอยุธยาที่ผ่านการรับรองคุณภาพโรงพยาบาลตั้งแต่ 1 ครั้งขึ้นไป 7 แห่ง จำนวน 381 คน และกลุ่มผู้ให้ข้อมูลเชิงลึกจำนวน 7 คน เครื่องมือวิจัยประกอบด้วย (1) แบบสอบถามจำนวน 4 ตอนซึ่งมีค่าความเที่ยงทั้งฉบับเท่ากับ 0.98 และค่าความตรงเชิงเนื้อหาระหว่าง 0.60-1.00 และ (2) แบบสัมภาษณ์ ตัวแปรแฝงที่ศึกษามี 7 ตัวแปร ตัวแปรสังเกตได้ 24 ตัวแปร ผู้วิจัยวิเคราะห์ข้อมูลเชิงปริมาณโดยการหาค่าความถี่ ค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ค่าสัมประสิทธิ์สหสัมพันธ์แบบเพียร์สัน และวิเคราะห์เส้นทางอิทธิพล ในส่วนของข้อมูลที่ได้จากการสัมภาษณ์ ผู้วิจัยได้ใช้มาเป็นหลักฐานสนับสนุนผลการวิเคราะห์ทางสถิติ ข้อค้นพบสำคัญพบว่า (1) ตัวแปรการนำส่งอิทธิพลต่อตัวแปรการพัฒนาคุณภาพโรงพยาบาลโดยผ่านตัวแปรการวัดวิเคราะห์และจัดการความรู้ และตัวแปรการจัดการกระบวนการตามลำดับ โดยงานวิจัยนี้ได้ชี้ให้เห็นว่าตัวแปรต่างๆในรูปแบบที่ศึกษาสามารถร่วมกันอธิบายความแปรปรวนของอิทธิพลต่อการพัฒนาคุณภาพโรงพยาบาลชุมชนในจังหวัดพระนครศรีอยุธยาได้ร้อยละ 79.3 (2) ตัวแปรการจัดการกระบวนการส่งผลทางตรงต่อการพัฒนาคุณภาพโรงพยาบาลเพียงตัวแปรเดียว โดยที่ตัวแปรการมุ่งเน้นทรัพยากรบุคคลไม่ส่งผลต่อการพัฒนาคุณภาพ ทั้งนี้ผลการสัมภาษณ์เชิงลึกชี้ว่า โรงพยาบาลทั้ง 7 แห่งขาดแคลนบุคลากรทำให้ผู้ปฏิบัติงานต้องทำหน้าที่ที่มีใช่วิชาชีพตนด้วยซึ่งอาจส่งผลถึงผลลัพธ์การพัฒนาคุณภาพที่ไม่ดีนัก (3) รูปแบบความสัมพันธ์เชิงสาเหตุระหว่างปัจจัยที่ส่งผลต่อการพัฒนาคุณภาพโรงพยาบาลชุมชนในจังหวัดพระนครศรีอยุธยามีความสอดคล้องกลมกลืนกับข้อมูลเชิงประจักษ์ (chi-square = 209.68, df = 188, p = .133, GFI = .957, AGFI = .931 , RMSEA = .017) |
| Other Abstract: | This research aimed to study the causal factors influencing Hospital Accreditation of community hospitals in Ayutthaya Province. Data from 381 healthcare-related officials in 7 re-accredited community hospitals in Ayuthaya were collected; moreover, the 7 top officers were selected to provide in-depth information. The research instruments included (1) the questionnaire consisting of 4 sections with its reliability of 0.98 and the IOC value lying between 0.60-1.00 and (2) the interview construction questionnaire. The proposed model consisted of 7 latent variables and 24 observed variables. The quantitative analysis was done by frequency, percentage, mean, standard deviation, Pearson's product-moment correlation coefficient, and path analysis. In addition, in-depth information could explain the quantitative results. The key results could be summarized as follows: (1) Leadership affected Measurement Analysis and Knowledge Management which influenced Process Management. Process Management then created Hospital Accreditation. This research, moreover, indicated that the variables in this model accounted for 79.3% of the variance. (2) Process Management directly affected Hospital Accreditation while Human Resource Focus did not affect Hospital Accreditation because hospitals lacked healthcare officers which could not bring about the good results. (3) The casual relationship model of factors influencing Hospital Accreditation of community hospitals in Ayutthaya Province was fitted with the empirical data (chi-square = 209.68, df = 188, p = .133, GFI = .957, AGFI = .931 , RMSEA = .017). |
